Study of Factors Associated with the Practice of Vaccination against COVID-19 among Medical Science Students in Dakar (Senegal)

Introduction: Almost a year after the start of the COVID-19 pandemic, Senegal embarked on a vaccination campaign to protect its population. The objective of this study was to identify the factors influencing the COVID-19 vaccination practices within a health training university in Dakar. Methodology: This was a cross-sectional, descriptive and analytical study carried out from October 21 2021 to February 21 2022 among students at the Elhadj Ibrahima Niass private University in Dakar, Senegal. A 28-question online questionnaire was sent to all students enrolled at the university during this period. Results: Of the 576 students who responded, the average age was 22.60 years, with females predominating (57.81%). Medical students accounted for 66.14% of participants, pharmacy 22.40% and dental surgery 11.46%. Of these, 42.01% were bachelor’s students, 29.51% master’s students and 28.47% doctoral students. Vaccination coverage was 50.35%. Students who considered COVID-19 to be very dangerous were more likely to be vaccinated (OR = 5.05 [2.24-11.9]). Those with poor knowledge of vaccines were less likely to be vaccinated (OR = 0.07 [0.03-0.18]), as were those with poor knowledge of contraindications to vaccination (OR = 0.49 [0.28-0.86]). No association was found between vaccination status and socio-demographic or educational characteristics. Conclusion: In view of the importance of knowledge about COVID-19 vaccination, our results suggest that it is important to involve health students in the Ministry of Health’s awareness-raising strategies, because their support is necessary for better public awareness.

The Role of Music Therapy in Supporting Intellectually Disabled Youth in Senegal

Introduction: Music therapy is a practice for helping and supporting people with intellectual and relational difficulties. This study illustrated the benefits of music therapy for young people living with intellectual disabilities (YLID) in an African context. Methodology: This study investigated six young individuals with intellectual disabilities who had undergone three years of music therapy. They were participants in the inclusive non-academic training program at the National School of Arts in Dakar from 2017 to 2019. Data collection utilized individual interviews with the youths, evaluation grids from teachers and psychiatrists. Guardians provided informed consent along with the assent of the young participants. Results: The six young were aged between 18 and 30 years old, with an average age of 24.6 years. Four of the YLID were male. Three young people with intellectual disabilities had delayed psychomotor development. Observations revealed the beneficial influence of music therapy on the health and well-being of young individuals. Music played a role in alleviating stress and anxiety among youth with intellectual disabilities (YLID), enhancing their mood and mental health. It assisted in navigating challenging situations and heightened alertness among YLID. Additionally, music therapy contributed to improvements in dyslexia, fine and gross motor skills, and memory development among intellectually disabled youth, ultimately facilitating their integration into society. Conclusion: In light of our results, music therapy makes a major contribution to the empowerment of YLID. Engaging in musical activities helps young people connect with others through instrumental expression and a sense of accomplishment. By facilitating music therapy, it becomes possible to combat discrimination and stigmatization, thus promoting the social inclusion of intellectually disabled youth. Therefore, it is important to promote music therapy in Senegal to meet the needs of YLID.

Impact of COVID-19 in the Hospital Activities of a Cardiology Department: Case of the General Hospital Idrissa Pouye in Dakar, Senegal in 2020

Introduction: In Senegal, at the end of 2020, there were a cumulative 18,728 confirmed cases of COVID-19 and 390 deaths. Patients with cardiovascular disease were the most affected by this morbidity and mortality. This is why we were given the objective of analyzing the effect of COVID-19 on the hospital activities of a cardiology service in Senegal. In this case that of the General Idrissa Pouye Hospital (HOGIP), by comparing admissions, deaths, average monthly length of stay and overall annual mortality of the service in 2020 with each of the four previous years. Methodology: This was a descriptive study from January 2016 to December 2020 by retrospective data collection. The collection ran from January 5, 2021 to March 30, 2021. Statistical analysis was performed with Rstudio 4.1.0 statistical analysis software. We expressed these variables as their absolute values and then we calculated the difference between the absolute value of 2020 and that of each of the previous years, both on a monthly and annual scale for each of the variables. Finally, we calculated the annual global mortality (each year from 2012 to 2020). Results: The total number of admissions in 2020 was significantly lower than in 2016 (-7.59%), 2017 (-8.78%) and 2018 (-2.16%). On the other hand, it was clearly higher than that of 2019 (+12.83%). A record in admissions was found during the month of December 2020 compared to 2016 (+64.70%), 2017 (+95.34%), 2018 (+127.02%) and 2019 (+127.02%). The average monthly length of stay in 2020 was less than or equal to those of 2016 (-7.62%), 2017 (-8.57%), 2018 (-3.03%) and 2019 (-0.00%). The total number of annual deaths in 2020 was significantly lower than in 2016 (-24.39%), 2017 (-27.06%), 2018 (-39.81%) and 2019 (-23.46%). Endocarditis is an inflammation of the endocardium and its structures (valves), most often of infectious origin, described by William Osler in 1885. In the 21st century, infective endocarditis remains a reality in our countries. We report a complicated case of infective endocarditis (IE). This is a 53-year-old woman, obese and passive smoker who died on the 5th day of her hospitalization following an infective endocarditis (IE) with bacterial strains resistant to the usual antibiotics: daughter of acute lithiasic cholecystitis, mother of major mitral valve perforation, brain abscess, ischemic stroke and atrial fibrillation. All were responsible for septic shock and fatal coma. Surgical management of the infective endocarditis in the first hours of her admission could have improved her prognosis. To conclude, in addition to its interests and its clinical particularities, our present observation has highlighted major public health problems specific to our sub-Saharan African countries, namely: The problem of the double health burden, the problem of delays in seeking care, the problem of resistance to antibiotics and the problem of the insufficiency of reference health technical platforms.

