Metabolic and bariatric surgery(MBS)is an effective treatment for patients with morbid obesity and its ***,many patients experience weight regain(WR)after achieving their nadir *** the definition of WR is challenging as postoperative WR has various *** factors for WR after MBS include anatomical,racial,hormonal,metabolic,behavioral,and psychological factors,and evaluating such factors preoperatively is ***-term regular follow-up and timely treatment by a multidisciplinary team are important because WR after surgery is *** lifestyle interventions that focus on appropriate dietary education,physical activity education or interventions,and behavioral psychological interventions are suggested,more well-designed studies are needed because studies evaluating intervention methods and the effectiveness of WR prevention are *** drugs can be used to prevent and manage patients with WR after MBS;however,more research is needed to determine the timing,duration,and type of anti-obesity drugs used to prevent WR.

Kearns-Sayre syndrome （KSS） is an extremely rare form ofmitochondrial myopathy characterized by progressive external ophthalmoplegia, bilateral pigmentary retinopathy, and cardiac conduction abnormalities. In addition, KSS patients might present with proximal muscle weakness, diabetes mellitus, hearing loss, cerebellar ataxia, and elevated cerebrospinal fluid protein levels.

The seriousness of metabolic syndrome is not due to the disease itself but its promotion of other diseases, such as erectile dysfunction and cardiovascular and cerebrovascular diseases. We investigated the effects of Korean red ginseng （KRG, Panax ginseng） extract on erectile function in a rat model of metabolic syndrome. We divided the rats into three groups： control, metabolic syndrome＋normal saline （N/S） and metabolic syndrome ＋ KRG. To determine the occurrence of metabolic syndrome in all groups, body weight and various biochemical parameters （e.g., blood glucose, insulin, cholesterol） were measured, and the intra-abdominal glucose tolerance test was performed. To investigate penile erection, the peak intracavernosal pressure （ICP）, mean arterial pressure （MAP） and Masson＇s trichrome stain were evaluated. Erectile function was also investigated by measuring the cyclic guanosine monophosphate （cGMP） levels of the corpus cavernosum. We found that the various biochemical parameters and body weight were similar in the metabolic syndrome＋KRG group and the control group, although the values were slightly higher. The peak ICP/MAP ratio of the metabolic syndrome＋N/S group was markedly decreased compared to the other groups. The cGMP level of the corpus cavernosum in the metabolic syndrome＋N/S group was significantly lower than that of the other groups. As demonstrated in this model of metabolic syndrome with erectile dysfunction, KRG may improve erectile function.

Objective： To investigate the anti-oxidative stress and preventive effect of modified Gongjin-dan （WSY-1075） in a detrusor underactivity rat model. Methods： Rats were randomly allocated to three groups： sham- operated （control）, bladder outlet obstruction-induced detrusor underactivity （BOO-DU）, and BOO-DU with WSY-1075 （WSY） groups. WSY-1075 was orally administrated to rats 200 mg daily for 2 weeks pdor to the operation and 4 weeks after the operation. Bladder outlet obstruction was surgically induced in rats by ligation around the urethra avoiding total obstruction. Cystometrography was conducted on rats in each group for examination of bladders. Results： Compared with the control group, bladder outlet obstruction led to a significant increase in oxidative stress with consequent changes to molecular composition, and decrease in maximal detrusor pressure （P〈0.05）. WSY-1075 treatment significantly suppressed oxidative stress and prevented degenerative and dysfunctional changes in bladder, as compared with BOO-DU group （P〈0.05）. Conclusion： WSY-1075 had beneficial effect on prevention of BOO-DU.

