Lessons from “real life experience” of rifaximin use in the management of recurrent hepatic encephalopathy

Acute humoral rejection （AHR） is uncommon after ABO- compatible liver transplantation. Herein, we report two cases of AHR treated with plasmapheresis and rituximab in two ABO-compatible liver-transplant patients with preformed anti-human leukocyte antigen donor-specific antibodies. Patient 1 experienced a biopsy-proven AHR at day 10 post-transplant. She was treated by steroid pulses, and OKT3. Because of persisting signs of biopsy-proven AHR at day 26, she was treated by plasmapheresis and rituximab. Uver enzyme levels did not improve, and she died on day 41. Patient 2 experienced a biopsy-proven AHR on day 10 post-transplant. She was treated by steroid pulses, plasmapheresis, and rituximab. Liver enzymes returned to within normal range 18 d after diagnosis. Uver biopsies, at 3 and 9 mo post-transplant, showed complete resolution of AHR. We conclude that plasmapheresis should be started as soon as AHR is diagnosed, and be associated with a B-cell depleting agent. Rituximab may be considered as a first-line therapy.

Optimizing the Particle Size of Shale and Laterite Used as Constructed Wetland Substrates for Wastewater Treatment

Constructed Wetlands (CWs) are currently one of the most promising techniques for wastewater treatment, having demonstrated their effectiveness. However, the choice of substrate particle size is critical to the smooth operation of the process, as hydrodynamic constraints require a coarse particle size, whereas wastewater treatment recommends a fine particle size. This study investigates the suitability of laterite and shale as substrates of different sizes (1 - 3, 3 - 5 and 5 - 8 mm) in CWs for domestic wastewater treatment. The study was carried out in an experimental pilot plant consisting of 12 parallelepiped beds (C × C = 0.4 × 0.4 m2;H = 0.6 m) filled from bottom to top with 0.1 m of gravel and 0.4 m of shale or laterite of different grain sizes with two replications. During the six months of operation, plant biomass and stem diameter of Pennisetum purpureum used as vegetation in the CWs were determined. Raw and treated water were also sampled and analyzed for pollutants, including chemical oxygen demand (COD), biochemical oxygen demand (BOD5), total Kjedahl nitrogen (TKN), total phosphorus (TP), and total suspended solids (TSS), using International Organization for Standardization (ISO) analytical methods. P. purpureum developed much better in the CW beds lined with shale;plant biomass ranged from 13.8 to 14.7 kg/m2 and from 11.2 to 12.5 kg/m2 in the beds lined with shale and laterite, respectively, as did stump diameter, which ranged from 15.5 to 16.1 cm and from 11.10 to 12.7 cm, respectively. However, the highest values for biomass and stump diameter for each material were obtained in the beds lined with 1 - 3 mm geomaterials. Pollutant removal efficiencies were highest in the CWs lined with laterite and shale of 1 - 3 mm grain size (76.9% - 83% COD, 78% - 84.7% BOD5, 55.5% - 72.2% TKN, 58.4% - 72.4% TP, 78.1% - 80.2% TSS), with the highest values recorded in the shale-lined beds. Adsorption of Arsenic by Laterite, Sandstone and Shale in a Fixed-Bed Column

The removal of arsenic from water is essential for the protection of public health. To investigate the adsorption capabilities of laterite, sandstone, and shale for the removal of arsenic from aqueous solutions, column experiments were conducted. In this study, raw materials and heat-treated (calcined) materials were examined. The experiments assessed the influence of various parameters, including initial concentration, bed depth, and the effects of heat treatment. The findings revealed that the breakthrough curves were influenced by the initial concentration of arsenic, the depth of the bed, and the type of material used. For an initial arsenic concentration of 5 mg/L, columns containing 85 cm of calcined laterite, sandstone, and shale produced volumes of 7460 ml (1492 min), 3510 ml (702 min), and 4400 ml (880 min) of water with arsenic levels below 0.01 mg/L, respectively. These calcined materials demonstrate significant potential for the effective removal of arsenic from water.

