Self-management of coronary heart disease in older patients after elective percutaneous transluminal coronary angioplasty

Objective To explore how older patients self-manage their coronary heart disease （ChD） aider undergoing elective percutaneous transluminal coronary angioplasty （PTCA）. Methods This mixed methods study used a sequential, explanatory design and recruited a convenience sample of patients （n = 93） approximately three months after elective PTCA. The study was conducted in two phases. Quantitative data collected in Phase 1 by means of a self-administered survey were subject to univariate and bivariate analysis. Phase 1 findings in- formed the purposive samplhag for Phase 2 where ten participants were selected from the original sample for an in-depth interview. Qualita- tive data were analysed using thematic analysis. This paper will primarily report the findings from a sub-group of older participants （n = 47） classified as 65 years of age or older. Results 78.7% （n = 37） of participants indicated that they would manage recurring angina symptoms by taking glyceryl trinitrate and 34% （n = 16） thought that resting would help. Regardless of the duration or severity of the symptoms 40.5% （n = 19） would call their general practitioner or an emergency ambulance for assistance during any recurrence of angina symptoms. Older participants weighed less （P = 0.02） and smoked less （P = 0.01） than their younger counterparts in the study. Age did not seem to affect PTCA patients＇ likelihood of altering dietary factors such as fruit, vegetable and saturated fat consumption （P = 0.237）. Conclusions The findings suggest that older people in the study were less likely to know how to correctly manage any recurring angina symptoms than their younger counterparts but they had fewer risk factors for ChD. Age was not a factor that influenced participants＇ likelihood to alter lifestyle factors.

The biology and ecology of biological soil crusts,a soil surface community of mosses,lichens,cyanobacteria,green algae,fungi,and bacteria,have only recently been a topic of *** efforts began in the western U.S.(Cameron,harper,Rushforth,and ***),Australia(Rogers),and Israel(Friedmann,Evenari,and Lange)in the late 1960s and 1970s(e.g.,Friedmann et al.1967;Evenari 1985,reviewed in harper and Marble 1988).however,these groups worked independently of each other and,in fact,were often not aware of each other’s *** addition,biological soil crust communities were seen as more a novelty than a critical component of dryland eco*** then,researchers have investigated many different aspects of these communities and have shown that although small to microscopic,biological soil crusts are critical in many ecological processes of *** often cover most of desert soil surfaces and substantially mediate inputs and outputs from desert soils(Belnap et al.2003).They can be a large source of biodiversity for deserts,as they can contain more species than the surrounding vascular plant community(Rosentreter 1986).These communities are important in reducing soil erosion and increasing soil fertility through the capture of dust and the fixation of atmospheric nitrogen and carbon into forms available to other life forms(Elbert et al.2012).

Cost-effectiveness of lutetium(^177Lu)oxodotreotide vs everolimus in gastroenteropancreatic neuroendocrine tumors in Norway and Sweden

BACKGROUND Gastroenteropancreatic neuroendocrine tumors(GEP-NETs)represent a relatively rare and heterogenous group of *** available treatment options for patients with progressive GEP-NETs include lutetium(177 Lu)oxodotreotide(177 LuDotatate)and everolimus[as well as sunitinib for patients with pancreatic NETs(P-NETs)].AIM To perform a health economic analysis to determine the cost-effectiveness of 177 LuDotatate compared with everolimus in patients with unresectable or metastatic midgut-NETs or P-NETs in both Sweden and *** Simulations were performed using a three-state partitioned survival model and analyses were performed separately for patients with midgut-NETs and *** input data were sourced from an indirect comparison that utilized survival data from clinical trials of 177 Lu-Dotatate and *** analyses were performed from the healthcare payer perspective over a time horizon of 20 *** Sweden,future costs and clinical outcomes were discounted at 3%per *** Norway,a discount rate of 4%per annum was *** For Sweden,improved survival outcomes and higher lifetime costs with 177 LuDotatate resulted in an incremental cost-effectiveness ratio(ICER)of SEK 391194 per quality-adjusted life year(QALY)gained for midgut NETs and SEK 16764 per QALY gained for P-NETs for 177 Lu-Dotatate compared with *** Norway,the corresponding ICERs were NOK 244444 per QALY gained and NOK 106451 per QALY gained,***-way sensitivity analyses revealed that the results were most sensitive to changes in drug acquisition costs and health state utility *** In both Sweden and Norway,from a healthcare provider perspective,177 LuDotatate is likely to be considered cost-effective relative to everolimus for the treatment of patients with unresectable or metastatic,progressive midgut-NETs or P-NETs.

