Objective: This study aimed to investigate the circadian typology of medical students at public universities from one of the northeastern states in Brazil, active in the morning and afternoon shifts, checking the compatibility between their study time, productivity, income and chronotype. Methods: To obtain the results, the assessment of the chronotype in 342 medical students of both sexes was considered. Each participant was submitted to the questionnaire developed by Horne and Ostberg, adapted by Cardinali, Colomberk and Rey. It’s composed of 9 questions, providing the general ranking of each individual. Based upon scored, the chronotypes were classified. Results: The sample was composed of 49.12% (168) male and 50.87% (174) female subjects, a similar gender distribution of enrolled students during data collection time. By applying the questionnaire, the prevalent rating among the medical students as moderately morning (29.82%) and intermediate (43.85%) has been relevant. The academic performance assessment in two morning-intermediate groups (8.16) had no significant distance from others considered afternoon active (8.20). Conclusion: The health sciences student, especially Medicine, faces constant adaptation of scheduling, in the attempt of fulfilling the requirements throughout the course, causing chronogram desynchronization effects and forced readaptation to each different school semester.

Green Fertilization Enhances the Photosynthetic Performance and the Growth of Leguminous Trees for Restoration Plantation in Central Amazon

The leguminous tree species Dipteryx odorata (Aubl.) Willd. and Inga edulis Mart. were grown in the field to reforest a degraded area. To investigate the photosynthetic responses and the initial growth of Amazonian forest species under fertilization, the study species were subjected to different treatments: T1 = unfertilized-control, T2 = chemical fertilization, T3 = green fertilization and T4 = green and chemical fertilization. D. odorata showed the highest absolute growth rates in height under treatments T4 (10.2 cm·month-1) and T2 (12.2 cm·month-1). I. edulis exhibited the best performance for this same parameter in T4 (23.0 cm·month-1). The highest photosynthesis values (Pn) for both D. odorata (13.1 μmol·m-2·s-1) and I. edulis (20.6 μmol·m-2·s-1) were found in T4. The two species also showed the highest performance index values (PIABS) under T4 (D. odorata = 2.9 and I. edulis = 5.2). Our results suggest that the combination of green and chemical fertilization was the most effective of the four treatments evaluated for improving the photosynthetic performance and the growth of both species during the initial establishment of the species in a degraded area.

The Systemic Arterial Hypertension (SAH) stands out among the chronic non-transmissible pathologies that impact the cause and/or aggravation of cardiovascular diseases (CVD) on a global level, as the disease is an underestimated disorder due to non-perceptive symptoms and associated with factors and risk markers of another CVD. Therefore, establishing the risk of progression and aggravation of the SAH, according the Framingham Risk Score (FRS), allows to reducing morbidity and improving preventative measures for DCVs. This observational and transversal study approaches the data collection of patient records at the Health Family Strategy of Senhor do Bonfim, BA, which established differences by descriptive and inferential statistical analysis (correlation and regression). The aspects of hypertension associated with risk factors for atherosclerosis were analyzed, determining the risk of developing cardiovascular events in 10 years by FRS. From 432 families, 746 patients were selected, of which 340 are hypertensive individuals (SAH = 45.57%) and 406 (NSAH = 54.42%) non-hypertensives. Among the SAH the majority (31.17%) was in the age range of 63 - 77, but, in both groups, women were in stronger number. There was greater prevalence in SAH for all the characteristics analyzed, smoking (13.20%), sedentary (29.41%) and cardiovascular accident (22.60%). The SAH group is more susceptive to the CVD progress in 10 years by FRS (P < 0.0001 ANOVA). In the NSAH group, there were significant associations among all the variables analyzed as was expected, without differences between the linear correlation and regression, indicating the physio-metabolic equilibrium of the factors and markers evaluated by FRS. Already in SAH group, despite the correlations have been significant too, the regression analysis revealed that only Total Cholesterol (P = 0.0086);LDL (P < 0.0001), Glucose (P < 0.0006) and Age (P < 0.0001) have significative association with FRS. The continuous assessment of child health services generates knowledge about the real needs of health services and actions in order to achieve better results and indicators. Thus, this study proposes to analyze child health indicators, based on the information systems of health services. This is a descriptive, ecological epidemiological study carried out using secondary data from the following information systems: Information System on Live Births and Mortality Information System, in a historical series of ten years (2005-2014) of the state of Rio Grande do Norte and its health micro regions. It was observed a decrease in infant, neonatal, preterm, late neonatal and postnatal mortality coefficients when compared to the first and last year of the observed period, as well as the concentration of these in micro-regions of health. There was also a positive association between the early neonatal component and infant death. In this perspective, it is important to evaluate the quality of prenatal care, childbirth, puerperium, the first week of integral health and follow-up of the child’s growth and development, as well as to relate the socio-demographic conditions that may be associated with infant morbimortality. Therefore, the importance of situational health assessment studies for the effectiveness of care provided is highlighted, identifying weaknesses and monitoring the effectiveness of strategies.

