Peripheral nerve injuries induce a severe motor and sensory deficit. Since the availability of autologous nerve transplants for nerve repair is very limited, alternative treatment strategies are sought, including the use of tubular nerve guidance conduits(tNGcs). However, the use of tNGcs results in poor functional recovery and central necrosis of the regenerating tissue, which limits their application to short nerve lesion defects(typically shorter than 3 cm). Given the importance of vascularization in nerve regeneration, we hypothesized that enabling the growth of blood vessels from the surrounding tissue into the regenerating nerve within the tNGc would help eliminate necrotic processes and lead to improved regeneration. In this study, we reported the application of macroscopic holes into the tubular walls of silk-based tNGcs and compared the various features of these improved silk^(+) tNGcs with the tubes without holes(silk^(–) tNGcs) and autologous nerve transplants in an 8-mm sciatic nerve defect in rats. Using a combination of micro-computed tomography and histological analyses, we were able to prove that the use of silk^(+) tNGcs induced the growth of blood vessels from the adjacent tissue to the intraluminal neovascular formation. A significantly higher number of blood vessels in the silk^(+) group was found compared with autologous nerve transplants and silk^(–), accompanied by improved axon regeneration at the distal coaptation point compared with the silk^(–) tNGcs at 7 weeks postoperatively. In the 15-mm(critical size) sciatic nerve defect model, we again observed a distinct ingrowth of blood vessels through the tubular walls of silk^(+) tNGcs, but without improved functional recovery at 12 weeks postoperatively. Our data proves that macroporous tNGcs increase the vascular supply of regenerating nerves and facilitate improved axonal regeneration in a short-defect model but not in a critical-size defect model. The fluorine doping along with heavy metal oxides was remarkably raised the lifetime and the quantum efficiency of 3H4 in silicate glasses. The fluorine doping changed the local structure around Tm3+ ions, then low energy vibrations related to fluorine are considered to largely reduce the multi-phonon relaxation rates in the oxyfluoride silicate glasses.

Sexual function and psychological characteristics ofpenile paraffinoma

Aim: To identify the sexual, emotional and psychological status of men who augmented their penis with mineral oil injection for their small penis. Methods: Men who had penile paraffin were asked to answer the semi-structured questionnaire. The questionnaire was designed to assess the motivation, method of penile injection, changes in erectile function and satisfaction after penile injection. ScL (Symptom checklist)-90-R, STAI (State and Trait Anxiety Inventory) and Zung SDS (Self-rating depression scale) were also included in the questionnaire for psychological evaluation. Results: A total of 357 men completed the questionnaire. The first-ranked motivation of the injection was recommendation by their acquaintances (48.9 %). The majority of the respondents had the procedure by non-medical person (78.0 %). Before injection, 17.2 % had a sense of inferiority in their penis and 32 % worried about their weak erectile function. After injection, 33.0 % have found relief from their sense of inferiority and 17.8 % wish to feel improvement in their erectile function. Most of the respondents (91 %) were not satisfied with their penis and 74 % of them replied that they want to remove the injected material. Only 15.6 % did not experience side effects. Most of the subjects have suffered from various side effects such as inflammation, skin necrosis, pain, etc. No evidence of psychiatric pathology was found in psychological evaluation. conclusion: The motivations of mineral oil injection were recommendation by their acquaintances or desire to be more mannish. Most of them had suffered from various side effects and only a small number of them felt improvement in their sense of inferiority, in their penis and erectile function. Increased public awareness is needed for the prevention of this physically and psychologically debilitating problem.

BAcKGROUND Transcutaneous oxygen pressure(TcpO2)is a precise method for determining oxygen perfusion in wounded *** device uses either electrochemical or optical *** To evaluate the usefulness of TcpO2 measurements on free flaps(FFs)in diabetic foot ulcers(DFUs).METHODS TcpO2 was measured in 17 patients with DFUs who underwent anterolateral thigh(ALT)-FF surgery and compared with 30 patients with DFU without FF *** Significant differences were observed in the ankle-brachial index;duration of diabetes;and haemoglobin,creatinine,and c-reactive protein levels between the two ***2 values were similar between two groups except on postoperative days 30 and 60 when the values in the ALT-FF group remainedcrease>50 *** Even if the flap is clinically stable,sympathectomy due to adventitia stripping during anastomosis and arteriovenous shunt progression due to diabetic polyneuropathy could lead to low TcpO2 values in the ALT-FF owing to its thick fat tissues,which is supported by the slow recovery of the sympathetic tone following ***,TcpO2 measurements in patients with DFU who underwent FF reconstruction may be less accurate than in those who did not.

