Baseline Laboratory Profile of HIV Positive Patients on Antiretroviral Therapy in Jos North Central Nigeria: Implications for Pre-vention, Treatment, Care and Support

Background: We characterized baseline laboratory data of patients to identify priority treatment areas, most affected populations, anticipated clinical complications and assessed the potential burden of retention in care in Jos, Plateau State—North Central Nigeria. Methods: This study was a cross-sectional design from January 2004 to December 2005 at Faith Alive Foundation (FAF). All participants were HIV-positive and underwent pre-antiretroviral therapy counseling based on the national antiretroviral therapy (ART) guidelines and baseline laboratory testing. Data were captured on Microsoft Excel, validated on Epi Info and analyzed on SPSS version 21 at P < .05 level of significance. Results: Total of 1499 (463 men and 1036 women) participants were evaluated in this study. The age and sex related distribution of participants showed that majority (80.3% for males and 92.5% for females) were 16 - 45 years old. Amylase and creatinine were significantly higher in males than females (P = .02). Anaemia was the most common baseline abnormality (63% for females and 58% for males), while baseline CD4 count was significantly lower in males than females (P < .02) and at one-third had elevated liver enzymes (AST and ALT). Conclusion: The baseline laboratory profile of most HIV positive patients in Jos and environs of North Central–Nigeria was characterized by anaemia, elevated creatinine, and abnormal liver transaminase levels (AST and ALT). This pre-ART laboratory result suggests that HIV-positive individuals have multiple clinical abnormalities which may require more extensive care than just treating the HIV disease.

Hydrogeochemical Characterization of Groundwater from Fissured Aquifers in the Angovia Mine Operating Permit Area (Central-West Côte d’Ivoire)

The main objective of this study is to determine the hydrogeochemical specificities of the groundwater of the Angovia mine operating permit, located in the Yaouré mountains in the center-west of Côte d’Ivoire. To do so, descriptive and multivariate statistical analysis methods with the SOM (Self Organizing Maps) algorithm were applied to the physicochemical parameters of 17 boreholes using the calcite (ISC) and dolomite (ISD) saturation indices. The results obtained have shown that the groundwater in the Angovia mine operating permit area has an average temperature of 27.52°C (long rainy season) and 27.87°C (long dry season) and has an average pH of 7.09 ± 0.35 during the main rainy season and 7.32 ± 0.35 during the main dry season. They are mineralized with an average electrical conductivity of 505.98 ± 302.85 μS/cm during the long rainy season and with 450.33 ± 233.74 μS/cm as average during the long dry season. The main phenomena at the origin of groundwater mineralization are water residence time, oxidation-reduction and surface inflow. The study of the relative age of the water shows that the groundwater in the Angovia mine operating permit area is mainly undersaturated with respect to calcite and dolomite. They are therefore very old in the aquifer with a slow circulation speed during the long rainy season and the long dry season.

HIV Treatment Cascade Assessment of a Community-Based Test and Start Model for Key Populations in Lagos State Nigeria: Where Are the Gaps?

Background: Community-based test and start (TAS) models have shown the potential to improve clinical outcomes for key populations because early access to treatment will have population level impact by reducing transmission through the promotion and provision of hassle-free quality HIV testing, on the spot ART initiation and unrestricted access to culturally competent health professionals. We assessed the effectiveness of a pioneer community-based test and start (TAS) model for key populations in Lagos along UNAIDS 90/90/90 cascade. Methods: Men who have sex with men (MSM), female sex workers (FSW), and persons who inject drugs (PWID), >14 years of age were recruited through peer-referral for HIV testing services (HTS) at a community-based KP-friendly clinic in Lagos between June 2015 and September 2016. The clinic provides comprehensive HIV services, including HIV treatment. Those who test positive are enrolled and provided with TAS services. Using routine service data, we deployed descriptive cross tabulations at p < 5% to determine the performance along the 90/90/90 cascade. Results: In total, 8812 KPs comprising MSM (51.6%), FSW (32.7%) and PWID (15.7%) were tested during this period. About one-half (46.4%) were young KP (15 - 25 years). The majority (76.1%) were male, single (86.1%), almost all (99%) had at least primary school level education, and about one-quarter (25.1%) were employed. Among MSM, 506 (11.1%) tested positive, 276 (54.5%) were initiated on ART, of whom 155 conducted viral load testing and 88 (56.7%) had achieved viral suppression (<1000 copies per milliliter). Among FSW, 88 (3.1%) tested positive, only 28 (31.8%) were initiated on ART, of whom 15 conducted viral load testing and 9 (60%) achieved viral suppression. Among PWID, of 15 (1.1%) who tested positive, only 2 (13%) were initiated on ART and none had conducted viral load testing. Client satisfaction index was 96.1%. Approximate Compositional Values and Tissue Fatty Acid Profiles of Nile Tilapia (Oreochromis niloticusL.) Fed Azolla-Diets in Earthen Ponds

