Response of Soil Microbial Properties to Long-Term Application of Organic and Inorganic Amendments in a Tropical Soil (Saria, Burkina Faso)

Soil microbial biomass carbon (MBC), β-glucosidase, acid phosphatase and fluorescein diacetate (FDA) activities and bacterial community structure were assessed in a long-term (26 years) experiment, at physiological stages of sorghum growth, comparing different management methods for organic (manure, straw residues) and inorganic (urea) amendments at the INERA field station in Saria (Burkina Faso). Annual application of manure led to the highest soil microbial biomass and enzyme activities. Investigations indicated that only microbial biomass and β-glucosidase activities were affected during the cropping season. Phosphatase and FDA enzyme activities did not depend on the crop development stages. The application of N fertilizer modified phosphatase and FDA enzyme activities, the activities being higher in soils amended with N fertilizer. The bacterial community structure was analyzed by PCR-denaturing gradient gel electrophoresis (PCR-DGGE) targeting the eubacterial 16S rRNA gene. Cluster analysis of PCR-DGGE patterns showed two major clusters, the first containing the mineral fertilization and straw treatments and the second, the straw + urea, manure and manure + urea treatments. Sorghum grain yields were the highest for manure treatments. In this long-term experiment, applying straw did not produce a better grain yield than that obtained in the un-amended plot.

Epidemiology and Prognosis of Retroplacental Hematoma in a Maternity Ward at a Regional Hospital Center in Southern Senegal

Objectives: To study the epidemiological profile and the prognosis of the retro-placental hematoma (HRP) at the maternity ward at Kolda Regional Hospital Center. Materials and methods: This was a retrospective descriptive cross-sectional study conducted over a period of 11 years: from January 1st, 2006 to December 31st, 2016, at Kolda Regional Hospital Center. It included all patients admitted for HRP during this period. The studied parameters concerned sociodemographic characteristics, gynecological obstetrical history;clinical, therapeutic and prognostic data (non-inclusion or exclusion criteria). The data were collected from medical records, the delivery room and the surgery room registers using a collection sheet developed for this purpose. The statistical analysis of the variables studied was done with the software Epi-info 3.5. For the qualitative variables, we calculated the frequencies and for the quantitative variables, we studied the distributions. Results: During the study period, 15,343 were recorded deliveries and we carried out the diagnosis of HRP in 301 patients (1.97%). The average age of the patients was 24 years with an average parity of 4.8 deliveries. Almost all the patients (87.5%) were evacuated and half had delivered by caesarean section. Maternal and fetal deaths were 7% and 72.1%, respectively. Uterine atony accounted for 21.2% of complications. One third of the cases of uterine atony had resulted in a hysterectomy. The average duration of hospitalization was 6 days. Conclusion: The retro-placental hematoma is a serious medico-obstetric emergency. It is burdened with high maternal and fetal morbidity and mortality in developing countries.

Associated Factors with the Child’s Fully Immunized in the Tambacounda Health District (Senegal)

To improve vaccination coverage among children under one year of age in the Tambacounda health district, a household survey was carried out among mothers or babysitters. The objective was to study the factors related to child’s fully immunized in children aged 12 to 23 months. The cross-sectional, descriptive and analytical survey was carried out during the month of April 2019. A multistage cluster survey selected a sample of 657 mothers and babysitters. The data was collected using a questionnaire made from the World Health Organization reference guide. Data entry and analysis were done with Epi Info software and R. Among the women surveyed, biological mothers were the most representative (96.9%). In the series, 74.1% had a good knowledge of the age for initiating vaccination, 78.2% knew the number of contacts. The vaccination record of the children was available in 92.2%, and 71.0% of them had presented an adverse event. The proportion of children fully immunized was 41.0%. Complete childhood vaccination was positively associated with income-generating activity in women (0R = 2.4) and the short distance (<100 m) between home and place of vaccination (OR = 1.5). It was also improved by having a qualified health worker as a vaccinator (OR = 1.4) satisfaction in relation to visit (OR = 2.0), the advice given by the vaccinator (OR = 1.7) and the fixing of the date of the next vaccination appointment (OR = 2.5). The implementation of a good strategy for improving the quality of immunization services is an important element for strengthening immunization coverage in the Tambacounda health district.

