Global dynamics of a model of hepatitis B virus infection in a sub-Saharan African rural area

We formulate and systematically study a deterministic compartmental model of Hepatitis *** model has some important and novel features compared with the well-known basic model in the ***,it takes into account the differential susceptibility that follows the vaccine formulation employing three-doses *** points up the HbeAg status of carriers,their levels of viral replication,the fact that treatment being not curative is recommended only to a small proportion of chronic carriers,and finally the fact that only inactive carriers are able to recover from *** model has simple dynamical behavior which has a globally asymptotically stable disease-free equilibrium when the basic reproduction number R0≤1 and an endemic equilibrium when R0>*** the use of Lyapunov functions,when it exists,we prove the global asymptotic stability of the endemic equilibrium under some *** data from Tokombere,a rural area in Cameroon,numerical simulations are *** numerical simulations first confirm analytical results,second they suggest that a policy based on treatment could not significantly impact the course of the ***,they show as it is well known that vaccination is a very effective measure to control the ***,they show that neonatal vaccination influences more the course of infection than mass vaccination ***,they picture how much loss between consecutive doses of vaccine could be ***,it is suggested that for a Sub-saharan African rural area,two-thirds of expected incidence of Hepatitis B virus infection and one third of expected prevalence of chronic carriers could be averted by 2030 if the birth dose vaccination becomes systematic and if mass vaccination rate increases to up 10%.

AIM:To assess the adoption of Carbon dioxide(CO2)insufflation by endoscopists from various European countries,and its ***:A survey was distributed to 580 endoscopists attending a live course on digestive ***:The response rate was 24.5%.Fewer than half the respondents(66/142,46.5%)were aware of the fact that room air can be replaced by CO2 for gut distension during endoscopy,and 4.2%of respondents were actually using CO2 as the insufflation *** aware of the possibility of CO2 insufflation mentioned technical difficulties in implementing the system and the absence of significant advantages of CO2 in comparison with room air as barriers to adoption in daily practice(84%and 49%of answers,respectively;two answers were permitted for this item).CONCLUSION:Based on this survey,adoption of CO2 insufflation during endoscopy seems to remain relatively exceptional.A majority of endoscopists were not aware of this possibility,while others were not aware of recent technical developments that facilitate CO2 implementation in an endoscopy suite.

Actigraphy: A new diagnostic tool for hepatic encephaiopathy

AIM： To assess the actigraphy, an ambulatory and continuous monitoring of wrist motor activity fitted to study sleep/wake patterns in hepatic encephalopathy （HE）. METHODS： Twenty-five cirrhotic patients （17 M, 8 F, mean age 56± 11 years, 24/25 alcoholic, Child-Pugh A, B, C： 2, 6, 17） were included. The patients were classified into 3 groups： stage 0 group （n = 12）, stage 1-2 group （n = 6）, and stage 3-4 group （n = 7） of encephalopathy. Over three consecutive days, patients had clinical evaluation 3 times a day with psychometric test, venous ammoniemia, flash visually evoked potentials （VEP）, electroencephalogram and continuous actigraphic monitoring for 3 d, providing 5 parameters： mesor, amplitude, acrophase, mean duration of activity （MDAI） and inactivity （MDII） intervals. RESULTS： Serum ammonia and VEP did not differ among the 3 groups. Electroencephalography mean dominant frequency （MDF） correlated significantly with clinical stages of HE （r=0.65, P=0.003）. The best correlation with HE stage was provided by actigraphy especially with MDAI （r= 0.7, P〈10^-4） and mesor （r= 0.65, P〈 10^-4）. MDAI correlated significantly with MDF （r= 0.62, 0.004） and was significantly shorter in case of HE compared to patients without HE （stage 0： 5.33± 1.6 min; stage 1-2：3.28±1.4 min; stage 3-4：2.52±1.1 min; P〈0.05）. Using a threshold of MDAI of less than 4.9 min, sensitivity, specificity, positive predictive value, negative predictive value for HE diagnosis were 85%, 67%, 73% and 80%, respectively. CONCLUSION： Actigraphy may be an objective method to identify HE, especially for early HE detection. Motor activity at the wrist correlates well with clinical stages of HE. MDAI and mesor are the most relevant parameters.

