Determinants of Outcomes and Prognosis Score in Obstetric Vesico-Vaginal Fistula Repair

An estimated 2 million women living in countries with limited resources currently have vesico-vaginal, recto-vaginal, or mixed types of fistulae because of the necrosis that occurs with obstructed labor. We evaluated factors readily assessed by the examining practitioner in a consecutive case series of surgical repairs of obstetrical fistulae, for the ability of those factors to stand as a prognostic guide, in a clinical score. Objectives: To identify the predictors of surgical repair outcomes and establish a Score combining these different determinants to facilitate the care of obstetric fistula. Methods: We conducted a multicentric prospective study between 2011 and 2014 in Democratic Republic of the Congo (DRC). Outcomes: We measured 3 months post-surgery in a series of 483 patients with obstetrical fistula repaired by the same surgeon included closure and failure appreciated by dye test. Multivariable generalized estimating equation models were used to generate adjusted odd ratios (OR) and 95% confidence intervals (CIs). The scores ranging from 3 to 14 were established from the outcomes determinants identified. Results: In total, 483 women were enrolled, and 390 cases were at their first surgery and were included in the obstetric fistula (OF) prognosis score and classification. Their mean age was 35 years at the time of the surgery and 25 years at the onset of OF. In 28.6%, the fistula patient was primigravida. Mean duration between onset of the fistula and surgical treatment was 8 years. In 24%, the fistula patients lived separated from their partners. Overall closure rate of the fistulas was 85.7%. Severe vaginal fibrosis (p 0.01), big fistula size (0.01), small distance from fistula to external urethral meatus (0.01) and prior surgery (p 0.48) predicted failed fistula closure. Investigating Land-Use Change on Street Tree Ecosystems

In the early 1940’s, during the early stages of the Manhattan Project (WWII), of rural communities in Anderson County, Tennessee was rapidly converted into laboratory facilities and the city of Oak Ridge. The environment that became Oak Ridge experienced not only pollutants from the laboratory activities, but also alterations from the land-use change?from rural to urban areas. Therefore, a study was conducted to determine the impacts of land-use change from rural to urban area on 1) street tree diversity and performance;and 2) the biological, chemical and physical properties, and nutrient dynamics of street tree ecosystem soils. There were a total of 607 street trees, composed of 37 different species, on the five main roadways in Oak Ridge, Tennessee. The street tree inventory revealed that the street tree ecosystems had a high relative abundance of Acer rubrum (21.91%) and Pyrus calleryana (19.93%). Chemical, rather than physical, soil and site properties in street tree ecosystems had the greatest impact on street tree performance. Soils differed street by street in their biological, chemical, and physical properties but were not influenced by traffic rates. There were also differences in soil microbial biomass carbon (MBC) during the winter on streets based on their diversity of trees;however, the most diverse street was among the lowest in soil microbial biomass. Seasonally, the winter proved to have not only greater amounts of soil microbial biomass carbon and nitrogen (MBN), but significantly less extractable organic carbon (EOC) and nitrogen (EON) and total labile carbon (TLC) than that in the spring. Overall, this study provided insights into the post urbanization impacts on the street trees, soils, sites, and nutrient dynamics within street tree ecosystems of Oak Ridge.

Microscopic enteritis(ME) is an inflammatory condition of the small bowel that leads to gastrointestinal symptoms,nutrient and micronutrient *** is characterised by microscopic or sub-microscopic abnormalities such as microvillus changes and enterocytic alterations in the absence of definite macroscopic changes using standard modern *** work recognises a need to characterize disorders with microscopic and submicroscopic features,currently regarded as functional or non-specific entities,to obtain further understanding of their clinical *** consensus working party reviewed statements about the aetiology,diagnosis and symptoms associated with ME and proposes an algorithm for its investigation and *** the 5th International Course in Digestive Pathology in Bucharest in November 2012,an international group of 21 interested pathologists and gastroenterologists formed a working party with a view to formulating a consensus statement on ME.A five-step agreement scale(from strong agreement to strong disagreement) was used to score 21 statements,*** was strong agreement on all statements about ME histology(95%-100%).Statements concerning diagnosis achieved 85% to 100% agreement.A statement on the management of ME elicited agreement from the lowest rate(60%) up to 100%.The remaining two categories showed general agreement between experts on clinical presentation(75%-95%) and pathogenesis(80%-90%) of *** was strong agreement on the histological definition of *** agreement on management indicates a need for further investigations,better definitions and clinical trials to produce quality guidelines for *** ME consensus is a step toward greater recognition of a significant entity affecting symptomatic patients previously labelled as non-specific or functional enteropathy.

Environmental Assessment and Dielectric Performances of the Secondary Electrical Insulation in Low Voltage Motors Impregnated by Different Techniques

A recent technique, using to potentially replace the conventional impregnating one in low voltage electric motors, has been studied in this communication. The conventional technique that uses?both primary and secondary insulations;?i.e.: enamel and varnish, has been compared to this recent technique which only uses one component,?i.e.: self-bonding wires. Self-bonding wires polymerization is very quick compared to that of the conventional technique. Environmental impact and both dielectric and mechanical properties of these two techniques have been estimated and compared. The goal is to estimate if this recent technique has not only a better environmental footprint but also good technical properties. The dielectric properties that have been measured are the dielectric strength and?the lifetime under pulse?voltage while mechanical consists in measuring the bonding strength that is crucial for impregnation.?For that purpose, a Life Cycle Assessment (LCA) that can simultaneously evaluatenumerous impacts on ground, water and air,?has been performed. The results show clearly that the recent technique significantly reduces the environmental footprint. Both mechanical and dielectric properties are then compared and analyzed.

