左冠状动脉主干外科血管成形术的远期疗效

目的评估左冠状动脉主干外科血管成形术治疗以左冠状动脉主干狭窄为主要病变的冠状动脉心脏病的远期疗效。方法回顾性分析1983年9月至2004年12月因左冠状动脉主干狭窄接受左冠状动脉主干外科血管成形术治疗的162例患者的临床资料。采用单因素方差分析手术病死率相关因素，采用Kaplan—Meier生存曲线法分析远期生存率。结果围手术期病死率8％，与是否为单纯左冠状动脉主干病变，是否为急诊手术相关。出院患者随访率95％，中位随访时间102个月。42例患者发生远期冠状动脉事件，9例患者死亡。10年及20年累计生存率分别为81％和52％，无冠状动脉事件生存率分别为77％和41％。结论左冠状动脉主干外科血管成形术治疗以左冠状动脉主干狭窄为主要病变的冠状动脉心脏病，可以获得较好的远期疗效，具有良好的临床应用价值。

目的　比较非体外循环搭桥术与体外循环搭桥术病人的术中及术后早期恢复情况。方法　 39例病人接受了非体外循环下冠脉搭桥术 ,同期 33例病人接受了体外循环下冠脉搭桥术。两组均采用中等剂量阿片静脉复合全麻。结果　病人的麻醉时间、手术时间、术后机械通气时间和在监护室的停留时间 ,非体外循环组均明显短于体外循环组 (P <0 0 1)。围术期平均输血量和输血病人数在 1支桥病人中非体外循环组明显少于体外循环组 (P <0 0 5 )。术后房颤发生率和围术期心肌梗死发生率两组间无明显差异 ,术后脑卒中、低心排及死亡的发生率各组均为 0 %。结论　与体外循环搭桥术相比 ,非体外循环搭桥术缩短了病人的麻醉、手术及在监护室停留的时间 ,加快了病人的恢复 ,从而提高了手术的安全性。

AIM To investigate the survival benefit of bilateral internal mammary artery(BIMA) grafts in patients with left ventricular *** Between 1996 and 2009,we performed elective,isolated,primary,multiple cardiac arterial bypass grafting in 430 consecutive patients with left ventricular ejection fraction ≤ 40%. The early and long-term results were compared between 167 patients undergoing BIMA grafting and 263 patients using left internal mammary artery(LIMA)-saphenous venous grafting(SVG).RESULTS The mean age of the overall population was 60.1 ± 15 years. In-hospital mortality was not different between the two groups(7.8% vs 10.3%,P = 0.49). Early postoperative morbidity included myocardial infarction(4.2% vs 3.8%,P = 0.80),stroke(1.2% vs 3.8%,P = 0.14),and mediastinitis(5.3% vs 2.3%,P = 0.11). At 8-year follow-up,Kaplan-Meier-estimated survival(74.2% vs 58.9%,P = 0.02) and Kaplan-Meier-estimated event-free survival(all cause deaths,myocardial infarction,stroke,target vessel revascularization,heart failure)(61.7% and 41.1%,P < 0.01) were significantly higher in the BIMA group compared with the LIMA-SVG group in univariate analysis. The propensity score matching analysis confirmed that BIMA grafting is a safe revascularization procedure but there was no long term survival(P = 0.40) and event-free survival(P = 0.13) in comparison with LIMA-SVG *** Our longitudinal analysis suggests that BIMA grafting can be performed with acceptable perioperative mortality in patients with left ventricular dysfunction.

目的回顾性分析66例心源性休克合并多器官功能不全（MOD）患者的血流动力学支持治疗临床资料，探究影响患者死亡率的主要风险因素。资料与方法1998-2003年，66例心源性休克合并多器官功能不全患者，年龄（44．8土10．4）岁，转入法国南锡（Nancy）大学医疗中心，寻求机械循环辅助及心脏移植。对所有病例均首先采取药物支持疗法，40．9％（27／66）患者在接受12-24h药物支持治疗后，循环状态仍不稳定，并有MOD渐进性加重趋势，改用机械循环辅助治疗。结果12．1％（8／66）患者在药物支持治疗下循环状态重新稳定，脱离药物支持。53．0％（35／66）患者在药物或机械辅助下成功过渡到心脏移植，全系列总死亡率为34．8％（23／66）。Logistic回归分析结果显示，患者入院时初始MOD评分和血清乳酸浓度是影响患者死亡率的独立风险因素。初始MOD评分〉9及初始血清乳酸浓度〉4mmol／L的病例死亡概率明显增加。结论在心源性休克合并MOD患者的血流动力学支持治疗中，初始MOD评分及血清乳酸浓度水平对于判断预后具有重要价值。