Epidemiological, Clinical and Therapeutic Aspects of Acute Respiratory Distress in Children in Medical Emergencies at the Bangui Pediatric University Hospital

Introduction: Respiratory distress is a clinical condition accompanied by an increase in work of breathing, with the respiratory accessory muscles brought into play to ensure normal arterial oxygenation. It is a major cause of morbidity and mortality in pediatrics. The aim of our study was to investigate the epidemiological, clinical and therapeutic aspects of respiratory distress in children aged between 1 month and 15 years seen in the emergency department of the Bangui paediatric university hospital. Methodology: This was a 3-month descriptive cross-sectional study, from January 1 to March 31, 2023. All children aged 1 month to less than 15 years presenting with respiratory distress were included. Results: A total of 3021 children were admitted to the emergency medical services of Bangui’s pediatric university hospital. Of these, 164 were included in the study. The predominance was male, with a sex ratio of 1.09. The 0 - 2 age group was the most represented, with 67 patients (42.85%). The majority of patients came from Bangui, accounting for 146 (89.02%) of cases. Respiratory difficulty (59.15%), characterized by dyspnea and cough, associated with fever, vomiting, physical asthenia and diarrhea, were the main reasons for consultation. The main pathologies noted were respiratory 92 (56.10%), followed by cardiac pathologies 21 (12.8%). Antibiotic administration (76.82%) was the most common therapeutic procedure used in the management of respiratory distress. Conclusion: Respiratory distress remains an important cause of infant mortality in our context, with major management problems.

Factors Influencing the Continuum of Care for Children and Adolescents Treated for HIV at the Pediatric Teaching Hospital of Bangui

Introduction: Reducing and maintaining viral load is crucial to reducing morbidity and mortality associated with HIV infection in children. The aim of this study was to determine the factors influencing the maintenance of children and adolescents on antiretroviral therapy in the continuum of care. Methodology: This was a descriptive and analytical cross-sectional study conducted from August 1 to August 31, 2023. It included all children living with HIV, under 15 years of age, with at least two viral load results and whose parents consented to participate in the study. Participants were recruited during their child’s treatment renewal consultations. Results: The study included 143 children, mostly boys (55.2%), with a mean age of 11.54 years (±2.8). More than half (55.2) were unaware of their HIV-positive status, and most treatments were administered by parents (60.8%). Of the 99 children with an undetectable viral load at the first test, 23 (23.2%) showed a viral rebound at the last test, mainly due to poor treatment compliance (p Conclusion: Virological rebound after suppression in children is worrying. It is crucial that the national AIDS program improves Therapeutic Education, trains health workers to communicate results and encourages ongoing dialogue with young people to reinforce adherence and maintain viral suppression.

Density Results in Sobolev Spaces Whose Elements Vanish on a Part of the Boundary

This paper is devoted to the study of the subspace of Wm＇＂ of functions that vanish on a part γ0 of the boundary. The author gives a crucial estimate of the Poincare constant in balls centered on the boundary of γ0. Then, the convolution-translation method, a variant of the standard mollifier technique, can be used to prove the density of smooth functions that vanish in a neighborhood of γ0, in this subspace. The result is first proved for m = 1, then generalized to the case where m 〉 1, in any dimension, in the framework of Lipschitz-continuous domain. However, as may be expected, it is needed to make additional assumptions on the boundary of γ0, namely that it is locally the graph of some Lipschitz-continuous function.

After the microbiology based on Pasteur's method and polymerase chain reaction(PCR), the diagnosis company named Dendris has proposed a third-generation of diagnosis enabling the search of a broad range of pathogens with strong sensitivity and specificity. This extraordinary profile was possible thanks to the use of phosphorus dendrimers for which various techniques of deposition on a given support were investigated and described and analyzed in this report.

Red osier dogwood(Cornus sericea L.),widely distributed throughout North America,is essential for wildlife,thus *** is recommended for reclamation or revegetation of sites disturbed by oil and gas extraction because it tolerates a wide range of soil types and high pH *** germination of this species is extremely difficult with long stratification requirements and poor germination,cuttings facilitate *** this study,to develop techniques to propagate the species from stem cuttings,four concentrations of indole-3-butyric acid(IBA)and two cutting ages previous year’s growth(PYG)and current year’s growth(CYG)were investigated for survival and *** 4 months,survival rate,height growth,and shoot biomass from PYG cuttings were enhanced with IB *** contrast,IB A did not affect these parameters in CYG *** morphology was significantly affected by IB A concentration and cutting ***,at higher concentrations,increased root surface and *** root surface area and length of PYG cuttings were increased,which may facilitate the absorption of essential resources and consequently increase *** results indicate that increasing the concentration of IB A may be an effective way to have better plant survival and growth of previous year’s cuttings in red osier dogwood.
