Dissemination of Resistance Integrons and Genes Coding for Blse and Cabapenemases in the Urban Drainage network in Cote d’Ivoire

Antibiotic resistance has become a major threat to human health worldwide. Environment, particularly the water environment, has long been overlooked as a player in the antibiotic resistance cycle, although its role remains unclear. These can provide an ideal setting for the acquisition and dissemination of antibiotic resistance, as they are frequently affected by anthropogenic activities. The objective of this study was to establish a diffusion map of resistance integrons used as genetic markers of resistance associated with antibiotic resistance conferring genes (ARGs). Total DnA extracts from non-cultivable bacterial communities were used for the analyses. These communities were obtained from wastewater samples from 14 sites upstream and downstream of drainage channels or effluents in the cities of Abidjan, Bouaké, and Yamoussoukro. The results obtained correspond to the number of positives among the treated samples (n = 39). Among the genetic markers of dissemination, class 1 integrons were the most evident in 94.8% of samples in Abidjan (93.3%), Bouaké (100%) and Yamoussoukro (91.6%). Class 2 integrons and class 3 integrons were found respectively in 41% and 51% of all samples. Genes coding for β-lactamases and blaTEM was identified in almost all samples at a rate of 97.4%. A co-presence of the three genes blaTEM, blaSHV and blaCTX-M is also remarkable in the sites of the city of Yamoussoukro. Among the genes coding for carbapenemases, only blaKpC 17.94%, blanDM 30.76% and blaOXA48 38.46% were detected in the samples.

The emergence of antiretroviral resistance mutations represents a major threat to the achievement of national and global goals for the elimination of HIV-1 infection. The global strategy in 2019 in Cte d'Ivoire is a new national policy for the management of people living with HIV with the administration of dolutegravir (DTG)-based fixed-dose combination. The aim of our study was to evaluate HIV-1 resistance to antiretrovirals (ARVs) in infected adult subjects in Cte d’Ivoire in the context of a systematic switch to a DTG-based combination. Between February 2022 and October 2023, a cross-sectional survey with random sampling was conducted in 06 services caring for people living with HIV. A total of 139 participants were included in the study. Adults with a viral load ≥ 1000 copies/mL were tested for HIV-1 ARV resistance mutations. Molecular analyses were performed using protocol of AnRS-MIE (national Agency for Research on AIDS and emerging infectious diseases). The interpretation is performed by HIVGRAD (https://***/cms/grade/). The frequencies of HIV-1 resistance to non-nucleotide reverse transcriptase inhibitors (nnRTIs), nucleotide reverse transcriptase inhibitors (nRTIs), integrase inhibitors (IInTs) and protease inhibitors (pIs) were 82%, 73%, 19% and 11% respectively. The main mutations observed in the different classes were K103n (45%), M184V (64%), E157Q (19%) and L10V/M46I/A71V/I54V (6%) respectively. This study reveals the emergence of resistance to DTG-based fixed-dose combinations, favored by high rates of resistance to nRTIs and nnRTIs. This finding underlines the need for enhanced viral load monitoring and HIV-1 genotyping tests to guide the choice of nRTIs for combination therapy. In addition, monitoring for mutations to second-generation nRTIs is essential, given the scale-up of DTG-based regimens currently underway in Cte d’Ivoire.

Introduction: Acute rheumatic fever (AAR) is a non-suppurative complication of late infection by group A. Infections due to streptococci remain a public health problem in the Central African Republic. The present study aims to determine the incidence rate of AAR cases and its complications. Methodology: This was a retrospective and analytical study over a period of 4 years (from January 2015 to December 2018) at the national Laboratory of Clinical Biology and public Health (LnBCSp) in Bangui. It focused on samples concerning the diagnosis of AAR and patient files seen in consultation in the capital’s reference health establishments. Laboratory registers and patient consultation files were used to collect data. They were entered into Excel 2010 to be analyzed with Epi Info 7. A univariate analysis by logistic regression, Ficher’s exact test, and chip>2p> at the 5% threshold (p Results: We analyzed 94 cases meeting Jones’s criteria. The ages of the patients ranged from 18 to 85 years (mean age 52 years and mode 45 years). The incidence rate of AAR for the female sex was higher than that of the male sex during the study period (p > 0.05). It went from 166.6 in 2015 to 200 in 2016 and 2018 cases of AAR per 1000 people per year. The average incidence rate was 296.18 cases of AAR per 1000 people per year. The average incidence rate was 223.5 cases of AAR per 1000 people per year for joint damage. Joint damage represented 80.85% (RR = 0.62;Chip>2p> = 4.88;95% CI [0.39-0.97];p Conclusion: The study allowed data on the impact rate and complications of the AAR in Bangui. Special attention to data management will help produce a complete result on the problem. Joint complications predominated. Sustaining an effective surveillance system and preventing infection would help reduce the risk of AAR occurrence.