Use of Extra-Narrow-Diameter Implants in Reduced Alveolar Ridge:A Case Report

Background: Narrow-diameter implants (3.0 - 3.5 mm range) have been introduced for the replacement of teeth with insufficient bone structure and/or limited mesiodistal or interimplant spaces, and appear to offer clinical results similar to those obtained with implants of greater diameter. Studies using extra-narrow-diameter implants (2.8 mm) are scarce. Case Presentation: A 59-year-old male patient received two extra-narrow-diameter implants, 2.8 × 11 mm in the region between elements 11 and 14. Together, two 3.5 × 8.5 mm SYSTHEX®platform 4.1 implants were installed in the region of elements 15 and 16 to provide greater stability in the occlusion. Of four previous implants on the maxillary left side, one in the region between the elements 23 and 24 that was located in a very apical position and vestibularized was removed. The provisional was already installed on the elements 11, 21, and 22 with the metal cores already prepared and with the Globteck®implants in the region of the elements 23, 24, and 27. The functional and esthetic results were satisfactory. Conclusions: Insertion of extra-narrow-diameter implants of 2.8 mm in the maxillary anterior region is a reliable option in a patient with absence of elements 12 and 13, restoring masticatory function and aesthetics in the upper arch.

From the estimation of the mass balance model, which can also be classified as “black box” model, it is possible to infer the impact of management on the system considered. This study aimed to evaluate water pollution generated by wastewater from a polyculture system of tilapia and shrimp and discuss the management employee and their relation to the quality of the effluent released. It used a pond measuring 1500 m2, average depth 1.6 meters, where 12 cages of 1 m3 populated with tilapia juveniles were installed 33 days after the shrimps’ population. The tilapia juveniles were distributed in densities from 200 to 400 fish per cubic meter, reaching the density of 2.4 fish per square meter within the total pond area. Shrimp post-larvae were released outside the cages within the pond area in a density of 3.3 organisms per square meter. Total density considering fish and shrimps was of 5.7 organisms per square meter in the pond area. Water samples were taken weekly in affluent and effluent of the pond (January-August/2009). The mass balance model was calculated from the difference between the estimated load for the output and input of the pond. The average flow rate was 4.46 L/s. The average loads nitrogen was 0.072 Kg/day (affluent) and 0.179 Kg/day (effluent) and phosphorus 0.0136 Kg/day (affluent) and 0.031 Kg/day (effluent). The mass balance resulted in mean values of 0.11 ± 0.06 Kg/day for total nitrogen and 0.017 ± 0.010 Kg/day for total phosphorus indicating that the system exported nutrients. The use of Best Management Practices (BMP) likes better feed and water management as a way to minimize nutrient export.

Objective: To assess comprehensive care in the elderly population, as well as the quality of care in Primary Health Care. Methods: This is an exploratory descriptive study with a quantitative approach, conducted from July to December 2012, in the city of Santa Cruz, Rio Grande do Norte, Brazil. A sample of 130 subjects chosen by drawn was calculated, and data collection was performed at their homes. Results: There were interviewed 130 people, 92 (70.8%) women and 38 men (29.2%), with a minimum age of 60 and maximum of 96 years, with a mean of 72.8, median of 72.0 and a standard deviation of 8.3. Regarding the quality of care ratings of the PHC team, 48.5% (n = 63) of respondents stated this to be good, while 32.3% (n = 42) rated this as fair. Conclusions: In this perspective, one of the most appreciated meanings that were given to comprehensive care by health care professionals refers to holistic knowledge of each patient, resulting in the non-fragmentation of care. Thus, it is noticed that comprehensiveness has some weaknesses that need to be corrected, which shows the need for education and training of professionals assigned to primary health care services.

The study seeks to report the clinical evolution of a patient who suffered an inadvertent subarachnoid puncture and evolved with a herpetic viral meningitis, confirmed by cerebrospinal fluid (CSF) analysis. Female, 48 years old, submitted to epidural anesthesia, for surgery, with median puncture, between L2-L3 and inadvertently accessed the subarachnoid space, with clear cerebrospinal fluid. The same puncture was used and the solution with 15 mg hyperbaric bupivacaine and 80 mcg morphine was administered. There was no complication during the entire surgical procedure. On the first postoperative day, she started complaining of headache and nausea, being treated with dipyrone, non-steroidal anti-inflammatory drugs (NSAIDs) and ondansetron. On the third day, she presented two episodes of seizure that ceased with the use of diazepam 10 mg. On the fifth day, the patient presented fever and seizures, which did not stop with medication, requiring sedation, orotracheal intubation and transfer to the intensive care unit. Lumbar puncture was performed for CSF analysis, which was positive for Herpes simplex type I. The patient was extubated, on the second day of hospitalization, maintained with a maintenance dose of phenytoin, sumatriptan, dipyrone, metoclopramide and acyclovir 500 mg, remaining intravenous, without presenting new seizures for 45 days, when she was discharged. After 30 days, he returned for a review consultation without making a complaint.
