Small bowel adenocarcinoma in neoterminal ileum in setting of stricturing Crohn’s disease:A case report and review of literature

BACKGROUND Small bowel adenocarcinomas(SBA) are rare malignancies with exceedingly low survival rates, with different presentation in Crohn’s disease(CD). CD-induced SBA poses diagnostic challenges given overlapping presentation with stricturing CD and lack of diagnostics for early detection. Moreover, guidance is lacking on the impact of recently approved therapeutics in CD on SBA management. Here, we aim to highlight the future of CD-induced SBA management and discuss the potential merit of balloon enteroscopy and genetic testing for earlier *** SUMMARY We report the case of a 60-year-old female with longstanding Crohn’s ileitis, presenting with acute obstructive symptoms attributed to stricturing phenotype. Her obstructive symptoms were refractory to intravenous(Ⅳ) steroids, with further investigation via computed tomography enterography not providing additional diagnostic yield. Ultimately, surgical resection revealed SBA in the neoterminal ileum, with oncologic therapy plan created. However, this therapy plan could not be initiated due to continued obstructive symptoms attributed to active CD. Ultimately, infused biologic therapy was initiated, but her obstructive symptoms continued to remain dependent on Ⅳ corticosteroids. Review of diagnostics by a multidisciplinary care team suggested metastatic disease in the peritoneum, lending to a shift in the goals of care to *** With the diagnostic and therapeutic challenges of concurrent SBA and CD, multidisciplinary care and algorithmic management can optimize outcomes.

Tunable construction of transition metal-coordinated helicene cages

We report a facile and tailored method to prepare globally twisted chiral molecular cages through tunable coordination of bis-bipyridine-terminated helicene ligands to a series of transition metals including Fe(Ⅱ), Co(Ⅱ), Ni(Ⅱ) and Zn(Ⅱ). This system shows an efficient remote transfer of stereogenecity from the helicene core to the bipyridine-metal coordination sites and subsequently the entire cages. While the Fe(Ⅱ), Co(Ⅱ) and Ni(Ⅱ)-derived M_(2)L_(3)(M for metal and L for ligand) cages exhibit quasi-reversible redox features, the Zn(Ⅱ) analogues reveal prominent yellow circularly polarized luminescence. Interestingly,with the addition of Na_(2)SO_(4), the Zn_(2)L_(3)cages reassemble into sextuple-stranded Zn_(6)L_(6)(SO_(4))_(4)cages in which three Zn_(2)L_(2) units are bound together by four sulfates and further coalesced by offset inter-ligandπ-π interactions.

Climbing-Specific Fitness Profiles and Determinants of Performance in Youth Rock Climbers

With the recent decision by the International Olympic Committee to include rock climbing in the 2020 Olympic Games, climbers and coaches are seeking information to enhance training methods and improve performance. The purpose of this study was two-fold:(1) to evaluate climbing-specific fitness and establish percentile rankings among youth climbers;and (2) determine the relationship between fitness and climbing performance. Anthropometrics, fitness, and performance on three indoor bouldering problems ranging in difficulty from V0-V8 were assessed in 64 youth climbers (35 girls, 29 boys) aged 7-17 from the United States. Data are reported by age groups (7-11;12-17 y) and gender. Percentile rankings of fitness scores were computed for girls and boys separately. Analysis of variance was used to compare fitness by age groups and by gender. Regression analysis evaluated the association between climbing performance and fitness. Fitness scores were generally higher among boys than girls, and older vs. younger climbers. Multivariable linear regression revealed that, after adjusting for age, gender, and anthropometrics, fitness variables explained 49% of the variance in performance. Climbing-specific fitness measures previously established on adults are associated with bouldering performance in youth climbers, and therefore may be useful for monitoring progress in training.