Hydrocephalies and Vestibular Schwannoma of Great Size: Therapeutic Strategies: About a Series of 32 Cases

Introduction: The association hydrocephalus and vestibular schwannoma (VS) has been known for a long time. However, there is no therapeutic consensus, especially the place of drainage of cerebrospinal fluid (CSF). We report the result of our experience on the management of this pathology from a group of patients with a high volume VS (Koos IV) and operated consecutively. After reflections based on current literature data, we propose a therapeutic decisional algorithm. Materials and Methods: This is an analytical, retrospective study of 171 patients operated on KOOS IV vestibular schwannoma from January 2003 to December 2016 at the Marseille University Hospital Center. Of these, 32 patients with hydrocephalus and stage IV vestibular schwannoma were included. Radio-diagnostic criteria for hydrocephalus were based on Evans’ index, cortical furrow status, and the presence of trans-ependymal resorption. Our sample was divided into 2 groups. The first consisted of patients first operated on their hydrocephalus and secondarily treated with schwannoma surgery (group I);patients who underwent surgical resection of their first-line tumor were group II. Epidemiological, clinical, radiological, therapeutic and monitoring data were analyzed. The comparison of the quantitative variables was made by Fisher’s test. Results: During our study period (13 years), 171 cases of stage IV SV had been operated. The association between hydrocephalus and SV stage IV of Koos accounted for 18.7%. The average age of our patients was 53 years with a sex ratio of 0.7. The clinical picture was primarily composed of otological signs (90.6%), headache (56.3%) and cerebellar involvement (43.8%). The average diameter of VS inponto-cerebellar angle (PCA) was 31.5 mm. The treatment consisted of placing a first shunt of the CSF in 34.4% (group I). Study of Factors Associated with the Age of Natural Menopause in Menopausal Women Aged 30 to 80 Years from the Keur Massar Health District in 2015 (Senegal)

Introduction: Menopause is a biological phenomenon that occurs at a more or less early or late age in the life of every woman. The general objective is to study the factors associated with the age of natural menopause in menopausal women aged between 30 and 80 years who have gone through down Keur Massar health district in 2015. Methodology: This was an observational, cross-sectional, descriptive and analytical study conducted in Keur Massar health district from December 1st to 10th, 2015. Two-stage cluster sampling was conducted. Data were collected during an individual interview at home after consent. They covered socio-economic conditions, health and nutrition status, gynecological-obstetrical history, and lifestyle. A multivariate analysis was performed using the Cox model with a 5% risk of alpha error. Results: A total of 627 postmenopausal women were surveyed. Mean age of these women was 54.5 years (±8.6 years). A proportion of 72.1% of the women was married and 46.7% was in school. Mean age of natural menopause was 47.1 years (±5.03 years). The median age of natural menopause was 47 years. Factors associated with the age of natural menopause were high parity of more than 4 children (Hra = 0.35 [0.13 - 0.92]), the existence of frequent childhood illnesses (Hra = 1.72 [1.13 - 2.64]), use of the intra-uterine device—IUD—as a modern contraceptive method (HRa = 0.39 [0.24 - 0.63]), sibling size (HRa = 0.85 [0.78 - 0.93]), family size (HRa = 1.03 [1.01 - 1.06]), and polygamy regime (HRa = 0.56 [0.37 - 0.85]). Conclusion: Many factors in childhood as well as during the period of female genital activity can impact the age of natural menopause. It is then important to improve the health status of women, children and the standard of living of the population.

