Predictive Models of Clinical Improvement in Rituximab-Treated Myositis Patients Using Clinical Features, Autoantibodies, and Biomarkers

Background: Response to rituximab so far is unpredictable in patients with refractory myositis. Predictive models of clinical improvement are developed using clinical, laboratory, and gene expression/cytokine/chemokine variables in rituximab-treated refractory myositis patients. Methods: We analyzed data for 200 myositis patients (76 with adult polymyositis (PM), 76 with adult dermatomyositis (DM), and 48 with juvenile (DM)) in the rituximab in myositis trial. Clinical improvement is defined as the change from baseline to 24 weeks in Physician Global Visual Analog Scale (VAS). We analyze the association of baseline variables with improvements: demographics, myositis subtype, clinical and laboratory parameters, autoantibody status, and interferon (IFN)- regulated chemokines. Multivariable linear regression models are developed by using stepwise variable selection methods. Results: A “base” multivariable model to predict improvement with clinical and laboratory variablesonly is built with modest predictive ability (adjusted R2 = 0.21). This model includes two significant factors at baseline: Physician Global VAS and Muscle Disease Activity VAS. A “final” multivariable model to predict improvement including non-standard laboratory measures is developed and demonstrated better predictive ability (adjusted R2 = 0.32). This model includes Physician Global VAS, IFN chemokine score and IL-2 levels. The “final” model explained 11% more variability than the “base” model. Conclusions: Changes in disease activity over time following treatment with rituximab in refractory myositis can be predicted. These models can be clinically useful to optimize treatment selection in myositis.

Introduction and Objectives: Soil-Transmitted-Helminthiasis (STH) is a public health problem in Cameroon. The control strategies currently in place, particularly chemoprevention, has shortcomings linked to the target population, which are school-age children. The objective was to determine the prevalence and the risk factors associated with geo-helminthiasis in children aged 0 to 3 years in a rural health district. Method: From December 2020 to May 2021, a descriptive and analytical cross-sectional study of 376 children between 6 and 36 months was carried out in the Akonolinga health district. This was a cluster sampling in 4 health areas. Stool samples were collected and analysed using the mini-FLOTAC method. The results expressed as the number of eggs per gram of stool. A questionnaire on socio-demographic and lifestyle data was administered to the parents. The Chi-squared test was used to measure the association between geo-helminth infection and the data collected. A multivariate analysis using logistic regression was performed (p 0.05). Results: The prevalence of STH was 19.4% (Ascaris lumbricoides: 16% and Trichuris trichiura: 8%). Risk factors were: consumption of contaminated water (AOR = 1.93 [1.03 - 3.6];p = 0.040), early contact of the child with the ground (before age of 4 months) (AOR = 4.9 [2.1 - 11.37];p .001), habit of walking barefoot (AOR = 2.91 [1.1 - 7.97];p = 0.038), and living in a habitat with unpaved ground (AOR = 7.4 [1.55 - 35.7];p = 0.012). Conclusion: The prevalence of STHs in infants was high. Preventive chemotherapy should be extended to this age-group, and other measures intensified.

Introduction: Despite communication on Human immunodeficiency virus (HIV), close to 5.9 million people didn’t know they were living with HIV. One of the modes of transmission of HIV is vertical transmission. It remains the principal way of transmission of HIV for children below 15 years, with over 90% of them being infected. In Cameroon, the mother and child transmission of HIV is still a problem with 5.5% of infection of exposed infant at the early infant diagnosis in 2019. Awareness on prevention of mother to child transmission (PMTCT) is an important component for the fight against HIV. We therefore aimed at gauging the level of knowledge of a group of adult clients on HIV and PMTCT. Methodology: We conducted a cross sectional and analytical study at the Yaoundé Gyneco-Obstetric and Pediatric hospital for a period of one month. Were included, adults who visited the hospital for a free screening campaign on HIV. Data were collected using a questionnaire on their characteristics and knowledge of HIV and PMTCT. Data were entered using CSPro and analysed using IBM SPSS. Results: There was a gap in knowledge on HIV, considering satisfactory knowledge from 25 points and above, it was noted that 34.6% of our respondents had a non-satisfactory knowledge. For PMTCT, knowledge was average with the majority scoring 10 points on 18. Considering satisfactory knowledge on PMTCT was from 10 and above, 35.9% of our respondents had a non-satisfactory knowledge. Female sex was associated with satisfactory knowledge for both HIV and PMTCT. Conclusion: Concerning PMTCT, the knowledge of the mode of transmission, Antiretroviral (ARV) drugs for prevention to the babies or feedings recommendations on exposed HIV infants were poor. There is a gap in favor of women regarding knowledge between HIV and PMTCT. Therefore, men are highly encouraged to take part to health promotion activities on HIV and PMTCT.

We report a case of a unilateral conjunctival neoformation in a 44-year-old female melanoderma subject. It was a conjunctival squamous cell carcinoma (CSC). The risk factor was exposure to sunlight and dust. No HIV or HPV infection was found. The treatment consisted of simple removal of the tumour with a healthy margin of 3 to 4 mm. After a postoperative follow-up of 18 months, there was no recurrence. As CSC is the most common conjunctival malignancy and given the rate of recurrence with the possibility of associated local invasion, early diagnosis with long-term follow-up is necessary.

本文回顾了在多年冻土区改善路基稳定性方法的效果和成本,是基于文献研究成果的综述。在冻土地基进行路堤加载或者其吸热特性降低时都会引起多年冻土地基的不稳定。研究了各种工程策略来减轻这种影响,从基本原理,效果,成本等总结了四种类型的方法,即控制路基融化、冷却路基、隔离路基和降低路基填充重量。文献表明,控制路基融化、隔离路基或降低路基填充重量不能逆转冻土地基的长期退化。通过路堤的空气对流、通风管道、热虹吸管、热量消耗或者综合这些措施对降低路堤温度、提高稳定性非常有效。地理位置、气候条件和施工材料等因素都会引起成本不同。资料显示进行正常的维护比进行路基降温更廉价,但某些环境下维护并不可行。

Specimens of the forest species such as Pentaclethra macrophylla, Petersianthus macrocarpus, Pycnanthus angolensis and Terminalia superba have been sampled from LUKI Biosphere reserve in the Democratic Republic of the Congo in order to determine their wood density with the perspective to decarbonisation. These parameters have been found out experimentally utilizing a drying technique in an oven including techniques of immersion in an Erlenmeyer full of water. The corresponding results indicated that the four species wood density is respectively 0.85, 0.80, 0.77 and 0.51. These preliminary results will be useful in our ongoing project on carbon dioxide absorption capacity of Congo rainforest tree species.
