Prehypertension, Hypertension and Associated Cardiovascular Risk Factors among Adult Congolese Urban Dwellers: Results of the Vitaraa Study

Objective: To assess the prevalence of prehypertension and hypertension, their determinants and associated cardiovascular risk factors in Congolese urban dwellers. methods: From July 2007 to march 2008, we collected information on lifestyle habits, medical history and anthropometric data in 1292 household members of Adoula Quarter, Kinshasa, aged 20 years or more, 731 women (56.6%). We obtained measurements of BP, blood glucose, serum lipids and qualitative proteinuria. We defined blood pressure categories according to JNC7 classification and used logistic regression analyses to assess their independent determinants. We obtained age adjustment of continuous and categorical variables using GLm and Genmod procedures, respectively. Results: Prehypertension was observed in 30.3% of subjects, 34.9% of men and 26.7% of women (P = 0.0045). The prevalence of hypertension amounted to 30.9% with no difference between genders. Participants with prehypertension had average age, BmI and waist circumference intermediate between those with normal BP and hypertensive subjects. Their glucose and lipids levels were similar to those of normotensives. The prevalence of prehypertension amounted to 33% at age 20 - 29 years and decreased to 16.7% at ≥60 years whereas the prevalence of hypertension increased from 11.2% to 71.4%. The rates of diabetes mellitus were similar accross blood pressure catogo- ries whilst prevalences of overweight/obesity, abdominal adiposity, dyslipidemia and metabolic syndrome significatively increased (P = 0.05 or less). Among participants with prehypertension, 73% had two or more additional cardiovascular risk factors. In the logistic model the probability of prehypertension was higher in men (OR: 1.429;95% CI: 1.099 - 1.857) and participants with overweight/obesity (OR: 1.666;1.146 - 2.422), lower in participants aged ≥ 55 years (0.427;0.267 - 0.683) and those with high fruit intake (0.691;0.488 - 0.977). Background and Objective: 24-h ambulatory blood pressure monitoring (ABPm) aids to precisely identify patients with true resistant hypertension (tRH). The present study was aimed to assess the frequency and correlates of tRH among patients with clinically suspected RH. methods: medical records of treated hypertensive patients referred in four healthcare centers for BP control evaluation by 24-h ABPm were reviewed to assess the prevalence of tRH. Inclusion criteria were age ≥ 18 years, clinical diagnosis of RH. Data on demographic, clinical, laboratory, 2D-echocardiography and 24-h ABPm parameters were retrieved from patient’s medical records. True RH (tRH) was defined as office blood pressure (BP) ≥ 140/90 mmHg and 24-h ambulatory BP ≥ 130/80 mmHg. Simple and multiple linear regression analyses were used to assess factors associated with systolic BP (SBP) as a proxy of RH among patients with tRH. P 0.05 defined the level of statistical significance. Results: Of 636 patients referred for BP control evaluation by 24-h ABPm, 75 (11.7%) had suspected RH by office BP measurements. After 24-h ABPm, pseudo or apparent RH (aRH) and tRH were observed in 15 (2.3%) and 60 (9.4%) patients, respectively. BmI (p = 0.007) and blood glucose (p = 0.024) were positively associated with SBP whereas a negative association was observed with eGFR (p = 0.022) among tRH hypertensive patients in multiple regression analysis. Conclusion: True RH was a common finding among patients with clinical RH and associated with obesity and silent target organ, especially kidney dysfunction. The present study highlights the diagnostic and prognostic importance of 24-h ABPm among patients with clinical RH.

Background: Flight and cabin crew are known to be at increased risk for atherosclerotic cardiovascular disease (ASCVD). However, ASCVD risks have not yet been compared in flight and cabin crew in low resource settings like sub-Saharan Africa. Objectives: To assess absolute ASCVD risk estimate and its clinical correlates among flight and cabin crew. methods: From June 1st 2015 to December 30th 2015, 379 consecutive aviation navigants (Flight crew: 62.5%, pilots: 46.2%, women: 29.6%, Caucasians 23.2%) were enrolled in a cross-sectional survey of ASCVD risk estimate using the Framingham tools. They underwent a physical examination for either initial or renewal medical certificate Class 1 or 2 including blood chemistry, ECG, and echocardiogram as per International Civil Aviation Organization (ICAO) and Civil Aviation Authority (CAA-DRC) medical regulations. We modeled the risk of moderate and high ASCVD estimate in a stepwise logistic regression. Results: Low, moderate and high ASCVD risk estimates were observed respectively in 248 (65.4%), 64 (16.9%), and 67 (17.7%) navigants. moderate and high ASCVD risk estimates predominated among flight than cabin crew (23.6% vs. 5.6%;p 0.0001 and 28.3% vs. null;p 0.001), low ASCVD risk estimate among cabin than flight crew (94.4% vs. 48.1%;p ≤ 0.001). Efficiency and productivity analysis of vegetable farming within root and tuber-based systems in the humid tropics of Cameroon