Fetal Instrumental Extractions at the Maternity Section of the Dakar Nabil Choucair Health Centre (Senegal) in 2017: Epidemio-Clinical Aspects, Indications and Prognosis

Introduction:?An instrumental extraction is performed in order to shorten the phase of expulsion of the fetus outside the maternal pelvic pathway, when there is a suspected fetal state or a defect in progression of the fetal mobile. It can be responsible for immediate or late maternal complications, which are not specific because they can occur after a normal delivery. The objectives of this work are to describe the epidemiological-clinical and prognosis aspects of deliveries assisted by instrumental extractions in a reference maternity unit in Dakar.?Materials and Methods:?This was a retrospective, descriptive and analytical study between January 1, 2017 and December 31, 2017, a period of 12 months at the maternity ward of the Nabil Choucair Health Center. The collection was carried out using the survey form completed on the basis of the analysis of the files, the delivery register and the anaesthesia register of the operating room. The parameters studied were about socio-demographic?characteristics, indications and prognosis. The data entry was carried out using the Sphinx version 5 software and the data analysis using the Epi info version?3.5?software. Results:?During the period of our study, we collected 94 instrumental?extractions. The frequency of instrumental extractions was?1.7%. The average age of the patients was 25 years with extremes of 16 to 43 years old. The average parity was 1 with extremes 1 to 6. Among parturient women, 12?patients (12.8%) had a history of suction cup, two (2.1%) had received forceps and three (3.2%) had a cesarean section. The average gestational age was 39 Weeks of Amenorrhea (WA), the average uterine?height was 32?cm, fetal heart sounds were normal in 98% parturient’ cases. The vaginal?touch had found a fully dilated cervix, a rupture of the amniotic sac with clear amniotic?fluid in 98% of parturient women and a fetus with an anterior left iliac occipito topin 69.4% of cases. Two Cases of Hallopeau Type of Pemphigus Vegetans in Two Women in Burkina Faso

Pemphigus of Hallopeau (PH) is a mild and pustular form of pemphigus vegetans, termed initially as “pyodermite végétante” described by Hallopeau. We report cases in two black women. Two women aged 32 and 36 respectively (case 1 and case 2) were seen for a pustulosis associated with blisters and erosions. Their history and their review have found the mouth ulcerations. The clinical examination showed pustules on the cephalic end, the trunk, the limbs and the large folds. Lesions had a polycyclic aspect and spread in a centrifugal way, developing towards budding, squamous and scabby erosions quickly surrounded by new pustules. Regression left a pigmented macula. Histological examination of the skin lesion showed images similar to vulgaris pemphigus (PV), together with hyperacanthosis and papillomatosis. Aspects observed in immunofluorescence were identical to those of PV (case1). General corticoid therapy led to a rapid disappearance of lesions in Case 1 but in Case 2 the Lever protocol combining corticoid and methotrexate have been used. PH is rarely described in our regions. Clinical diagnosis is not obvious for the uninitiated. Beyond the clinical presentation, PH has the same paraclinical diagnosis and the same treatment as PV.

Factors Associated with the Use of Cervical Cancer Screening in the Mbour Health District (Senegal)

Cervical cancer with an estimated incidence between 1700 and 2000 new cases per year and mortality around 70% to 80% has become the leading cancer in Senegal. The objectives of this present work were to study the factors associated with cervical cancer screening in the Mbour health district in 2015. It was a cross-sectional, descriptive and analytical study carried out on a sample of 599 women chosen according to a two-stage survey. The survey used a questionnaire where data on individual characteristics, knowledge, attitudes and practices are collected during an individual interview after informed consent. The data collected were entered in the Epi Info 3.5.3 software and analyzed with the R 3.1.2 software. The average age was 44.4 years with a standard deviation of 8.9. The proportion of women screened for cervical cancer was 19.4%. Screening for cervical cancer was positively related to the possession of an income-generating activity (Adjusted OR = 2.39 [1.4 - 4.2]), increase of education and knowledge of a person suffering from cancer in the environment (adjusted OR = 2.40 [1.3 - 4.2]). Knowledge of the methods of diagnosis of cervical cancer was positively associated with cervical cancer screening (adjusted OR = 20 [11.1 - 33.3]). The lack of awareness and the lack of financial resources and education for women were the main obstacles to cervical cancer screening for women in Mbour. The health system should focus on building the capacity of women to fight cervical cancer.