The approximate general composition and the fatty acid profile of Nile tilapia fed Azolla-diets in ponds were studied for 90 days. Six isonitrogenous (29.2% CP) and isoenergetic (16.9 kJ·g>sup>-1) diets were formulated to contain 0% (A0), 10% (A10), 20% (A>sub>20), 30% (A30), 40% (A40) and 50% (A50) of Azolla meal (AM), as partial fish meal (FM) substitutes. Diet A0 without AM served as a control. Fish growth decreased as AM level exceeded 20% in diets (P 0.05). Crude lipid was significantly lower in fish fed A50 and significant differences were also found in crude ash (P 20, A30 and A50. High level of the fern reduces growth without negatively affecting fatty acid in fish. Fish PUFA, especially the (n - 3) fatty acids, are affected positively, even when fed 50% AM, which is good for the quality of the fish produced in regard to the benefits for the health of consumers.

Two 90-day experiments were conducted in the same period in tanks and in earthen ponds to study the effects of culture systems on growth and fatty acid (FA) of Nile tilapia (Oreochromis niloticus L.) fingerlings (mean weight: 16.3 g) fed Azolla, as partial replacement for fish meal (FM). Diets were isonitrogenous (29.2% CP) and isoenergetic (16.9 kJ·g-1) containing 0% (A0), 10% (A10), 20% (A20), 30% (A30), 40% (A40) and 50% (A50) of Azolla meal (AM) respectively. Diet A0, without AM, acted as a control. In both experiments, fish growth decreased as AM level increased in diets (P P 30-A50). Interestingly, those fish were rich in eicosapentaenoic acid (EPA, 20:5n-3) and docosahexaenoic acid (DHA, 22:6n-3). Consequently, the n-3/n-6 values decreased in tanks, whereas an increasing trend was found in ponds, and values were significantly higher in ponds than in tanks (0.35 - 0.53 vs. 0.08 - 0.17). The results suggest raising Nile tilapia with Azolladiets in ponds to improve growth and produce fish with a better FA composition, for the wellbeing of consumers.

Nitrogen (N) and phosphorus (P) wastes produced by Nile tilapia Oreochromis niloticus L. fed Azolla, an aquatic atmospheric nitrogen fixing fern, was evaluated for 90 days in pond experiment. Six isonitrogenous (29.2% crude protein) and isoenergetic (16.9 Kj·g–1) diets A0, A10, A20, A30, A40 and A50, containing 0%, 10%, 20%, 30%, 40% and 50% of Azolla meal (AM) respectively, as partial fishmeal (FM) substitutes, was provided to experimental fish. The Azolla-free diet A0 served as a control. Fish specific growth rate (SGR) was higher with the control diet, the lower values being obtained in A50-fed fish (P Azolla could be used in diet to sustain Nile tilapia growth and as “environmentally-friendly” ingredient to limit P loss, while providing N to the field, beneficially in tropical marshland pond where this nutrient is already limiting.

Soil Organic Carbon and Nitrogen Dynamics in Arabica Coffee Agroforestry Systems in the Noun Division, West Cameroon

Agroforestry systems (AFSs) offer viable solutions to climate change because of the below-ground biomass (BGB) that is maintained by the soil. Therefore, spatially explicit estimation of their BGB is crucial to account for emission reduction efforts. A study to assess soil organic carbon (SOC) and nitrogen dynamics in Arabica coffee agroforests was conducted in two subdivisions (Foumbot and Kouoptamo) of the Noun Division in western Cameroon. The methodological approach involved the collection of 150 soil samples taken at different depths: 0 - 10, 10 - 20 and 20 - 30 cm. Depending on the depth, the SOC stock is 27.93 ± 1.13 tC/ha at 10 cm depth, 22.37 ± 1.47 tC/ha at 20 cm and 20.79 ± 0.31 tC/ha at 30 cm. According to the age classes of the Arabica coffee systems (ACA), the C/N ratio in our study area averaged 26.94 ± 13.60 for the (5 - 20) year old systems in Foumbot and 60.64 ± 48.80 for the (20 - 35) year old systems in Kouoptamo. Depending on the depth, at 10 cm this ratio is higher in Kouoptamo than in Foumbot with a maximum value of 57 and 38 respectively for the two subdivisions. In view of the results obtained, it would be important to analyse the types of microorganisms responsible for the decomposition of organic matter which is linked to soil organic carbon.