针对我国后续深空探测任务的测控需求,本文详细研究了射电跟踪数据中斜坡多普勒频率数据的处理方法,研制了一套火星探测器精密定轨实测数据处理软件.本文处理了MRO探测器2007年1月的斜坡多普勒频率数据,成功实现了精密定轨.结果表明,定轨后残差的RMS在0.74‒1.19 mm/s之间,均值约为0.81 mm/s,重建轨道的位置精度达到米级,速度精度达到毫米级,与JPL发布轨道的精度相近,并进一步探讨了大气阻力和太阳光压力的取值对MRO探测器轨道精度的影响.这一研究可为我国后续深空探测任务的射电数据处理提供技术支持.

A substantial body of research has been conducted with the objective of enhancing the understanding of Non-Timber Forest Products (NTFP) in Central Africa. A significant proportion of the studies focus on medicinal plants, in contrast to those that examine food products. Nevertheless, studies on edible wild mushrooms as NTFPs and cultivated mushrooms, on their economic potential and value chains in the Great Lakes Region, remain scarce and largely absent for some NTFPs. In light of the aforementioned considerations, the present study aims to examine the economic potential and value chain of the production sector of wild edible mushrooms (WEM) and cultivated mushrooms (CM) within the Virunga National Park (PNVi) and its surrounding areas in North Kivu, Democratic Republic of the Congo (DRC). To this end, a sample of 432 respondents was selected, distributed equitably among the actors of the Goma-Kitshanga-Mweso value chain, the Goma-Rutshuru chain, the Beni-Mutwanga chain, and the Beni-Mangina chain. A digital questionnaire consisting of open and closed questions was administered, and observations were carried out concurrently. The quantities of mushrooms sold were weighed and photographed. The results demonstrated that, when the eight-month fruiting period of wild and cultivated mushrooms was considered as a single year for all actors within the value chain, a picker could sell 6.1 kg at $2 and earn $12. A seller could sell 16 kg at $4 and earn $64, while a grower could sell 502 kg at $3 and earn $1506. Consequently, the CM grower generates a significantly higher profit than other participants in the value chain, despite the expenses he incurs. Ultimately, the domestication of WEM, highly prized by the local population of the Virunga National Park, has the potential to enhance their household incomes, as evidenced by the findings of this study.

Arabidopsis cryptochromes cry1 and cry2 are blue-light signalling molecules with significant structural similarity to photolyases-a class of blue-light-sensing DNA repair enzymes. Like photolyases, purified plant cryptochromes have been shown to bind both flavin and pterin chromophores. The flavin functions as a light sensor and undergoes reduction in response to blue light that initiates the signalling cascade. However, the role of the pterin in plant cryptochromes has until now been unknown. Here, we show that the action spectrum for light-dependent degradation of cry2 has a significant peak of activity at 380 nm, consistent with absorption by a pterin cofactor. We further show that cry1 protein expressed in living insect ceils responds with greater sensitivity to 380 nm light than to 450 nm, consistent with a light-harvesting antenna pigment that transfers excitation energy to the oxidized flavin of cry1. The pterin biosynthesis inhibitor DHAP selectively reduces cryptochrome responsivity at 380 nm but not 450 nm blue light in these cell cultures, indicating that the antenna pigment is a folate cofactor similar to that of photolyases.

Present study provides a simple analytical formula,the“Klingel-like formula”or“Pascal’s Formula”that can be used as a reference to test some results of existing railway codes and specifically those using rigid *** develops properly the 3D Newton-Euler equations governing the 6 degrees of freedom(DoF)of unsuspended loaded wheelsets in case of zero wheel-rail friction and constant ***,by solving numerically these equations,we got pendulum like harmonic oscillations of which the calculated angular frequency is used for assessing the accuracy of the proposed formula so that it can in turn be used as a fast practical target for testing multi-body system(MBS)railway *** to the harmonic property of these pendulum-like oscillations,the squareω2 of their angular frequency can be made in the form of a ratio K/M where K depends on the wheelset geometry and load and M on its *** on K and M are useful to understand wheelsets *** analytical formula is derived from the first order writing of full trigonometric Newton-Euler equations by setting zero elastic wheel-rail penetration and by assuming small *** trigonometric equations are numerically solved to assess that the formula providesω2 inside a 1%accuracy for usual wheelsets *** decreasing the conicity down to 1×10^(−4) rad,the relative formula accuracy is under 3×10^(−5).In order to test the formula reliability for rigid contact formulations,the stiffness of elastic contacts can be increased up to practical rigidity(Hertz stiffness×1000).