Vitamin D, parathyroid Hormone, Insulin Sensitivity and Islet β-Cell Secretory Function in Diabetic patients from South Kivu in the Democratic Republic of Congo: Cross-Sectional Study

Background: The role of vitamin D and parathyroid hormone in the metabolic profile of type 2 diabetes mellitus in sub-Saharan Africa has not been adequately assessed. The aim of this study was to determine the prevalence of low vitamin D level and secondary hyperparathyroidism and their association with insulin sensitivity and β-cell secretory function among Congolese type 2 diabetics. Methodology: Fasting glycaemia, fasting insulin, 25OH D3 and human parathyroid hormone (hpTH) were measured in one hundred and eighty-four type 2 diabetic patients followed as outpatients in South Kivu. Levels of 25OH D3 65 pg/ml defined low vitamin D and elevated parathyroid hormone levels, respectively. The HOMA model was used to measure insulin sensitivity and β-cell secretory function. Results: Medians (IQR) were 25.3 (20.4 - 32.4) ng/ml for 25OH D3 and 53.7 (38.4 - 115.7) pg/ml for hpTH. 58.7% of diabetics had insulin resistance, 126 (68.5%) had low vitamin D and 80 (43.5%) had hyperparathyroidism. In multivariate analysis, hpTH (partial r = −0.28;p = 0.0002) and 25OH D3 (partial r = 0.16;p = 0.03) showed an independent association with insulin sensitivity after adjustment for body mass index and waist circumference. Finally, hpTH (partial r = 0.27;p = 0.0002) was the sole determinant of β-cell secretory function. Conclusions: This study confirms the high prevalence of low vitamin D level and secondary hyperparathyroidism and their association with insulin resistance and impaired islet β-cell secretory function among Congolese with type 2 diabetes mellitus. Vitamin D and calcium supplementation should be envisaged for cases of deficiency in this region.

Efficiency of Legumes in Increasing Yam Yield under Sandy Ferrasol in the Forest Zone of Southwestern Côte d’Ivoire

The cropping system is based on traditional farming in rural areas. In this way, yields of some food crops like yam are very low considering their high need for nutrients. The nutrient-rich land is under high land pressure as it is occupied by cash crops such as oil palm and rubber at the expense of food crops and soil fertility is declining. The study was conducted in Grand-Lahou to assess the effect of organic matter provided by legumes in increasing yam yield. The randomized complete block design included four (4) treatments: Arachis hypogaea + yam association (T1), Acacia mangium + yam association (T2), Arachis + Acacia mangium + yam association (T3) and pure yam crop (T0). The parameters measured were: the soil nutrients content, the rate of mineralization of organic matter through the C/n ratio as well as the yield of yam through different techniques that allowed the objective of the study to be achieved. It was noted a slight improvement of nitrogen and organic carbon in all treatments with a higher value recorded in the T2 treatment. The C/n ratio decreased to 12 in all treatments compared to the control, which reflects a normal decomposition of the organic matter. Treatment T1 increased yam yield (8.4 t/ha) in contrast to treatments T2 (4.1 t/ha) and T3 (2.8 t/ha). Legume-based treatments T2 and T3 were associated with a significant decrease in yam yield, resulting in losses of −46.34% to −25.09%, while treatment T1 resulted in a gain of 28.57%. As a result, the treatments involving legumes contributed to normalizing the organic matter content of the soil. The simultaneous association of these two legumes led to a decrease in yam production, contrary to the treatment associating only groundnut.

Evaluating and quantifying the effect of various spruce budworm intervention strategies on forest carbon dynamics in Atlantic Canada

Spruce budworm (SBW) outbreaks are one of the most devastating natural disturbances in spruce-balsam fir forests of eastern north America. Both early intervention strategy (EIS) and foliage protection strategy (Fp) are being tested to limit forest losses, but the quantitative impact on forest carbon (C) dynamics is still unclear. In this study, we designed 19 separate scenarios of no intervention or varying success of EIS, Fp, and their combination on SBW caused defoliation and mortality. We then used the TRIpLEX-Insect model to quantify their effects on forest C dynamics in the forests of the four provinces of Atlantic Canada. A scenario applying Fp to 10%of the area with the greatest potential C losses of living biomass, protecting foliage in 10%of the forests is more realistic than higher proportion of Fp given the high cost and large areas involved, resulted in reducing average cumulative net ecosystem productivity (nEp) from 2020 to 2039 by 56%–127%compared to a no outbreak *** results showed that Fp would have to be applied everywhere to reduce tree mortality and increase nEp more than 8 years of successful EIS applied. However, if EIS can be successfully implemented for 12 years, it will maintain more forest C than Fp applied everywhere during a moderate outbreak. We also found that the combination of EIS followed by Fp in 10%of the areas disturbed by the SBW could maintain average cumulative nEp at similar levels to no defoliation in every province of Atlantic Canada. Black/red spruce forests younger than 60years old underwent the smallest changes in C dynamics whether using EIS, Fp, or both. This highlights the importance of forest species, forest age, and their interactions on the effectiveness of a treatment during SBW outbreak. Overall, 31%–76%of the study area in Atlantic Canada could convert from a C sink to a source by 2039,if no protective measures are used under the worst-case scenarios, thus contributing to future climate warming.

