Difficult Conversations and Painful Decisions: When Should Patients with Progressive Cancer Stop Chemotherapy?

Introduction: The decision to stop anti-cancer treatment is fraught with many challenges for the oncologist, the patient, and their caregivers. This review examines the special considerations surrounding the decision to cease chemotherapy in terminally ill cancer patient. Methods: A comprehensive literature search was conducted to find relevant publications on chemotherapy cessation. A total of 2700 records were retrieved and 141 were identified as eligible for inclusion in this review. Results: Palliative chemotherapy does not achieve the goal of tumor-related symptom reduction for patients who have experienced progressive disease with more than two prior lines of chemotherapy. ECOG performance status is a crucial predictor of response to therapy and chemotherapy-related complications. Challenges to stopping chemotherapy at the end of life are multifactorial and are both patient and physician-driven. Racial, ethnic, and income-based disparities are seen in the timing and quality of end-of-life conversations offered by physicians to their patients. Conclusions: The decision to cease chemotherapy is one that should be approached with careful consideration and accurate information. Clear communication, compassion and empathy are important components to the therapeutic relationship. Early involvement of palliative care and clear conversations about prognosis and the expected utility of further chemotherapy is essential to conduct the best possible care for cancer patients at the end of life.

Rethinking screening for breast cancer and prostate cancer.

For a screening test to be optimal, once thescreening test is implemented, an initial increasein cancer incidence is expected and then a returnto prescreening rates should occur. Over time, thetest should result in an increase in incidences inlocalized cancers but a decrease in incidences inregional cancers. However, as this study shows,use of mammography for breast cancer screeningand prostate-specific antigen (PSA) evaluationfor prostate cancer screening represent less thanoptimal tests, and more intermediate case scenariosin which the incidences of both cancers haveincreased substantially while the incidences ofregional cancers have only slightly declined overtime.

Plasma microRNA profiles distinguish lethal injury in acetaminophen toxicity: A research study

AIM: To investigate plasma microRNA (miRNA) profiles indicative of hepatotoxicity in the setting of lethal acetaminophen (APAP) toxicity in mice. METHODS: Using plasma from APAP poisoned mice, either lethally (500 mg/kg) or sublethally (150 mg/kg) dosed, we screened commercially available murine microRNA libraries (SABiosciences, Qiagen Sciences, MD) to evaluate for unique miRNA profiles between these two dosing parameters. RESULTS: We distinguished numerous, unique plasma miRNAs both up- and downregulated in lethally compared to sublethally dosed mice. Of note, many of the greatest up- and downregulated miRNAs, namely 574-5p, 466g, 466f-3p, 375, 29c, and 148a, have been shown to be associated with asthma in prior studies. Interestingly, a relationship between APAP and asthma has been previously well described in the literature, with an as yet unknown mechanism of pathology. There was a statistically significant increase in alanine aminotransferase levels in the lethal compared to sublethal APAP dosing groups at the 12 h time point (P < 0.001). There was 90% mortality in the lethally compared to sublethally dosed mice at the 48 h time point (P = 0.011). CONCLUSION: We identified unique plasma miRNAs both up- and downregulated in APAP poisoning which are correlated to asthma development.

以哈萨克斯坦北部雨养耕作区为研究靶区,基于春小麦产量统计数据和遥感光谱指数,开展了春小麦估产最优预测时期及植被指数分析,采用回归分析、随机森林、支持向量机及双向循环神经网络模型估算春小麦产量,并对比分析了不同模型的模拟精度。结果表明:北哈萨克斯坦州、阿克莫拉州和库斯塔纳州2007-2016年春小麦估产的最佳预测时期为6月26日-8月5日,该时期是春小麦产量形成的关键时期。北哈萨克斯坦州春小麦估产最优植被指数为7月12日的绿度叶绿素指数(Green chlorophyll index,CI_(green)),阿克莫拉州春小麦估产最优植被指数为8月5日的绿度动态宽波段指数(Green wide dynamic range vegetation index,WDRVI_(green)),库斯塔纳州春小麦最优估产植被指数为7月12日的WDRVI_(green)。对比分析4种模型模拟春小麦产量的精度,在样本点较少的情况下,双向循环神经网络模型相比其他模型在估算哈萨克斯坦北部三州春小麦产量上精度较高;春小麦产量与植被净初级生产力NPP相关性分析结果显示,北哈萨克斯坦州、阿克莫拉州和库斯塔纳州决定系数R2在0.50以上面积占比分别为44%、94%和77%,表明上述估产模型可应用于哈萨克斯坦北部三州春小麦估产,尤其是阿克莫拉州和库斯塔纳州。

Relatively little is known about fire regimes in grassland and cropland in Central *** this study,eleven variables of fire regimes were measured from 2001 to 2019 by utilizing the burned area and active fire product,which was obtained and processed from the GEE(Google Earth Engine)platform,to describe the incidence,inter-annual variability,peak month and size of fire in four land cover types(forest,grassland,cropland and bare land).Then all variables were clustered to define clusters of fire regimes with unique fire attributes using the K-means *** showed that Kazakhstan(KAZ)was the most affected by fire in Central *** regimes in cropland in KAZ had the frequent,large and intense characters,which covered large burned areas and had a long *** in grassland mainly occurred in central KAZ and had the small scale and high-intensity characters with different quarterly *** in forest were mainly distributed in northern KAZ and eastern *** fires in grassland underwent a shift from more to less frequent from 2001 to 2019 in Central Asia,vigilance is needed because most fires in grassland occur suddenly and cause harm to humans and livestock.