The study analyzes the technical efficiency of vegetable farmers in root and tuber-based farming systems within selected sites of the humid tropics of Cameroon. multistage sampling was used to collect primary data from a cross-section of vegetable farmers drawn from eight selected sites in Santa sub division, Northwest region of Cameroon. Stochastic frontier analysis was used to estimate the technical efficiency of vegetable farmer and to examine its determinants. The results showed that farmyard manure was the most productive factor input, followed by farm equipment and labor. The mean technical efficiency level was 67%, revealing production shortfalls and indicating possibilities of significantly increasing production with the current input Jevels. Female, as well as more educated farmers were found to be significantly more efficient than their counterparts. The results also showed that farmers become less technical efficient as farm sizes become larger. Our study findings suggest that smallholder farmers＇ access to manure, farm implements, and increased women participation in vegetable farming, will produce huge payoffs in vegetable production efficiency in Cameroon.

Analysis of Flood Risks Upstream from the Imboulou Hydroelectric Dam on the Léfini River, a Tributary of the Right Bank of the Congo River

The analysis of the risk of flooding upstream from the Imboulou hydroelectric dam on the Léfini River in the Republic of Congo-Brazzaville, focused on the evolution of annual rainfall and flow in the study area during the period from 1970 to 2020 before and after the building of the dam in 2005, by applying statistical methods. These methods were used to analyse the spatial and temporal evolution of rainfall and flow at the unique hydrometric station located at the RN2 (National Road N°2) bridge in the village of mbouambé in the Pool region. This work has shown that rainfall is not the cause of flooding in the Léfini catchment area. The monthly flow coefficient (mFC) showed exceptional flooding from November and December onwards after the dam was built, resulting in a variability of flows, with periods of high and low water. In addition, the annual average flow (AAF) and the maximum average flow (mAF) increased after the dam was built. maximum average flows (mAF) were higher than annual average flows (AAF) throughout the period of study (1970-2020). The annual and monthly rainfall-runoff relationship showed changes after the dam was built, particularly from 2009 and during the months of November and December. .

Hsp90 and ECm29 Are Important to maintain the Integrity of mammalian 26S Proteasome

The proteasome is a protease complex composed of a core particle (CP) and regulatory particles (RPs) that bind to both ends of the CP. ECm29 is a protein that associates with the proteasome and is involved in the maintenance and regulation of the proteasome assembly. However, ECm29 deficient mice can form functional proteasome. In this paper we sought to identify the mechanisms and/or proteins that help and allow the maintenance of the proteasome activity in the absence of ECm29. We analyzed the proteasome components of ECm29-deficient mice and identified Hsp90 as a protein associated with the proteasome. Furthermore, the inhibition of Hsp90 led to a partial disassembly of the proteasome only in ECm29-deficient cells. Those findings attest to the importance of Hsp90 for the maintenance of the proteasome integrity in the absence of ECm29.

Synthesis of Branched Poly(butylene succinate):Structure Properties Relationship

A series of branched poly（butylene succinate）（PBS） were synthesized with several branching agents namely trimethylol propane（TmP）, malic acid, trimesic acid, citric acid and glycerol propoxylate. The structure of the branched polymers was analyzed by SEC and ~1H-NmR. The effect of branching agent structure on crystallization was also investigated and played a significant role. Isothermal studies showed that glycerol propoxylate could act as a nucleating agent. By contrast high content of TmP disturbed the regularity of the chain and hindered the crystallization of PBS. From the non-isothermal kinetic study, it was found that glycerol propoxylate increased noticeably the crystallization rate due to the flexible structure of the branching agent. A secondary nucleation was observed with glycerol propoxylate attributed to the crystallization of amorphous fraction included between crystallites formed at the primary crystallization. Chain topology was obtained through rheological investigations and the synthesized polymers showed a typical behavior of a mixture of linear and randomly branched PBS. The incorporation of branches improved the processability of PBS for film blowing application and the modulus and the stress at break of the resulting film were significantly increased.
