Four early lactating Holstein cows were used to study the effect of live yeast (LY, Actisaf®CNCm I-4407, Lesaffre Feed Additives, marcq en Baroeul, France) supplementation on diet digestive utilization of dairy cows receiving concentrated corn silage-based diets with two rumen-degradable protein (RDP) levels. For a 33 d period, cows were fed a total mixed ration (TmR) containing an adequate level (AL) of RDP or a low level (LL, 30% below AL) by using soybean meal or tanned soybean meal, respectively: for 21 d with no LY addition followed by 12 d during which LY was added to the diet. The pH and redox potential (Eh) were recorded and ruminal fluid samples were collected over 3 consecutive days. Feces were collected individually over 48 h and individual dry matter intake (DmI) was measured for determining apparent nutrient digestibility. The effective degradability of individual feed ingredients composing both diets was evaluated with nylon bags technique. Structure of the ruminal bacterial community was studied and diversity index was calculated. Digestibility of organic matter (Om) and crude protein (CP) were lower for LL than those for AL. With LY, digestibility of Om and CP was increased: +2.4 and +0.8 points, for AL, and +3.7 and +5.9 points for LL, respectively. Live yeast reduced dietary N ruminal degradation with both AL and LL. Ruminal pH and Eh were lower with AL compared to LL: 5.95 and –167 mV vs. 6.13 and –144 mV. Live yeast increased ruminal total volatile fatty acids (VFA) (+8.6%), C2 (+10%), and C4 (+35%) contents for LL and decreased that of C3 (?9.8%) for AL. Neither the structure of bacterial populations of the rumen nor the diversity index (Shannon) was altered by treatments. Those results suggested a specific interest in using LY in RDP deficient diets for early lactating cows.

m>Objectivem>: In order to take a decision about the revascularization approach to be adopted, it is of fundamental importance to determine whether coronary artery stenoses induce ischemia or not. An index, named (Fractional Flow Reserve), based on pressure measurements has been proposed to this aim and is usually interpreted in terms of flows. The objective of this work is to compute simultaneously pressures and flow rates in the coronary network of patients with three-vessel disease, in order to study more precisely the relationship between these two quantities. m>Approachm>: 22 patients have been included in the study. Some pressure and flow rate measurements were collected during by-pass surgery. These clinical data allow determining parameters for a patient’s specific model, based on the electric/hydraulic analogy. Collateral pathways are included in the model, as well as the severity of the disease and the impact of revascularization. m>main Resultsm>: For patients with stenoses on LAD, LCx, LmCA and occlusion of the RCA, the flow rate delivered to the right territory is of course a function of the aortic pressure, the left stenoses severity, and the pressure distal to the thrombosis. But it mainly depends on the capillary and collateral resistances, and on the proportion between them. Abnormal microvascular hemodynamics, may be present in patients with non-hemodynamic significant lesions as assessed by the pressure ratio. Complete revascularization with the 3 grafts is demonstrated to be fully justified. The direction of collateral flows may be reversed, depending on the pressure gradient. In any case, they remain low and become negligible when the 3 grafts are operating. m>Significancem>: Surgical decision based only on pressure measurements may miss some real hemodynamic problems due to the considered stenosis. This risk is even greater in case of serial stenoses.

Background: Alzheimer’s sufferers (AS) are unable to visually recognize facial emotions (VRFE). However, we do not know the kind of emotions involved, the timeline for the onset of this loss of ability to recognize facial emotional expressions during the natural course of this disease and the existence of any correlation with other comorbid cognitive disorders. For that reason, the authors aimed to determine whether a deficit in facial emotion recognition is present at the onset of Alzheimer disease, distinctly and concurrently with the onset of cognitive impairment or is it a prodromal syndrome of Alzheimer’s Disease before the onset of cognitive decline and what emotions are involved. A secondary aim was to investigate relationships between facial emotion recognition and cognitive performance on various parameters. method: Single Blind Case-control study. Setting in memory clinic. Participants: 12 patients, (AS) and 12 control subjects (CS) were enrolled. measurements: Quantitative information about the ability for facial emotion recognition was obtained from method of Analysis and Research on the Integration of Emotions (mARIE). The mini mental Status Examination (mmSE), the Picture Naming, the mattis Dementia Rating Scale (DRS), and the Grober & Buschke Free and Cued Selective Reminding Test (FCSRT) tests were used to measure cognitive impairment. Results: We note that the AS have a problem with the visual recognition of facial emotions with existence of a higher threshold for visual recognition. The AS is less sensitive to the visual recognition cues of facial emotions. AS is unable to distinguish anger from fear. This paper describes our contribution in the ANR (Agence Nationale de la Recherche) project called GELOCOm (GEo-LOCalisation de telephOnes mobiles) managed by the THALES Communications company, dedicated to the emergency localization of cellular phones. This contribution takes place in the field of antennas, with the development of broad-band systems: a circular array of six elements with separated outputs for the receiving part. In this paper, we present the design and the characterization of broad-band double ellipse array antenna. This special structure is chosen in order to obtain a good omnidirectional radiation pattern, enhance the gain and maximize the V/H polarization ratio. In comparison with the already existing antenna systems in the wireless market for similar purposes, the proposed antenna has considerably shown better performance which makes it competitive among other antenna models. For the design and optimization of antennas, we use CST mWS software. The antennas have been designed and successfully measured.