Novel UBE2B-associated polymorphisms in an azoospermic / oligozoospermic population

Aim： To assess whether abnormalities exist in the UBE2B gene in a population of infertile human males, and to establish biologic plausibility of any discovered mutations. Methods： We carried out polymerase chain reaction （PCR） amplification and sequence analysis of the 5＇-untranslated region and six exons of the UBE2B gene, including flanking intronic regions, in a group of fertile and infertile men. Following the identification of a putative promoter region that contained single or dual triplet deletions within a 10-CGG repeat island, we evaluated the binding affinity of these identified polymorphisms as compared to the wild-type sequence to transcription factor SP1 using a DNA-protein gel shift assay. Results： There was a novel exonic single nucleotide polymorphism （SNP） noted in exon 4 in 5% of infertile men. In silico 3D modeling of the altered protein showed an innocuous isoleucine for valine substitution. There were no mutations noted within any of the other exons. Three novel intronic SNPs were identified within the fertile group, and seven novel intronic SNPs identified in the infertile group. The DNA-protein gel shift assay noted that both single CGG deletion and double CGG deletion bands had approximately twice the binding affinity compared to the wild-type for SP1. The negative control confirmed no non-specific protein binding. Conclusion- By themselves, a single or double CGG deletion is unlikely to pose biologic significance. However, such deletions in this suspected promoter region are associated with increased binding affinity for SP1, and might represent one of several factors required for alteration of UBE2B gene expression.

近年来,利用政策管理多Agent和分布式系统受到越来越多的关注,政策的使用可以使得管理系统行为选择的规则与系统提供的功能相分离,由于政策指导的明确性,在多Agent系统中利用政策管理思想处理系统协调和安全问题。基于政策管理的应用在一些领域具有很大的吸引力,如安全领域。随着新技术的引进和互联网的应用,用户通过互联网使用服务,使得当今网络环境安全性受到更多的关注。这就需要基于政策的管理方法,使用政策用以支持受管系统行为的动态适应。基于政策的管理机制和自治的智能Agent,本文提出一个政策管理框架,该框架目的在于通过定义和存储政策以控制Agent的行为。为了表明我们方法的可行性,我们开发了一个工具,它允许相关Agent动态执行指定政策。最后,本文描述了一个作为实例研究的网上拍卖的多Agent系统,在系统中,拍卖条款和条件都可以用政策来描述,并用以管理和协调拍卖Agent。

Objective:To review the literature on long-term neurodevelopmental outcome after fetoscopic laser surgery for twin-twin transfusion syndrome(TTTS).Methods:A literature search in PubMed,Embase,Emcare,Web of Science,Cochrane library,and Academic Search Premier was *** criteria were studies between 2009 and 2019 in TTTS-survivors treated with fetoscopic laser surgery and followed-up after the neonatal period with cognitive developmental tests and neurologic *** criteria were non-English articles and reviews,case reports,letters,and ***:Nineteen articles were ***-term severe neurodevelopmental impairment(NDI)was reported by seven and ranged from 4.0%to 18.0%with a mean of 9.7%(95%confidence interval(CI):7.8-11.5).The prevalence of cerebral palsy ranged from 1.6%to 18.2%,with a mean of 5.1%(95%CI:4.1-6.2).The mean prevalence of minor impairment was 13.7%(95%CI:11.4-16.0).However,only 78.9%(15/19)studies used a validated neurodevelopmental *** studies lack uniform definitions of primary outcome,timing of follow-up,inclusion criteria,and methods,adequate comparison is ***:The prevalence of severe NDI and cerebral palsy after fetoscopic laser surgery for TTTS in the last decade remains stable around 9.7%and 5.1%,*** agreements on primary outcomes,methods,and follow-up are necessary to improve the knowledge of NDI in TTTS-survivors.

Kinetic Study of the Oxidative Degradation of Choiba Oil(Dipteryx oleifera Benth.)with Addition of Rosemary Extract(Rosmarinus officinalis L.)

Choibá (Dipteryx oleifera Benth.) is a promising source of edible oil with high nutritional quality and a significant content of oleic acid (52% - 54%). To promote Choibá as source of edible oil is necessary to ensure its stability along the time of production, distribution and storage. Loss of nutritional and organoleptic quality in lipids is mainly due to lipid peroxidation reactions. The aim of this research was to evaluate the oxidative stability of Choibá oil at 100°C ± 1°C with aeration (1150 mL air/min) supplemented with rosemary extract (Rosmarinus officinalis L.), at 1000 mg/L (RE1000) and 1500 mg/L (RE1500), and with BHT (200 mg/L) and from this results to evaluate the degradation kinetics and shelf-life of Choibá oil at 35°C, 45°C and 55°C without addition of antioxidants (Control) and with addition of best concentration of rosemary extract obtained from previous study. Progress in oil oxidation was measured through the extent of oxidation products: peroxide value (PV) and thiobarbituric acid reactive substances (TBARS). Results revealed that the addition of rosemary extract at 1500 mg/L significantly reduced de formation of hydroperoxides (PV), more than BHT. Through correlations between concentrations of antioxidant (including control without antioxidant) with peroxide values, the kinetics of degradation and shelf-life of Choibá oil with predictive models are evaluated in real time and accelerated (35°C, 45°C and 55°C) using the Arrhenius equation. In addition, the oxidation reactions of this oil follow a first order kinetic model for PV and zero order kinetic model for TBARS. The rate of formation of PV was dependent on the storage temperature, according to the Arrhenius equation with the activation energy of 4611.5071 J/mol for Control and 7409.5771 J/mol for RE1500 treatment. The result of TBARS didn’t adjust to Arrhenius model, thus measurement of malondialdehyde (MDA) wasn’t a useful parameter for shelf-life determination of Choibá oil.