Assessment of Women’s Dietary Diversity in Southern Senegal

Introduction: Lack of dietary diversity is a nutritional problem among poor people in developing countries whose daily diet is usually based on a starch source with one or two additional components. Senegal has food insecure regions such as Kolda and Kedougou despite improved agricultural production. This study aims to investigate the determinants of dietary diversity in the Kolda and Kedougou regions. Methodology: It was a cross-sectional, descriptive and analytic household-level study in two southern regions of Senegal (Kolda and Kedougou) between October and December 2015. This was a three-stage random sample survey. Dietary diversity in women of reproductive age was measured using the Women’s Dietary Diversity Score (WDDS) recommended by Food and Agriculture Organization of the United Nations (FAO). The WDDS is a global qualitative indicator of nutrition, which counts the number of food groups consumed by a person in the 24 hours preceding the survey. The data was collected during an individual interview. Linear regression was used to identify the associated factors of dietary diversity. The free and informed consent of the individuals surveyed and the protection of anonymity were respected. Results: A total of 1926 women were surveyed. The mean of WDDS was 2.9 with a standard deviation of 1.4. The Kedougou department has a better WDDS with 3.3 (±1.2) followed by the Kolda department with a score of 3.2 (±1.3). The higher the household’s welfare score, the better the WDDS with a ßcoefficient of 0.11 [0.09 - 0.13]. Households that are in urban areas have a better score than households in rural areas (0.27 [0.13 - 0.41]). The improvement of knowledge in food diversification, in good culinary practice improves the dietary diversity score in women. Conclusion: The WDDS is low in the southern regions of Senegal. Study of the Determinants of Dietary Diversity among Pregnant Women in the Dakar Region (Senegal) in 2017

Introduction: Many health problems in pregnant women result in part from inadequate nutrition. Much progress has been made in improving the nutritional situation in Dakar, but it remains a concern to this day. Promoting dietary diversity is an effective way to combat any kind of nutrient deficiency. This study aims to investigate the determinants of dietary diversity in pregnant women in the Dakar region. Method: It was a cross-sectional, descriptive, analytical study with a three-stage survey. The calculated sample was 292 pregnant women. Socio-demographic, economic, pregnancy-related data, related to practices and knowledge on nutrition were collected using a questionnaire and an observation grid. The collection was conducted from November 01 to December 15, 2017 among pregnant women in the Dakar region. Data were entered using epi info software and analyses were done with R. Simple linear regression was used to find the explanatory factors of dietary diversification. The fit of the regression model was done by the Hosmer Lemeshow test. The odds ratios were measured with a 5% alpha risk. Results: A total of 300 pregnant women were surveyed in two different health districts. Study of the Determinants of the Use of Breast Cancer Screening in Women 35 to 65 Years in the Region of Thies (Senegal)

Introduction: In Senegal, breast cancer is the second most common cancer of women after cervical cancer. It is a cancer accessible to clinical and radiological screening. The objective of this study was to study the determinants of the use of breast cancer screening among women aged 35 to 65 in the Thies region in 2015. Material and Methods: It was a descriptive and analytical cross-sectional study. It was conducted with a sample of women selected using a two-stage cluster survey. The study used a questionnaire to collect data on the individual characteristics of women, knowledge, attitudes and practice on breast cancer and quality of care in an individual interview at the homes of women after consent. The data was entered on the Epi Info 3.5.3 software and analyzed with the R 3.3.3 software. A description of qualitative variables across proportions and quantitative variables by mean and standard deviation was performed. Parametric or non-parametric tests were used to examine the association between the dependent variable (breast cancer screening) and explanatory variables (predisposing factors, resources, health needs) according to their applicability conditions and at a risk α equal 5%. Finally, all variables with p less than 0.25 were used to construct an explanatory model by simple logistic regression. Results: The study involved 960 women aged 35 to 65 years. The average age was 43.8 years with a standard deviation of 8.3 years. The rate of breast cancer screening was 25.7%. Educated women (OR adjusted = 1.89 [1.25 - 2.85]), women who resided in urban settings (adjusted OR = 1.79 [1.05 - 3.04]), those who experienced at least one risk factor for breast cancer (OR adjusted = 2.71 [1.81 - 4.06]) were more likely to use screening breast cancer. In contrast, women with low socioeconomic status (adjusted OR = 0.66 [0.45 - 0.97]) were less likely to be screened. Profile of Workers Tested Positive for Sars-CoV-2 in Sangalkam Health District (Senegal)

