Spinal manipulation therapy: Is it all about the brain? A current review of the neurophysiological effects of manipulation

Spinal manipulation has been an effective intervention for the management of various musculoskeletal disorders. However,the mechanisms underlying the pain modulatory effects of spinal manipulation remain elusive. Although both biomechanical and neurophysiological phenomena have been thought to play a role in the observed clinical effects of spinal manipulation, a growing number of recent studies have indicated peripheral, spinal and supraspinal mechanisms of manipulation and suggested that the improved clinical outcomes are largely of neurophysiological origin. In this article, we reviewed the relevance of various neurophysiological theories with respect to the findings of mechanistic studies that demonstrated neural responses following spinal manipulation. This article also discussed whether these neural responses are associated with the possible neurophysiological mechanisms of spinal manipulation. The body of literature reviewed herein suggested some clear neurophysiological changes following spinal manipulation, which include neural plastic changes, alteration in motor neuron excitability,increase in cortical drive and many more. However, the clinical relevance of these changes in relation to the mechanisms that underlie the effectiveness of spinal manipulation is still unclear. In addition,there were some major methodological flaws in many of the reviewed studies. Future mechanistic studies should have an appropriate study design and methodology and should plan for a long-term follow-up in order to determine the clinical significance of the neural responses evoked following spinal manipulation.

Well productivity in the Asmari carbonate formation of southwest Iran has decreased in recent years as a result of production *** production rate must be maintained below 1500 STB/day to prevent water *** this study,a gas lift well is modeled using data from one of the producing wells of this *** analysis is performed using lift-gas injection rates and wellhead pressures at different reservoir pressures and water cut conditions to optimize *** aspects are considered to optimize the artificial gas injection rates at different tubing head pressures and water cut *** the lift-gas injection rate from 0.4 MMscf/day to 1 MMscf/day enhances the oil production rate by 37.71%and 43.89%for 10%and 30%water cut conditions,*** injection rates of 5.2 MMscf/day and 5.4 MMscf/day are determined to be economically optimal for 30%water cut with tubing head pressures of 260 psig and 270 psig,respectively.

Two-Stent Strategy for Bifurcation Lesions in Percutaneous Transluminal Coronary Angioplasty: Real-World Evidence

Background: Bifurcation lesions pose a high risk for adverse events after percutaneous coronary intervention (PCI). Evidence supporting the benefits of the two-stent strategy (2SS) for treating coronary bifurcation lesions in India is limited. This study aimed to evaluate the clinical outcomes of various 2SSs for percutaneous transluminal coronary angioplasty for bifurcation lesions in India. Materials and Methods: This retrospective, observational, multicentric, real-world study included 64 patients over 8 years. Data on demographics, medical history, PCI procedures, and outcomes were recorded. Descriptive statistics were computed using the SPSS software. Results: Patients (n = 64) had an average age of 65.3 ± 11.1 years, with 78.1% males. Acute coronary syndrome was reported in 18.8%, chronic stable angina in 40.6%, and unstable angina in 34.4% of participants. Two-vessel disease was observed in 98.4% of patients, and 99.4% had true bifurcation lesions. The commonly involved vessels were the left anterior descending artery (50%), left circumflex coronary artery (34.4%), and first diagonal artery (43.8%). Mean percent diameter stenosis was 87.2% ± 10.1%. The mean number of stents used was 2.00 ± 0.34. The 2SS techniques included the T and small protrusion (TAP) (39.1%), double kissing (DK) crush (18.8%), and the culotte techniques (14.1%). Procedural and angiographic success rate was 92.18%. Major adverse cardiovascular events at 1-year follow-up occurred in 7.8% of cases. Conclusion: The 2SS for bifurcation lesions showed favorable in-hospital and follow-up outcomes. Findings can serve as a resource for bifurcation angioplasty in India. Larger real-world studies with robust methodology are needed to validate these results.