A number of studies have focused on the effects of rare earth elements(REEs) on crop plants,while little attention has been paid on how tolerant plant species respond to increasing mixed REE *** this study,ramie(Boehmeria nivea L.) was exposed to a series of REE concentrations prepared with equimolar mixtures of 16 REEs(i.e.0,1.6,8,16,80,160,400,800 μmol/L) in order to explore REE accumulation and fractionation characteristics in ramie and the responses of this plant to mixed *** show that ramie root and shoot biomasses are unaffected under lower REE concentrations(1.6-80 μmol/L),while the growth of ramie and the uptake of nutrients especially Ca and Mn are largely inhibited under higher REE concentrations(160-800 μmol/L).The p and Mo concentrations in the roots increase with the increasing REE concentrations in the solution,suggestive of an involvement of p and Mo in dealing with the high concentrations of REEs in this *** preferential uptake of Ce and heavy REEs(HREEs) and the preferential transport of HREEs within the plant lead to a positive Ce anomaly and a HREE enrichment in ramie *** study suggests that ramie could be a good candidate for the phyto re mediation of heavily REE-contaminated soils(e.g.,REE mine tailings in southern China).Our results also shed light on points of taking into account phytoremediation management strategies of REEcontaminated soils(e.g.,p and Mo fertilization).

Hydrochemical Characterisation and Assessment of the Level of Contamination of Groundwater Collected by private Waterworks in the Town of Moundou in the South of Chad

Groundwater is the main source of drinking water for large cities in most African countries. In Moundou, for example, the conventional groundwater supply system is failing. To compensate for this state failure, the population is building boreholes and wells, most of which tap the surface water table, generally referred to as the “water table”. The aim of this study is to characterize these waters in order to assess their level of contamination and, by extension, the degree of pollution of the water table. Major elements such as: Chloride (Clp>-p>), Sulfate (SO4p>2-p>), nitrate (nO3p>-p>), Calcium (Cap>2+p>), magnesium (Mgp>2+p>), sodium (nap>+p>) and potassium (Kp>+p>) were analysed by Liquid Chromatography and the Bicarbonate ion (HCO3p>-p>) was determined by the titrimetric method. The methodology applied is based on a combination of hydrochemical techniques and statistical analysis (pCA and CHA). A sampling campaign was carried out during high-water periods. The results of the physico-chemical analyses show mineralization ranging from 7.29 to 3670 μS/cm, with an average of 487.44 μS/cm. The groundwater studied is generally acidic, with a pH ranging from 3.26 to 6.41. Based on their anions, they are classified into four main hydrochemical facies: chloride and sulphate facies, calcium and magnesium facies, sodium and potassium facies and bicarbonate facies. The various correlations between major ions and statistical analyses have enabled us to identify three hydrogeochemical processes involved in water mineralization. The dominant process is silicate hydrolysis, followed by cation exchange, then anthropogenic input, which influences mineralization by polluting the water.

Epidemiologic, Diagnostic, and Therapeutic Features of Urinary Stone Disease in northern Benin from 2018 to 2023

Objective: To determine the epidemiologic, diagnostic, and therapeutic features of urinary in northern Benin from 2018 to 2023. patients and Method: The study was an observational and descriptive one. We exhaustively and retrospectively collected data from the medical records of the patients that were managed from February 1, 2018 to February 1, 2023 at the Department of Surgery of “Centre Hospitalier Universitaire Départmental Borgou-Alibori or CHUD-BA”. Results: Among the 4522 patients managed, 75% or 1.7% had urinary stone disease. Their mean age was 41 years. The sex ratio was 2.75, the males being predominantly affected. Of the 75 cases of urinary stone disease, 30.7% (n = 23) were renal, 46.7% (n = 35) were ureteral, and 22.7% (n = 17) were vesical. The patients came from variable geographical areas, ethnic groups, and professional sectors. Their main complaints were pain (57.3%), hematuria (14.7%), dysuria (12%), and fever (5.3%). 15 (20%) patients, i.e., 7 renal stone and 8 ureteral stone patients, had an obstructive renal failure. The imaging tools available were ultrasonography, tomography and computed tomographic scan. 11 of the bladder stone cases (64.7%) were associated with a bladder outlet obstruction. Overall, 38.7% of the patients had either diabetes mellitus (6.7%), hypertension or high blood pressure (6.7%), benign prostate hyperplasia (8%), urinary schistosomiasis (6.7%), ureteral pelvic junction obstruction (2.7%), and prostate cancer (1.3%). Of the 19 patients whose dietary behavior was investigated 15 (79%), 11 (57.9%) et 11 (57.9%) demonstrated a diet which was respectively rich in milk, fromage and meat. 100% of bladder stones were removed through cystolithotomy.
