Metabolic and performance responses of male runners wearing 3 types of footwear: Nike Vaporfly 4%, Saucony Endorphin racing flats, and their own shoes

Purpose We compared running economy(RE)and 3-km time-trial(TT)variables of runners wearing Nike Vaporfly 4%(VP4),Saucony Endorphin lightweight racing flats(FLAT),and their habitual running(OWN)*** Eighteen male recreational runners(age=33.5±11.9 year(mean±SD),peak oxygen uptake(VO_(2peak))=55.8±4.4 mL/kg·min)attended 4 sessions approximately 7 days *** first session consisted of a VO_(2peak) test to inform subsequent RE speeds set at 60%,70%,and 80%of the speed eliciting *** subsequent sessions,treadmill RE and 3-km TTs were assessed in the 3 footwear conditions in a randomized,counterbalanced crossover *** Oxygen consumption(mL/kg·min)was less in VP4(from 4.3%to 4.4%,p≤0.002)and FLAT(from 2.7%to 3.4%,p≤0.092)*** across intensities,with a non-significant difference between VP4 and FLAT(1.0%-1.7%,p≥0.292).Findings related to energy cost(W/kg)and energetics cost of transport(J/kg·m)were ***43-km TT performance(11:07.6±0:56.6 mm:ss)was enhanced *** by 16.6 s(2.4%,p=0.005)and *** by 13.0 s(1.8%,p=0.032).The 3-km times between OWN and FLAT(0.5%,p=0.747)were *** runners(n=11,61%)ran their fastest TT in *** Overall,VP4 improved laboratory-based RE measures in male recreational runners at relative speeds compared to OWN,but the RE improvements in VP4 were not significant *** runners exhibited better treadmill TT performances in VP4(61%)***(22%)and OWN(17%).The variability in RE(-10.3%to 13.3%)and TT(-4.7%to 9.3%)improvements suggests that responses to different types of shoes are individualized and warrant further investigation.

In recent decades,Haiti has been subject to man-made and natural disasters that have left its citizens vulnerable to a range of *** a weak state unable to protect its populace,Haitians are exposed to some of the highest levels of poverty and violence in the Western *** recent years,Haitians have experienced two crises that this study analyzes:the instability and political violence of“peyi lòk”as well as the global pandemic of *** community-based assessment explores the impact of these two crises on the mental health and psychological well-being of 38 Haitian university students in the understudied northern part of the *** indicate that both crises had similarities related to their psychological effects on young people,most notably in terms of traumatic experiences related to threats or violence,forced confinement,and large increases in population-wide ***,the extreme violence of“peyi lòk”and the widespread unpredictability of COVID-19 and its effects in the early days of the pandemic resulted in high levels of stress and *** crises also resulted in extreme economic hardship for students,with many reporting difficulties accessing basic needs such as food and *** study highlights how converging population-level crises in“complex emergencies”can heighten trauma and compromise mental health.

Evolution of respiratory system compliance and potential for lung recruitment in COVID-19–induced acute respiratory distress syndrome

Objectives:Over the last decade,neonatal repair has been advocated for many congenital heart ***,specific subgroups of complex congenital heart disease still require temporary palliation for which both surgical and endovascular techniques are currently *** reviewed our institutional experience with shunt palliation with an emphasis on risk factors for early ***:This is a single-center retrospective study on 175 patients undergoing surgery for central shunt or modified Blalock-Taussig *** data were extracted from a prospectively collected computerized *** identified risk factors for early mortality by uni-and multi-variable *** data were censored at the time of death or shunt take-down ***:Mean age and weight at surgery were 24 days(IQR[7–95])and 3.4 kg(IQR[2.9–4.8]),respectively,with 96 neonates(55%).Most patients had a biventricular heart disease(115 patients,66%),and 51 patients(29.1%)had univentricular heart *** was performed in 129 patients(74%).Cardiopulmonary bypass was used in 23 patients(13%).The median intensive care and overall length of stay were 4 days(IQR[2–9])and 18 days(IQR[13–29]),***-hospital mortality was 8.6%(15/175).By multivariable regression analysis,prematurity(HR 5.6[2.1–14.7]),CPB use(HR 6.7[2.2–18.6]),unplanned<30-day reoperation(HR 3.5[1.2–10])or catheterization(HR 4.5[1.2–16.9])were all significant predictors of early ***:Procedural-related mortality remains high(8.6%)in surgical shunt *** patients with prematurity,low weight at birth,or if the use of cardiopulmonary bypass is contemplated,alternative endovascular techniques of palliation should be considered together with longitudinal follow-up studies.