The predictability of T3 disease in staging MRI following prostate biopsy decreases in patients with high initial PSA and Gleason score

To obtain improved accuracy in predicting extracapsular extension （EcE） and seminal vesicle invasion （SVI）, we evaluated the variables affecting the predictability of staging magnetic resonance imaging （MRI, phased-array coil） and estimated their impact on accuracy between preoperative MRI staging and histological outcome. A total of 121 patients with localized or locally advanced prostate cancer who underwent robotic radical prostatectomy （RALP） were included. Following transrectal biopsy, all enrolled patients had undergone MRI for staging work-up. After RALP, only 43.8% （53/121） of the patients were matched with the MRI predicted stage. compared to the matched group in the prediction of EcE, the unmatched group had significantly higher initial prostate-specific antigen （PSA, 12.8 ng m1-1 versus 8.1 ng m1-1, P=0.048）. In the prediction of SVI, initial PSA （8.1 ng ml- 1 versus 17.3 ng ml- 1, P=0.009） and biopsy Gleason score （6.5 versus 7.6, P=0.035） were significantly higher in the unmatched group. When applying clinical cutoffs of initial PSA of 10 and 20 ng ml-1, the accuracy of MRI in the prediction of EcE was decreased in the group with PSA over 20 ng ml- 1 （75.6, 64.5 and 37.5%, P=0.01）, and this group had significantly decreased accuracy of MRI in the prediction of SVI （91.5, 77.4 and 37.5%, P〈0.01）. Applying the clinical cutoff of a Gleason score of 7, the accuracy of MRI in the prediction of SVI was decreased in the higher G leason score group （93.9, 82.1 and 62.9%, P= 0.01）. Thus, for these patient groups, to obtain margin negativity during radical prostatectomy, operative findings, rather than post-biopsy MRI images, may provide substantial information, implying a clinical advantage in conducting MRI before prostate biopsy.

Myasthenia gravis (MG) is an autoimmune disorder of neuromuscular junctionthat has higher incidence in younger women than men, which could be related todifferences in sex hormones physiology and immune system functioning betweenmales and females. MG can first present during pregnancy and variably affectpregnancy, labor, and postpartum period. In this paper, we had an updatedoverview on our understanding about MG presentation and its effect onpregnancy and vice versa, therapeutic options for MG pregnant women,management of pregnancy or labor complications in MG patients, and finally fetaland neonatal considerations in MG pregnant women. A multidisciplinaryapproach, involving obstetricians/gynecologists, neurologists, and anesthesiologists,plays a pivotal role in improving the clinical outcomes in both MGmothers and their infants during pregnancy, delivery and postpartum.

Detection of Breaking Waves Using X-Band Pulse Radar in the Nearshore Region

An X-band pulsed Doppler microwave radar has been used to determine the characteristics of breaking waves. Field experiments were conducted at the Shuang-Si estuary in the north of Taiwan in the winter of 2005. Analyses on maxima radar cross section and Doppler frequency shift are done to characterize wave breaking zones. Based on observations of breaking waves, the wave breaking zones are shown to be located at water depths of 1.8 to 2.2 m in the experimental site. In general, the results indicate that a radar system has the potential to delineate the spatial variation of breaking waves clearly and that this is sufficient to achieve a measurement operation for near-shore air-sea interaction events.

New method for rekindling the explosive waves in Maxwellian space plasmas

Our interest is to study the nonlinear wave phenomena in complex plasma constituents with Maxwellian electrons and ions. The main aim is to use a new method known as the(G′/G)method to exhibit the effects of dust charge fluctuations on the evolution of nonlinear waves. The coherent features of the shock and solitary waves along with the generation of high-energy waves have been amplified through the solution of the Korteweg–de Vries–Burgers equation,and the different natures of the waves were found successfully. Results are discussed graphically with the thoughtful choice of typical plasma parameters from different space plasma environments, exactly those found in cosmic dusty plasmas laden in ionospheric auroral region,radial spokes of Saturn's rings, planetary nebulae and solar F-corona region. All conclusions are in good accordance with the actual occurrences and could be of interest to further investigations of space. Moreover, the applicability of the present method is hoped to be a great enhancement by its use as ingenious mechanism used to evaluate the soliton dynamics and Burgers shock waves.