Background: COVID-19 has shown the vulnerability of our healthcare staff in terms of exposure. Staff contamination is a reality to be explored and prevented. Objective: It was to describe the epidemiological, clinical, therapeutic and evolutionary profiles of health staff infected with COVID-19 in the Health District of Sangalkam (Senegal). Materials and Methodologies: This was a retrospective study from April 1, 2020 to April 20, 2021. District health staff tested positive of Sars-CoV-2 were included. Results: Sixty-six professionals were sampled, of whom seventeen were positive (25.8%). The medical history found was diabetes type 2 (n = 1) and hypertension/obesity (n = 1). Only one victim had previously received an incomplete vaccination. The average age of the infected personnel was 38.4 years. Victims with an identified epidemiological link were more numerous (64.7%). Respiratory signs (cough, rhinorrhea) and fever predominated. Caregivers accounted for 76% of the victims, compared with 26% of the administrative staff. Most patients were followed at home (76.4%). One severe case was admitted to the intensive care unit. The recovery rate was 100%. The average length of absence was 13 days. Conclusion: Infection of the staff is of great concern. Regular and decontamination and systematic cleaning of consultation offices and hospital wards are necessary. In addition, vaccination, screening, strict compliance with barrier measures (wearing masks, hand washing, physical distancing).

Study of Factor Associated with Post Stroke Depression in the Teaching Hospital of Fann (Dakar-Senegal)

Introduction: Strokes are becoming more common in Africa, they are often accompanied by depressive symptoms, hence the need to investigate the factors associated with it for better management and prevention. Methodology: We conducted a prospective cross-sectional study amongst patients hospitalized with stroke at the Neurology/Neuroscience Clinic of the FANN UNHC (Dakar) in 2016. After a clinical diagnosis, patients are subjected to socio-demographic questionnaire and were administered with modified Rankin and MADRS scores. Results: Among the patients, 33.7% had Post-Stroke Depression, most were aged 50 to 70 years, male and married. History of stroke and depression was strongly associated with Post-Stroke Depression, while functional or cognitive impairment caused by stroke contributed to the development of Post Stroke depression. The type of ischemic or haemorrhagic stroke did not appear to affect the occurrence of the Post-Stroke Depression, but Post-Stroke Depression was more frequent in subjects with a left hemispheric lesion 47.8% (p Discussion: The occurrence of a Post-Stroke Depression is a factor of poor prognosis, early diagnosis and holistic care lead to improved quality of life.

Perceptions of Actors on the Community-Based Maternal and Neonatal Health Services Project at the Kolda and Sedhiou Regions

Introduction: The health of mothers and children remains a major concern in developing countries. Infant and maternal mortality rates, although declining, are still high. Access to health services is a major obstacle to reducing maternal and infant mortality. To support the Ministry of Health and Social Action in the implementation of its Strategic Plan for Reproductive Health (2012-2015), the NGO Micronutriment Initiative has developed a project based on community-based maternal and neonatal health services (CBMNHS) in the Kolda region. The general objective of this study is to collect the perceptions of the actors on the Community-Based Maternal and Neonatal Health Services project in the Kolda and Sedhiou regions. Method: This was a qualitative study consisting of an evaluation of the activities carried out within the framework of the CBMNHS project at the level of the intervention zone. The study focused on support groups, community actors, post nurses and implementers of the intervention. Sampling was comprehensive for community actors and nurse heads. Focus groups and in-depth individual interviews were used to assess the perceptions of women and support groups, as well as to better understand the dynamics of the intervention. The content analysis was performed to exploit the qualitative data. Results: All local and national health authorities interviewed are unanimous about the relevance of the project. The project’s strategies are essentially community-oriented. Thus, a large number of committees have been set up. A total of 1258 support groups were set up during the project, distributed as follows: 260 grandmother groups (Grandmothers Strategy);266 groups of pregnant women (Pregnant Women’s Solidarity Circles);248 Care Group;468 community watch and alert circles and 16 Future Fathers’ Groups (Future Fathers’ Solidarity Circles).
