Introduction: Gastroesophageal reflux disease (GERD) is a benign and chronic disease that can impair the quality of life. Objective: To study the epidemiological, clinical aspects and factors associated with typical GERD in the general population of Parakou in the Republic of Benin. Methods: This was a descriptive and analytical cross-sectional study with prospective data collection which took place from May 1, 2019 to June 30, 2019. The study population consisted of any subject aged at least 15 years and living in Parakou. Subjects aged at least 15 years, present on the day of the survey and having given their free and informed verbal consent were included. The Short form of QRS®(Reflux-Qual Short form) questionnaire and the “Hospital Anxiety and Depression” score were used to assess the quality of life and anxiety/depression, respectively. The sampling was probabilistic through a 2-stage cluster sampling. The variable of interest was the presence of GERD. Results: Out of 390 subjects surveyed, 28 had symptoms of typical GERD, representing a prevalence of 7.18%. The average age of these subjects was 27.11 ± 7.88 years. Fifteen subjects (53.57%) were male, giving a sex ratio of 1.15. Fourteen (50%) had a secondary level of education, 12 (42.86%) were pupils/students, 18 (64.29%) were single. The monthly income of 22 subjects (78.57%) was lower than the guaranteed interprofessional minimum wage (40,000 FCFA). The average duration of progression of symptoms was 15.89 ± 4.97 months. Symptoms occurred less than twice a week in 22 subjects (78.57%). The clinical manifestations were both diurnal and nocturnal in 12 subjects (42.86%). None of the 28 subjects with GERD had clinical warning signs. Asthma, diabetes, consumption of high fat or spice foods, carbonated drinks, non-steroidal anti-inflammatory drugs, alcoholic beverages and tobacco were statistically associated with the occurrence of GERD. Introduction: Radiological investigation which is the gold standard to perform following anastomotic urethroplasty in order to evaluate the quality of micturition is costly. In our context, due to financial limitations, reconstructive urologists prefer to use the uroflowmetry in order to assess the micturition post-surgery. Therefore, the objective of the study was to assess the quality of micturition using the uroflowmetry after anastomotic urethroplasty. Methodology: We conducted an 11-year retrospective review (1st January 2006-31st December 2017) and a cross-sectional descriptive observational study for a period of 8 months (November 2017-June 2018) at the Urology and Andrology Department of the Yaounde Central Hospital (YCH) of patients who were diagnosed with urethral stenosis and underwent an anastomotic urethroplasty at the YCH. We excluded patients who had incomplete files, patients lost to follow-up and did not do pre-operative uroflowmetry. Quality of micturition was evaluated using a uroflowmetry. Data was analyzed using EPI info 7.0. Parametric variables were reported as means and standard deviations and percentages and counts were used to report categorical variables. Results: We had a sample of 60 patients. The mean age was 42 ± 5 years with extremes ranging from 20 to 76 years. Twenty-seven (27) patients, or 45%, had no post-operative complications, and those who did had a urinary tract infection (26.70%). In our series, we had 82% excellent results (patient satisfied with his urination with bell-shaped urinary stream and urinary flow greater than or equal to 15 ml/sec);15% good results (patient with moderate dysuria with average urinary stream and urinary flow between 10 and 14.9 ml/sec) and 3% poor results (severe dysuria with urinary flow less than 10 ml/sec, urinary retention or urinary incontinence). Background: Infertility is a complex disorder with significant psycho-social and economic consequences. It globally affects 10% - 15% of couples. In Cameroon, little is known about what women do to overcome the psychosocial aspects of the disease. Objectives: This study aimed to identify the support systems and coping strategies of infertile women attending the outpatient consultation unit of the Gynaecological Endoscopic Surgery and Reproductive Teaching Hospital (CHRACERH), Yaoundé, Cameroon. Methods: A hospital-based cross-sectional study was conducted from the 14th of March to the 6th of April 2023 at CHRACERH Yaoundé. A total of 190 participants were recruited using a convenience sampling method. Data regarding socio-demographic characteristics, support systems and coping strategies were collected using a pretested questionnaire. Descriptive and analytic statistics were conducted using SPSS version 25. Results: The mean age of participants was 39.52 ± 7.64 years. The majority 78.9% of participants were workers (public, private sector, or traders) and were Christians 95.8%. The most common source of psychological support was from family 76.8 and husbands 72.63%. Most of the participants 89.5% resorted to prayer and getting busy 48.4% as a coping strategy. There was no statistically significant relationship between coping strategies and psychological disorders p > 0.05. Conclusion: The main support system of participants was family, husband, and friends. Prayer, getting busy and adoption were the most common coping strategies. There is a need for the Ministry of Public Health and other stakeholders to put in place other support systems and coping strategies (FELICIA) used elsewhere and provide adequate health education and infection control to prevent infertility in Cameroon.

