International Healthcare Terminologies for Morbidity New Era: SNOMED and ICD11

The Information Technology (IT) developments have changed the use of Healthcare terminologies from paper-based mortality statistics with the WHO international classifications of diseases (ICD) to the IT-based morbidity implementations for instance for Casemix-based healthcare funding and managing systems. This higher level of granularity is worldwide spread under the umbrella of several national modifications named ICD10 XM. These developments have met the increased use of the International Clinical Reference Terminology named SNOMED. When the updating of WHO ICD10 to WHO ICD11 was decided a merging was envisaged and a WHO SNOMED CT common work proposed a methodology to create a common formal ontology between the 11th version of the WHO International Classification of diseases and Health Problems (ICD) and the most used in the world clinical terminology named Systematized Nomenclature of Human and Veterinary Medicine - Clinical Terms (SCT). The present work follows this unachieved work and aims to develop a SNOMED-based formal ontology for ICD11 chapter 1 using the textual definitions of ICD11 codes which is a completely new character of ICD and the ontology tools provided by SCT in the publicly available SNOMED Browser. There are two key results: the lexical alignment is complete and the ontology alignment is incomplete with the validated SNOMED concept model can be completed with not yet validated attributes and values of the SNOMED Compositional Grammar. The work opens a new era for the seamless use of both international terminologies for morbidity for instance for DRG/Casemix and clinical management use. The main limitation is that it is restricted to 1 out of 26 chapters of ICD11.

Under the current context of climate change, supplementary irrigation may be needed for crop production resilience. We determined the effects of supplementary irrigation on sorghum grain yield in the dry Savannah region of Togo. A two-year trial was conducted in a controlled environment at AREJ, an agro-ecological center in Cinkassé. The plant material was sorghum variety Sorvato 28. The experimental design was a Completely Randomized Block with three replications and three treatments as follows: T0 control plot (rainfed conditions);T1 (supplementary irrigation from flowering to grain filling stage) and T2 (supplementary irrigation from planting to grain filling stage). Two irrigation techniques (furrow and Californian system) were used under each watering treatment. The results showed that irrigation technique significantly affected panicle length with no effect on 1000 grains mass. Panicle length and grain yields varied from 15.59 to 25.71 cm and 0.0 to 2.06 t∙h−1, respectively, with the highest values (25.66 cm and 2.06 t∙h−1, respectively) under the T2 treatment with the California system-based supplementary irrigation. The comparison of results obtained on treatment T0 and T2, shows that supplementary irrigation increased the yields by at least 68.62%. Supplementary irrigation during sowing and growing season (T2) improved sorghum yields in the dry savannahs of Togo, with a better performance of the California irrigation system.

Statistical Study of the Geoeffectivity of Halo Coronal Mass Ejections Associated with X-Class Flares during Solar Cycles 23 and 24

By analysing a long series of data (1996-2019), we show that solar cycle 23 was more marked by violent solar flares and coronal mass ejections (CMEs) compared to solar cycle 24. In particular, the halo coronal mass ejections associated with X-class flares appear to be among the most energetic events in solar activity given the size of the flares, the speed of the CMEs and the intense geomagnetic storms they produce. Out of eighty-six (86) X-class halo CMEs, thirty-seven (37) or 43% are highly geoeffective;twenty-four (24) or approximately 28% are moderately geoeffective and twenty-five (25) or 29% are not geoeffective. Over the two solar cycles (1996 to 2019), 71% of storms were geoeffective and 29% were not. For solar cycle 23, about 78% of storms were geoeffective, while for solar cycle 24, about 56% were geoeffective. For the statistical study based on speed, 85 halo CMEs associated with X-class flares were selected because the CME of 6 December 2006 has no recorded speed value. For both solar cycles, 75.29% of the halo CMEs associated with X-class flares have a speed greater than 1000 km/s. The study showed that 42.18% of halo (X) CMEs with speeds above 1000 km/s could cause intense geomagnetic disturbances. These results show the contribution (in terms of speed) of each class of halo (X) CMEs to the perturbation of the Earth’s magnetic field. Coronal mass ejections then become one of the key indicators of solar activity, especially as they affect the Earth.