Monitoring absolute vertical land motions and absolute sea-level changes from GPS and tide gauges data over French Polynesia

Strong Spatial Aggregation of Martian Surface Temperature Shaped by Spatial and Seasonal Variations in Meteorological and Environmental Factors

Spatio-temporal variation in the Martian surface temperature(MST)is an indicator of ground level thermal processes and hence a building block for climate ***,the distribution of MST exhibits different levels of spatial aggregation or heterogeneity,and varies in space and ***,the effect of regional differences in meteorological or environmental factors on the MST is not well ***,we investigated the degree of spatial autocorrelation of MST across the surface of Mars globally by Moran’s I,and identified the hot spots by GetisOrd G;*.We also estimated the regional differences in the influence of seasonally dominant factors including thermal inertia(TI),albedo,surface pressure,latitude,dust and slope on MST by a geographically weighted regression *** results indicate(1)that MST is spatially aggregated and hot and cold spots varied over time and space.(2)Hemispheric differences in topography,surface TI and albedo were primarily responsible for the hemispheric asymmetry of hot spots.(3)The dominant factors varied by geographical locations and *** example,the seasonal Hadley circulation dominates at the low-latitudes and CO;circulation at the high-latitudes.(4)Regions with extreme variations in topography and low TI were sensitive to meteorological and environmental factors such as dust and CO_(2)*** conclude that the spatial autocorrelation of MST and the spatial and seasonal heterogeneity of influencing factors must be considered when simulating Martian climate *** work provides a reference for further exploration of Martian climatic processes.

Background: Tropical forests play a fundamental role in the provision of diverse ecosystem services, such as biodiversity,climate and air quality regulation, freshwater provision, carbon cycling, agricultural support and culture. To understand the role of forests in the carbon balance, aboveground biomass(AGB) estimates are needed. Given the importance of Brazilian tropical forests, there is an urgent need to improve AGB estimates to support the Brazilian commitments under the United Nations Framework Convention on Climate Change(UNFCCC). Many AGB maps and datasets exist, varying in availability, scale and coverage. Thus, stakeholders, policy makers and scientists must decide which AGB product, dataset or combination of data to use for their particular goals. In this study, we assessed the gaps in the spatial AGB data across the Brazilian Amazon forests not only to orient the decision makers about the data that are currently available but also to provide a guide for future ***: We obtained a map of the gaps in the forest AGB spatial data for the Brazilian Amazon using statistics and differences between AGB maps and a spatial multicriteria evaluation that considered the current AGB datasets. The AGB spatial data gap map represents areas with good coverage of AGB data and, consequently, the main gaps or priority areas where further biomass assessments should focus, including the northeast of Amazon State, Amapá and northeast of Pará. BACKGROUND Secondary forms and hypertension-mediated organ damage(HMOD) may differ between younger and older hypertensive *** aim of the present study was to explore the specificity of HMOD and secondary forms in patients≥ 65 years in comparison to younger ones in a contemporary *** We analysed 938 patients recruited between 2004 and 2014(Cardiology department,Croix-Rousse Hospital,Lyon)who had at baseline HMOD and secondary forms screening among them 190 were≥ 65 *** The mean(2.1±0.8 vs.1.2±0.9,P 10 m/s was the most frequent HMOD in patients≥ 65 years(90.1%),while echocardiographic left ventricular hypertrophy was the most common in the younger patients(45.0%).Among ECG left ventricular indexes,only R wave in aVL lead was significantly more frequently observed in patients≥ 65 years(32.6%) than in younger ones(19.0%,P <0.001).The frequency of secondary hypertension was not significantly different between younger and older patients(respectively;30.5% vs.27.8%,P=0.487).The most frequent aetiology was primary aldosteronism regardless of age,followed by renovascular hypertension(6.3% vs.3.3%,P=0.038).Among older patients,3.2% were treated with adrenalectomy and 6.3% with percutaneous transluminal renal *** Extensive screening of HMOD in older patients may be questionable as nearly all patients had one;aetiology must however be explored as a third of older patients had a secondary form.

Case studies of earthquake-induced effects on concrete channels

Case studies are presented for two concrete lined channels shaken by strong ground motions during the 1994 Northridge earthquake, the high speed channel （HSC） and bypass channel （BC）, on the Los Angeles Department of Water and Power＇s Van Norman Complex. Performances of these two channels as they pass through different subsurface materials identify several important seismic aspects and the critical role of geotechnical earthquake engineering in assessing channel behaviors. Preliminary evaluations using detailed mapping of channel cracks, permanent ground movements, subsurface profiles, and nearby strong ground motion recordings from the 1994 earthquake show that the channel liners may have been damaged from both transient motions and permanent ground movements. Damage from permanent ground deformations is obvious by observation; therefore simplified analyses are presented only for transient movements. Site specific transient response analyses are performed to provide an initial assessment of the differing effects from transient and permanent ground movements on HSC and BC liner damage. These case studies are helpful for introducing the potential for lifeline damage from transient movements within zones of permanent ground movement, a concept not well understood in the earthquake engineering community