Hydrochemical Characterisation and Assessment of the Level of Contamination of Groundwater Collected by Private Waterworks in the Town of Moundou in the South of Chad

Groundwater is the main source of drinking water for large cities in most African countries. In Moundou, for example, the conventional groundwater supply system is failing. To compensate for this state failure, the population is building boreholes and wells, most of which tap the surface water table, generally referred to as the “water table”. The aim of this study is to characterize these waters in order to assess their level of contamination and, by extension, the degree of pollution of the water table. Major elements such as: Chloride (Cl-), Sulfate (SO42-), Nitrate (NO3-), Calcium (Ca2+), magnesium (Mg2+), sodium (Na+) and potassium (K+) were analysed by Liquid Chromatography and the Bicarbonate ion (HCO3-) was determined by the titrimetric method. The methodology applied is based on a combination of hydrochemical techniques and statistical analysis (PCA and CHA). A sampling campaign was carried out during high-water periods. The results of the physico-chemical analyses show mineralization ranging from 7.29 to 3670 μS/cm, with an average of 487.44 μS/cm. The groundwater studied is generally acidic, with a pH ranging from 3.26 to 6.41. Based on their anions, they are classified into four main hydrochemical facies: chloride and sulphate facies, calcium and magnesium facies, sodium and potassium facies and bicarbonate facies. The various correlations between major ions and statistical analyses have enabled us to identify three hydrogeochemical processes involved in water mineralization. The dominant process is silicate hydrolysis, followed by cation exchange, then anthropogenic input, which influences mineralization by polluting the water.

This study aims to characterize from a geotechnical point of view, the soils as well as the lateritic gravels along the Songololo-Lufu road route in the Kongo Central Province in the Democratic Republic of Congo (DRC). Ten soil samples and eight lateritic gravel samples were analysed and tested in the laboratory. For each sample, identification parameters were determined such as particle size analysis, natural water content, Atterberg limits (plasticity index and consistency index), but also compaction and lift parameters such as optimal water content, maximum dry density and CBR lift index. All materials and soils have been classified according to the Congolese Road Standard (NRC) and according to the American HRB classification. The test results show us that clay soils almost always contain between 70% and 90% fine fraction;the grained fraction represents less than 30% in clay samples. For lateritic gravels soils, the percentage of fine elements varies between 35% and 15%;in sand around 20%;the gravelly fraction represents a little more than 50% of the soil. The majority of soil facies encountered define a plasticity index lower than 15. As for the consistency index, we obtained values greater than 1, both for clayey soils and for gravelly soils. The classification according to NRC defined for these soils the types Ae1 and Ae2 for the clayey facies and the types GL1 and GL2 for the gravelly soils, while that of the HRB identified the classes and subclasses A-6 and A-7-6 for clayey soils, and subclass A-2-6 for gravelly soils. The optimal water content values obtained range between 10.2% and 23.10%;the maximum dry densities are between 1.66 and 2.07 t/m3 and the CBR index is between 6 and 26. As for the lateritic gravels materials of the Songololo region, the percentage of fine elements generally remains between 12% and 31%;the plasticity index is between 8 and 18;the optimal dry density is around 2 t/m3;the optimal water content is between 9.