Treatment of chronic viral hepatitis with nitazoxanide and second generation thiazolides

Nitazoxanide,the first thiazolide,was originally developed for the treatment of Cryptosporidium *** recently,antiviral activity of nitazoxanide against hepatitis B virus(HBV)and hepatitis C virus was recognized in in vitro *** basic studies led to phaseⅡclinical trials that demonstrated the safety and efficacy of nitazoxanide in combination with peginterferon,with or without ribavirin,in the treatment of chronic hepatitis C genotype *** sustained virologic response rate was 79%and 80%in two studies,which was higher than the response rate of 50%with the standard of care with peginterferon plus *** very preliminary studies of patients with chronic hepatitis B,nitazoxanide suppressed serum HBV DNA and led to loss of hepatitis B e antigen in the majority of patients and hepatitis B surface antigen in approximately a quarter of *** controlled studies of naive and nonresponder patients with chronic hepatitis C genotype 1 are underway,new second generation and controlled release thiazolides are being developed,and future studies of patients with chronic hepatitis B are planned.

Interendoscopist variability in proximal colon polyp detection is twice higher for serrated polyps than adenomas

AIM To assess the interendoscopist variability in the detection of colorectal polyps according to their location and histological *** This study was a retrospective analysis of prospectively collected data from a regional colorectal cancer(CRC) screening program; 2979 complete colonoscopies from 18 endoscopists were included. Variability in performance between endoscopists for detection of at least one adenoma(A), one proximal adenoma(PA), one distal adenoma(DA), and one proximal serrated polyp(PSP) was assessed by using multilevel logistic regression *** The observed detection rates among the 18 endoscopists ranged from 24.6% to 47.6%(mean = 35.7%) for A, from 19.1% to 39.0%(mean = 29.4%) for DA, from 6.0% to 22.9%(mean = 12.4%) for PA, and from 1.3% to 19.3%(mean = 6.9%) for *** adjusting for patient-level variables(sex, age), the interendoscopist detection rates variability achieved a significant level for A, PA, and PSP but not for DA(P = 0.03, P = 0.02, P = 0.02 and P = 0.08, respectively). This heterogeneity, as measured by the variance partition coefficient, was approximately threefold higher for PA(6.6%) compared with A(2.1%), and twofold higher for PSP(12.3%) compared with *** These results demonstrate significant interendoscopist variability for proximal polyp particularly for serrated p o l y p s, b u t n o t f o r d i s t a l a d e n o m a d e t e c t i o n. These findings contribute to explain the decreased effectiveness of complete colonoscopies at preventing proximal CRCs and the need to carefully assess the proximal colon during scope procedure.

Long-Term Persistent Absolute Insulin Secretion Deficiency in Diabetes Induced by Immune Checkpoint Inhibitors

Immune checkpoint inhibitors are today an immense hope in the management of cancers. However, since their widespread use, many cases of insulin-requiring diabetes appearing suddenly, as fulminant diabetes have been reported. Here, we describe 4 cases that occurred at different times after the beginning of immune checkpoint inhibitor therapy with Nivolumab alone or associated with Ipilimumab. There are 3 cases of newly diagnosed diabetes and 1 case of known type 2 diabetes formerly quite well balanced with Metformin. The clinical and biological characteristics of these patients are quite similar. They were all insulin-requiring at the discovery of diabetes and remained so throughout their follow-up. This type of diabetes which looks like type 1 diabetes seems rather to be a new entity.

Roadside vehicle particulate matter concentration estimation using artificial neural network model in Addis Ababa,Ethiopia