Acute humoral rejection （AHR） is uncommon after ABO- compatible liver transplantation. Herein, we report two cases of AHR treated with plasmapheresis and rituximab in two ABO-compatible liver-transplant patients with preformed anti-human leukocyte antigen donor-specific antibodies. Patient 1 experienced a biopsy-proven AHR at day 10 post-transplant. She was treated by steroid pulses, and OKT3. Because of persisting signs of biopsy-proven AHR at day 26, she was treated by plasmapheresis and rituximab. Uver enzyme levels did not improve, and she died on day 41. Patient 2 experienced a biopsy-proven AHR on day 10 post-transplant. She was treated by steroid pulses, plasmapheresis, and rituximab. Liver enzymes returned to within normal range 18 d after diagnosis. Uver biopsies, at 3 and 9 mo post-transplant, showed complete resolution of AHR. We conclude that plasmapheresis should be started as soon as AHR is diagnosed, and be associated with a B-cell depleting agent. Rituximab may be considered as a first-line therapy.

AIm:To look at a comprehensive picture of etiology- dependent gene abnormalities in hepatocellular carcinoma in Western Europe. mETHODS:With a liver-oriented microarray,transcript levels were compared in nodules and cirrhosis from a training set of patients with hepatocellular carcinoma (alcoholism,12;hepatitis C,10)and 5 *** or tight selection of informative transcripts with an abnormal abundance was statistically valid and the tightly selected transcripts were next quantified by qRTPCR in the nodules from our training set(12+10) and a test set(6+7). RESULTS:A selection of 475 transcripts pointed to significant gene over-representation on chromosome 8 (alcoholism)or-2(hepatitis C)and ontology indicated a predominant inflammatory response(alcoholism)or changes in cell cycle regulation,transcription factors and interferon responsiveness(hepatitis C).A stringent selection of 23 transcripts whose differences betweenetiologies were significant in nodules but not in cirrhotic tissue indicated that the above dysregulations take place in tumor but not in the surrounding *** 23 transcripts separated our test set according to etiologies. The inflammation-associated transcripts pointed to limited alterations of free iron metabolism in alcoholic vs hepatitis C tumors. CONCLUSION:Etiology-specific abnormalities(chromo- some preference;differences in transcriptomes and related functions)have been identified in hepatocellular carcinoma driven by alcoholism or hepatitis *** may open novel avenues for differential therapies in this disease.

A mEmS electrostatic kinetic energy harvester(e-KEH)of about 1 cm2,working at ultralow frequency(1–20 Hz),without any supported additional mass on its mobile electrode,and working even without a vacuum environment is *** prototype is especially suitable for environments with abundant low frequency motions such as wearable *** proposed e-KEH consists of a capacitor with a finger-teeth interdigited comb *** greatly reduces the air damping effect,and thus the capacitance variation remains important regardless of the presence of *** the new design,the energy transduced per cycle of excitation is no less than 33 times higher than the classic design within 10–40 Hz/2 g_(peak),while is 85 times higher at 15 Hz/2 g_(peak).An enclosed miniature ball combined with non-linear stoppers enables the oscillation of the movable electrode through impact-based frequency up-conversion mechanism,which is also improved by the low air *** to this new design,a higher efficiency than the classic gap-closing comb structure is obtained,as a larger range of working frequency(1–180 Hz)in air.A maximum energy conversion of 450 nJ/cycle is obtained with a bias voltage of 45 V and an acceleration of 11 Hz,3 g_(peak).Working with a diode AC-DC rectifier,the proposed KEH is able to support up to 3 RFID communications within 16 s while operated at 11 Hz,3 g_(peak).

Context: Exposure to burnout of staff involved with elderly patients is dependent on many factors either personal or linked to the professional environment. Social stress and systemic problems created particularly by difficulties inherent in the French hospital management system and the way people feel it, lead to a risk of burnout. One illustration of this is the rise in suicides at work. Quality of life at work, harassment and psycho-social risks are intimately linked. Affective factors, such as suffering for the medical carers in response to the distress of their patients aggravate the risk of burnout. methods: We have evaluated these parameters using a self-filled questionnaire form sent to all staff and filled in by computer, anonymously, in 4 establishments, in December 2012 and over the first semester of 2013. After the three factors studied by the ProQOL scale of quality of life at work, to do with burnout, satisfaction compassion and fatigue compassion, 5 other questions were added, connected with a feeling of harassment and several social and demographic matters. Burnout risk was retained on reaching a threshold of 30 for this ProQOL scale item. Results: After multivariate analysis including the parameters of the Stamm scale, harassment and the socio-demographic factors studied, (age, sex, seniority, profession, and work departments) 4 factors are significantly associated with the risk of burnout, one negatively, compassion satisfaction, three positively, compassion fatigue, harassment experience and seniority. Conclusions: The risk of burnout is linked to subjective factors—the way quality of life at work is perceived and harassment experienced. Some professions, such as nurses, are particularly exposed and require these risk factors to be foreseen.