A numerical analysis has been carried out to investigate the problem of magnetohydrodynamic(MHD)boundary-layer flow and heat transfer of a viscous incompressible fluid over a fixed *** surface boundary condition is taken into account for thermal boundary condition.A problem formulation is developed in the presence of thermal radiation,magnetic field and heat source/sink parameters.A similarity transformation is used to reduce the governing boundary-layer equations to couple higher-order nonlinear ordinary differential *** equations are numerically solved using Keller–Box *** effect of the governing parameters such as radiation,Prandtl number,Hartman number,heat source/sink parameter on velocity and temperature profile is discussed and shown by plotting *** is found that the temperature is an increasing function of convective parameter A,radiation and heat source ***,the numerical results for the local skin friction coefficient and local Nusselt number are computed and presented in tabular *** a comparison with a previously published results on a special case of the problem has done and shows excellent agreement.

In the present study,the spatial-temporal distribution of b-value along the five faults area(the Judi fault,Thaple fault,Kathmandu fault,Motihari-Gauri shanker fault,and Motihari-Everest fault)was investigated after the Gorkha earthquake(M7.8).The earthquake catalog of 10,500 events was prepared by compiling the published *** study area is bounded in the central Himalaya from 26.5°to 29°in latitude direction and 84°to 87°in longitude *** frequency magnitude distribution shows the variation of the b-value along with fault areas from 0.45 to 0.69,indicating a common characteristic of aftershock *** particular,the Judi fault area,Thaple fault area,and Motihari-Everest fault area are characterized by the low b-values of 0.45±0.02,0.48±0.02,and 0.55±0.04,*** regions could be the source region for future *** low b-value observed for fault areas are also consistent with the thrust faulting pattern in the region as indicated by the focal mechanism of mainshock and major *** temporal variation of b-value shows inevitable fluctuations during25 April to 12 May *** the area selected,the Motihari-Everest fault area is in critical strain(mechanically locked)conditions,as indicated by the stepwise energy release pattern.

Multi-Site Air Pollutant Prediction Using Long Short Term Memory

The current pandemic highlights the significance and impact of air pollution on individuals. When it comes to climate sustainability, air pollution is amajor challenge. Because of the distinctive nature, unpredictability, and greatchangeability in the reality of toxins and particulates, detecting air quality is apuzzling task. Simultaneously, the ability to predict or classify and monitor airquality is becoming increasingly important, particularly in urban areas, due tothe well documented negative impact of air pollution on resident’s health andthe environment. To better comprehend the current condition of air quality, thisresearch proposes predicting air pollution levels from real-time data. This studyproposes the use of deep learning techniques to forecast air pollution ***, activation functions, and a number of epochs were used to create the suggested Long Short-Term Memory (LSTM) network based neural layer design. Theuse of proposed Deep Learning as a structure for high-accuracy air quality prediction is investigated in this research and obtained better accuracy of nearly 82% compared to earlier records. Determining the Air Quality Index (AQI) and danger levelswould assist the government in finding appropriate ways to authorize approaches toreduce pollutants and keep inhabitants informed about the findings.

The highly speciose gekkonid genus Cnemaspis Strauch,1887 is polyphyletic,with three distantly related and geographically isolated clades from Africa,South Asia(SA),and Southeast *** present,there are 85 known species within SA Cnemaspis,although the number continues to increase rapidly with focused surveys and rigorous taxonomic *** studies have provided valuable insights into the diversity and evolutionary history of SA Cnemaspis;however,most of these studies lack sufficient sampling in the Western Ghats(WG),where the genus has its greatest *** addressed this research gap by conducting extensive sampling across the WG and re-examining museum specimens,thus providing a systematic account of various extant Cnemaspis species along with their distribution and natural *** described 12 new species and a southern WG endemic clade of SA *** of the newly described species are endemic to the forests of the southern *** also identified 10 well-supported subclades that can be separated across morphological,geographic,and phylogenetic axes.A time-calibrated phylogeny and ancestral area reconstructions confirmed the Paleocene origin of SA Cnemaspis in the WG and provide insights into its evolutionary history and *** discovery of multiple endemic and deeply divergent lineages further highlights the evolutionary significance of the WG for lizards.