Grasshoppers （Orthoptera： Acrididae） select vegetation patches in local-scale responses to foliar nitrogen but not phosphorus in native grassland

Key elements such as nitrogen （N） and phosphorus （P） are often limiting relative to the nutritional needs of herbivores that feed on them. While N often limits insect herbivores in natural terrestrial ecosystems, the effect of P is poorly studied in the field, even though compelling hypotheses from the ecological stoichiometry literature predict its importance. We evaluated small-scale spatial distributions of, and herbivory by, grasshoppers among neighboring plots that vary in foliar-N and -P in tallgrass prairie. Grasshopper densities were 67% greater in N-fertilized plots but detected no effect to grasshopper densities from P-fertilizer. Leaf damage to the dominant grass Andropogon gerardii was 32% greater in N-fertilized plots, but no response to foliar-P was detected. herbivore damage to a common forb, goldenrod （Solidago missouriensis）, was not strongly linked by fertilizer treatments, although there was increased leaf damage in N-fertilizer treatments when no P was applied （a significant N ~ P interaction）. Under field conditions at local scales, we conclude that spatially heterogeneous distributions of grasshoppers are primarily affected by foliar-N in host plants with little evidence that P-levels contribute to the spatial patterns.

Dear Editor,LTR retrotransposons （LTR-TEs） are mobile genetic elements that often comprise a large portion of the host genome and are particularly abundant in genomes of plants （Lisch, 2013）. They transpose through an RNA transcript that is reverse transcribed to an extrachromosomal DNA （ecDNA） of the original element that can insert at a new genomic location （Wicker et al., 2007）.

Identification of QTL and underlying genes for root system architecture associated with nitrate nutrition in hexaploid wheat

The root system architecture(RSA) of a crop has a profound effect on the uptake of nutrients and consequently the potential yield. however, little is known about the genetic basis of RSA and resource adaptive responses in wheat(Triticum aestivum L.). here, a high-throughput germination paper-based plant phenotyping system was used to identify seedling traits in a wheat doubled haploid mapping population, Savannah×Rialto. Significant genotypic and nitrate-N treatment variation was found across the population for seedling traits with distinct trait grouping for root size-related traits and root distribution-related traits. Quantitative trait locus(QTL) analysis identified a total of 59 seedling trait QTLs. Across two nitrate treatments, 27 root QTLs were specific to the nitrate treatment. Transcriptomic analyses for one of the QTLs on chromosome 2 D, which was found under low nitrate conditions, revealed gene enrichment in N-related biological processes and 28 differentially expressed genes with possible involvement in a root angle response. Together, these findings provide genetic insight into root system architecture and plant adaptive responses to nitrate, as well as targets that could help improve N capture in wheat.

Early intervention to prevent premature mortality is vital for adults with congenital heart defects(ChD).Anatomic complexity and comorbid conditions are thought to contribute to ChD *** hypertension(hTN)and heart failure(hF)are the comorbid conditions among the most prevalent causes of death in the United States,and commonly accompany ChD,it is crucial to evaluate whether they are reliable predictors of mortality for adults with ChD(AChD)independent of anatomic ChD complexity.A retrospective cross-sectional analysis of AChD,aged 18–64,with concomitant hTN and/or hF and at least one health care encounter during 2008–2010 were *** 5,397 AChD patients(18.3%hTN without hF,4.4%hF without hTN,8.3%with both),3.0%died(n=163)during the study ***,the sample was 45.1%white,61.4%female,and 29.0%had a complex *** those who died,23.3%had hTN without hF,17.2%had hF without hTN,and 42.3%had *** analyses revealed that older age,male gender,black race,and having public health insurance were associated with increased mortality during the three-year study period compared to AChD patients who were younger,female gender,white race,and covered by private health *** patients diagnosed with non-complex ChD lesions(i.e.,shunts,valves,or shunts+valves)were at greater risk of dying compared to those with severe complex *** ChD type was assessed separately,those with valve lesions were more likely to die compared to those with complex ChD *** adjustment for age,gender,race,insurance and ChD complexity,AChD patients with hF,with or without hTN,were equally likely to die during the study ***,AChD patients with hF,without or without hTN,who had valve defects were more likely to die during the three-year study period compared to patients with complex ChDs.

FOXO3a as a sensor of unilateral nerve injury in sensory neurons ipsilateral, contralateral and remote to injury