Diastasis of the symphysis pubis is a rare postpartum complication. We report two cases of symphysis diastasis after dystocic delivery, in two patients who presented in the postpartum period with functional impotence, pubalgia and acute urine retention. A frontal radiograph of the pelvis confirmed the diagnosis, showing a symphyseal separation of 12 mm and 18 mm. In our case, management was mainly medical, with a favorable clinical outcome.

Flood Risk Assessment in the Lower Valley of Ouémé, Benin

In response to the increased frequency of flood events in recent years, it has become crucial to enhance preparedness and anticipation through precise flood risk assessments. To this end, this study aims to produce updated and precise flood risk maps for the Lower Valley of Ouémé River Basin, located in the South of Benin. The methodology used consisted of a combination of geographical information systems (GIS) and multi-criteria analysis, including Analytical Hierarchy Process (AHP) methods to define and quantify criteria for flood risk assessment. Seven hydro-geomorphological indicators (elevation, rainfall, slope, distance from rivers, flow accumulation, soil type, and drainage density), four socio-economic vulnerability indicators (female population density, literacy rate, poverty index, and road network density), and two exposure indicators (population density and land use) were integrated to generate risk maps. The results indicate that approximately 21.5% of the Lower Valley is under high and very high flood risk, mainly in the south between Dangbo, So-Ava, and Aguégués. The study findings align with the historical flood pattern in the region, which confirms the suitability of the used method. The novelty of this work lies in its comprehensive approach, the incorporation of AHP for weighting factors, and the use of remote sensing data, GIS technology, and spatial analysis techniques which adds precision to the mapping process. This work advances the scientific understanding of flood risk assessment and offers practical insights and solutions for flood-prone regions. The detailed flood risk indicator maps obtained stand out from previous studies and provide valuable information for effective flood risk management and mitigation efforts in the Lower Valley of Ouémé.

Noninvasive Respiratory Strategies in Patients with Acute Hypoxemic Respiratory Failure and COVID-19 in Gabon: A Prospective Observational Study

Importance: The best respiratory support technique to reduce intubation and mortality in patients with respiratory failure due to COVID-19 is controversial. Objective: To determine the respiratory support technique that could reduce the need for tracheal intubation and mortality in patients with respiratory failure due to COVID-19 admitted to intensive care units (ICUs) of Military’s Hospital (HIAOBO) in Gabon. Design, Setting, and Participants-Methodology: Prospective observational study over 10 months (January 2021-October 2021). We included patients admitted to intensive care for SARS Cov2 pneumonia who had benefited from available ventilatory support: high concentration face mask, High Flow Nasal cannula (HFNC), NIV (Non Invasive Ventilation), Continuous Positive Airway Pressure (CPAP). The choice was guided by the clinical condition, and the choice of the prescribing physicians. Recourse to mechanical ventilation was decided when faced with a Glasgow score of less than 13, an SpO2/FiO2 ratio ≤ 300, a FR ≥ 35/min, the impossibility of drainage of secretions. Main Outcomes and Measures: The primary outcome was the proportion of patients requiring intubation. The secondary outcomes were mortality in ICU. Results: The sample included 97 patients, the average age was55.6 years, hypertension was the main comorbidity (51.1%). Mean respiratory rate (RR) was 30.8 cycles/min, admission SpO2 was 83%, respiratory alkalosis was present in 63% of patients, mean CT involvement was 51%.Respiratory support was NIV (56.7%), CPAP (21.65%), high concentration face mask (18.55%). Sixteen percent (16%) of patients were intubated, 93% of them following failure of NIV. Mortality was 30%, mechanical ventilation was an independent risk factor for mortality. Conclusions: Non Invasive Ventilation, CPAP, and high-concentration face mask were frequently used in patients with COVID-related acute respiratory failure.