Context: The advent of Artificial Intelligence (AI) requires modeling prior to its implementation in algorithms for most human skills. This observation requires us to have a detailed and precise understanding of the interfaces of verbal and emotional communications. The progress of AI is significant on the verbal level but modest in terms of the recognition of facial emotions even if this functionality is one of the oldest in humans and is omnipresent in our daily lives. Dysfunction in the ability for facial emotional expressions is present in many brain pathologies encountered by psychiatrists, neurologists, psychotherapists, mental health professionals including social workers. It cannot be objectively verified and measured due to a lack of reliable tools that are valid and consistently sensitive. Indeed, the articles in the scientific literature dealing with Visual-Facial-Emotions-Recognition (ViFaEmRe), suffer from the absence of 1) consensual and rational tools for continuous quantified measurement, 2) operational concepts. We have invented a software that can use computer-morphing attempting to respond to these two obstacles. It is identified as the Method of Analysis and Research of the Integration of Emotions (M.A.R.I.E.). Our primary goal is to use M.A.R.I.E. to understand the physiology of ViFaEmRe in normal healthy subjects by standardizing the measurements. Then, it will allow us to focus on subjects manifesting abnormalities in this ability. Our second goal is to make our contribution to the progress of AI hoping to add the dimension of recognition of facial emotional expressions. Background and Objectives: The indication for treatment in HBsAg-positive patients with low viral load and normal transaminases requires an assessment of fibrosis. In resource-limited settings, free hepatic fibrosis evaluation tests can aid in therapeutic decision-making. Our study aims to demonstrate the utility of assessing hepatic fibrosis using non-invasive markers (APRI and FIB-4) in patients with chronic B viral hepatitis without cytolytic activity and low viral replication in our context. Patients and Methods: This is a retrospective cross-sectional study conducted between January 2018 and December 2021 at the University Hospital Center of Bouaké. Included were all patients aged ≥18 with normal transaminases (Results: Our study included 241 patients, with a mean age of 36.19 years (±10.52 years) and a male predominance of 52%. The mean FibroScan® value was 6.44 ± 2.3 kPa, and 68 patients (28.22%) had fibrosis >7 kPa. To exclude significant fibrosis (FS Conclusion: A significant proportion of HBV-infected patients with normal ALT and low viral load have active liver disease. Both FIB-4 and APRI biological scores are useful in identifying individuals without significant fibrosis with a good negative predictive value (>50%).

Irrigated Agriculture Facing the Challenge of Climate Change: Adaptation Strategies for Farmers in the Irrigated Perimeters of Môle Saint-Nicolas, Haiti

Môle Saint-Nicolas, like all other communes in the Republic of Haiti, faces increasing climate variability, impacting agricultural production and water resources. Consequently, there is a pressing need for adaptation to these climatic changes. This research aims to showcase the adaptation strategies deployed by farmers to cope with the increasing climate variability. Surveys were conducted through group and individual discussions with a randomly selected cohort of 150 farmers. Two types of analysis were performed: quantitative and qualitative. The quantitative data analysis was conducted using Statistical Package for the Social Sciences (SPSS) software. The findings reveal that farmers have perceived changes in rainfall patterns, temperature, wind, and their environment. These changes manifest as irregular rainfall, higher temperatures, prolonged drought periods, violent winds accompanied by rain, premature cessation of rains, and reduced flow from water sources. In response, the most common adaptation strategies adopted include selecting new cultivars, early-maturing varieties, crop rotation and diversification, canal dredging, new soil preparation methods, upstream water source protection, and micro-watershed management. The significance of this research lies in its contribution to enhancing farmers’ adaptive capacities by alerting stakeholders in the irrigated perimeters about the consequences of climate change, thereby incorporating the real needs of farmers in future projects.

Floods are phenomenon with significant socio-economic implications mainly for human loss, agriculture, livestock, soil loss and land degradation, for which many researchers try to identify the most appropriate methodologies by analyzing their temporal and spatial development. This study therefore attempts to employ the GIS-based multi-criteria decision analysis and analytical hierarchy process techniques to derive the flood risks management on rice productivity in the Gishari Agricultural Marshland in Rwamagana district, Rwanda. Here, six influencing potential factors to flooding, including river slope, soil texture, Land Use Land Cover through Land Sat 8, rainfall, river distance and digital Elevation Model are considered for the delineation of flood risk zones. Data acquisition like Landsat 8 images, DEM, land use land cover, slope, and soil class in the study area were considered. Results showed that if the DEM is outdated or inaccurate due to changes in the terrain, such as construction, excavation, or erosion, the predicted flood patterns might not reflect the actual water flow. This could result unexpected flood extents and depths, potentially inundating rice fields that were not previously at risk and this, expectedly explained that the increase 1 m in elevation would reduce the rice productivity by 0.17% due to unplanned flood risks in marshland. It was found that the change in rainfall distribution in Gishari agricultural marshland would also decrease the rice productivity by 0.0018%, which is a sign that rainfall is a major factor of flooding in rice scheme. Rainfall distribution plays a crucial role in flooding analysis and can directly impact rice productivity. Oppositely, another causal factor was Land Use Land Cover (LULC), where the Multivariate Logistic Regression Model Analysis findings showed that the increase of one unit in Land Use Land Cover would increase rice productivity by 0.17% of the total rice productivity from the Gishari Agricultural Ma
