Portal hypertension(PHT)in advanced chronic liver disease(ACLD)results from increased intrahepatic resistance caused by pathologic changes of liver tissue composition(structural component)and intrahepatic vasoconstriction(functional component).PHT is an important driver of hepatic decompensation such as development of ascites or variceal *** and an impaired intestinal barrier in ACLD facilitate translocation of bacteria and pathogen-associated molecular patterns(PAMPs)that promote disease progression via immune system activation with subsequent induction of proinflammatory and profibrogenic *** portal venous blood flow represents a critical pathophysiological mechanism linking PHT to increased intestinal permeability:The intestinal barrier function is affected by impaired microcirculation,neoangiogenesis,and abnormal vascular and mucosal *** close bidirectional relationship between the gut and the liver has been termed“gut-liver axis”.Treatment strategies targeting the gut-liver axis by modulation of microbiota composition and function,intestinal barrier integrity,as well as amelioration of liver fibrosis and PHT are supposed to exert beneficial *** activation of the farnesoid X receptor in the liver and the gut was associated with beneficial effects in animal experiments,however,further studies regarding efficacy and safety of pharmacological FXR modulation in patients with ACLD are *** this review,we summarize the clinical impact of PHT on the course of liver disease,discuss the underlying pathophysiological link of PHT to gut-liver axis signaling,and provide insight into molecular mechanisms that may represent novel therapeutic targets.

Arctic Ocean gateway fluxes play a crucial role in linking the Arctic with the global ocean and affecting climate and marine *** reviewed past studies on Arctic-Subarctic ocean linkages and examined their changes and driving *** review highlights that radical changes occurred in the inflows and outflows of the Arctic Ocean during the ***,the Pacific inflow temperature in the Bering Strait and Atlantic inflow temperature in the Fram Strait hit record highs,while the Pacific inflow salinity in the Bering Strait and Arctic outflow salinity in the Davis and Fram straits hit record *** the ocean heat convergence from lower latitudes to the Arctic and the hydrological cycle connecting the Arctic with Subarctic seas were stronger in 2000-2020 than in ***6 models project a continuing increase in poleward ocean heat convergence in the 21st century,mainly due to warming of inflow *** also predict an increase in freshwater input to the Arctic Ocean,with the largest increase in freshwater export expected to occur in the Fram Strait due to both increased ocean volume export and decreased *** Strait sea ice volume export hit a record low in the 2010s and is projected to continue to decrease along with Arctic sea ice *** quantitatively attribute the variability of the volume,heat,and freshwater transports in the Arctic gateways to forcing within and outside the Arctic based on dedicated numerical simulations and emphasize the importance of both origins in driving the variability.

Objective: To investigate the clinical effect of rituximab (RTX) in the management of progressive rheumatoid arthritis related interstitial lung disease (RA-ILD). Methods: A total of 10 patients with progressive RA-ILD were enrolled into this 48-week, open-label treatment study. Treatment was with RTX at 1000 mg at day 1, day 15, and again at weeks 24 and 26, with concomitant methotrexate therapy. Results: The study included 4 men and 6 women. Of 7 evaluable patients at week 48, the diffusing capacity to carbon monoxide had worsened by at least 15% in 1 patient, was stable in 4 patients, and increased by >15% of baseline value in 2 patients. The forced vital capacity declined by at least 10% in 1 patient, was stable in 4 patients, and increased by at least 10% in 2 patients. High resolution computed tomo-graphy of the chest showed improvement in 1 patient, and was unchanged in 5. Three patients were withdrawn, one who had an infusion reaction at week 0, one at week 5 who was hospitalized for congestive heart failure at week 5 and who later died at week 32 of complications following a traumatic hip fracture, and one died at week 6 of possible pneumonia. Conclusions: In this pilot study of 10 patients with RA-ILD treated with RTX, measures of lung disease remained stable in the majority of study completers. Further research is needed to clarify whether this treatment has a role in management of RA-ILD.