High Plasma Concentrations of Sclerostin, an Inhibitor of the Wnt Signaling Pathway, in Young Horses Affected by Osteochondrosis

Osteochondrosis (OC) is a developmental disease in horses with a significant impact on the horse’s welfare and performance. The early disturbance of enchondral ossification progresses to inflammatory and healing process in older horses. Metabolic pathway analysis showed an obvious dysregulation of several signaling pathways related to cartilage formation and cartilage repair such as Wnt/β-catenin, Indian hedgehog and TGF-β signaling pathways. Other regulated genes appeared to be involved in high carbohydrate diet, abnormal insulin metabolism or inflammation. Sclerostin is an osteocyte-secreted soluble antagonist of the Wnt/β-catenin signaling pathway. It is crucial for osteoblast development and activity and is increased in naturally occurring lesions of equine osteochondrosis. The aim of this study is to compare the circulating sclerostin levels between OC-affected (n = 20) and healthy horses (n = 19). A significant linear regression between plasma sclerostin and age is observed especially in the healthy young horses. The mean plasma sclerostin concentration is significantly higher in young horses suffering from osteochondrosis compared to the control horses. These results reinforce the possible role of the Wnt/β-catenin signaling pathway in the OC pathogeny. The inhibition of this essential pathway could disturb the osteo-chondral differentiation. More studies are currently needed to define the eventual clinic interest of plasma sclerostin as future biomarker in bone and cartilage diseases.

Advanced Thermopile IR Dual Line Sensor for Smart Home

This article presents all steps between the advanced design and the production of CMOS compatible thermoelectric effect infrared sensors dedicated to smart home applications. It will start by making a comparison between thermopile, bolometer and pyroelectric technologies. Although sensitivity performances available with bolometers appear to be better at first sight, it is found that thermopiles have non-negligible advantages that make them more suitable for this application field. Then the different steps necessary for the design will be described, starting from the thermoelectric model of the sensor (temperature gradient, electrical sensitivity, etc.) and considering all steps up to technological manufacturing in a clean room. The results obtained on the structures produced on a specific computer-controlled measurement bench (temperature regulation with an onboard preamplification card) will be presented. Finally, the results prove that the square structures have better performances (S = 82 V/W and NETD = 208 mK).

Dear Editor,Dengue is a worldwide zoonosis,transmitted by mosquitoes and highly prevalent in tropical and subtropical *** recent years,its incidence has increased with a massive healthcare burden worldwide[1]and recent climatic changes have expanded the geographic range of the insect vector with described outbreaks in Europe.[2]Most patients present pauci-symptomatic infections,but some patients could present severe manifestations with high reported mortality[[3],[4],[5]]and no specific *** bleeding,hemodynamic failure and various organ damage(cardiac,renal,neurological,and hepatic)are described in severe dengue(SD)patients.[5]In the future,intensivists worldwide will be increasingly treating SD and the spectrum of severe organ impairment should be well known.

Exercise-induced albuminuria and circadian blood pressure abnormalities in type 2 diabetes

AIMTo investigate the relationship between circadian vari-ations in blood pressure （BP） and albuminuria at rest, and during exercise in non-hypertensive type 2 diabetes （T2D） *** conducted a cross-sectional study in well controlled T2D patients, non-hypertensive, without clinical pro-teinuria and normal creatinine clearance. In each parti-cipant, we recorded the BP using ambulatory bloodTankeu AT et al . Exercise-induced albuminuria and BP in T2DMpressure monitoring （ABPM） for 24-h, and albuminuria at rest and after a standardized treadmill *** enrolled 27 type 2 patients with a median age of 52; and a mean duration of diabetes and HbA1c of 3.6 ± 0.8 years and 6.3% ± 0.5% respectively. Using a 24-h ABPM, we recorded a mean diurnal systolic blood pressure （SBP） of 128 ± 17 mmHg vs nocturnal of 123 ± 19 mmHg （ P = 0.004）, and mean diurnal diastolic blood pressure （DBP） of 83 ± 11 mmHg vs nocturnal 78 ± 14 mmHg （ P = 0.002）. There was a signifcant difference between albuminuria at rest [median = 23 mg, interquartile range （IQR） = 10-51] and after exercise （median = 35 mg, IQR = 23-80, P 〈 0.001）. Patients with exercise induced albuminuria had an increase in nocturnal BP values on all three components （128 mmHg vs 110 mmHg, P = 0.03 for SBP; 83 mmHg vs 66 mmHg, P = 0.04; 106 vs 83, P = 0.02 for mean arterial pressure）, as well as albuminuric patients at rest. Moreover, exercise induced albuminuria detect a less increase in nocturnal DBP （83 vs 86, P = 0.03） than resting *** induced albuminuria is associated with anincrease in nocturnal BP values in T2D patients.