Aim： To evaluate the efficacy and safety of SK3530, a newly developed type 5 phosphodiesterase inhibitor （PDE5I）, in Korean men with erectile dysfunction （ED）. Methods： A total of 119 patients were randomized at 10 centers in Korea to receive either SK3530 （50, 100, or 150 mg; n = 89） or placebo （n = 30） taken 1 h before anticipated sexual activity for an 8-week period. The patients were evaluated at baseline and 4 and 8 weeks after beginning therapy. Efficacy was assessed using the International Index of Erectile Function （IIEF）, Sexual Encounter Profile （SEP）, and the Global Assessment Question （GAQ）. Safety was analyzed by adverse events, laboratory values and vital signs. Results： At the end of the study, all the primary and secondary efficacy end-points were statistically significantly improved by SK3530 compared with placebo （P 〈 0.05）. Of the 89 patients in the treatment arm, 36 （42.3 %） achieved normal erectile function after treatment, including six patients with severe ED. Treatment-related adverse events occurred in 32 patients. The most common adverse events were flushing, headache, dizziness and eye redness （10.9%, 7.6%, 2.5% and 2.5%, respectively）, and most were mild. Only two patients discontinued treatment during the study period because of adverse events. Conclusion： The results of our phase Ⅱ study have confirmed the efficacy and safety of SK3530 in a broad population of men with ED of various etiologies and severity. The optimal doses in terms of efficacy and safety were determined to be 50 mg and 100 mg, respectively.

Pancreatic pseudocysts,which account for 70%-90% of pancreatic cystic lesions,characteristically are non-epithelially lined cystic cavities that are contiguous with the pancreas. Pancreatic pseudocysts can be caused by acute,chronic or traumatic pancreatitis and should be differentiated from other pancreatic diseases with cystic appearances,especially cystic neoplasms. We report a unique case of a pancreatic pseudocyst filled with semisolid lipids,which appeared by endoscopic ultrasound as a solid mass,and was therefore resected.

背景:由于关节软骨再生修复能力有限,自体软骨细胞移植技术为关节软骨损伤的修复提供了较好的修复材料。目的:多中心评估自体软骨细胞移植的短期疗效与安全性。设计、时间及地点:自身前后对照观察,于2001-03/2006-04在韩国80所附属医院完成。对象:软骨缺损患者261例,男169例,女92例,年龄15～70岁,平均36.47岁。软骨缺损面积2.0～20.0cm2。其中股骨髁损伤215例,滑车30例,髌骨25例,胫骨2例。方法:取关节非负重区软骨组织200～300mg,体外培养扩增后将软骨细胞植入患者关节缺损区。于移植后6个月对所有患者进行随访,膝关节功能评估标准采用美国膝关节协会评分系统[Knee society score-A(KSS-A)和Knee society score-B(KSS-B)]。主要观察指标:自体软骨细胞膝关节缺损区移植后KSS-A和KSS-B评分。结果:获得随访患者261例。患者移植前KSS-A评分为63.55±18.47,移植后6个月为88.74±11.47,差异有显著性意义(P<0.05)移植前KSS-B评分为59.56±24.92,移植后6个月为85.13±14.67,差异有显著性意义(P<0.05)。患者移植前后膝关节疼痛、活动度、前后稳定性、屈曲挛缩、伸直迟滞、行走能力和上下楼能力均明显改善(P<0.05)。综合评价移植有效率为97.3%,优良率为89.3%。结论:自体软骨细胞移植可有效的治疗关节软骨缺损,促进膝关节功能的恢复。

AIM:To evaluate the long-term efficacy and safety of endoscopic obliteration with Histoacryl  for treatment of gastric variceal bleeding and ***:Between January 1994 and March 2010 at SoonChunHyang University Hospital,a total of 127 patients with gastric varices received Histoacryl  injections *** hundred patients underwent endoscopic Histoacryl  injections because of variceal bleeding,the other 27 patients received such injections as a prophylactic ***:According to Sarin classification,56 patients were GOV1,61 patients were GOV2 and 10 patients were *** of the varices were large(F2 or F3,111 patients).The average volume of Histoacryl  per each session was 1.7±1.3 cc and mean number of sessions was 1.3±0.6.(1 session-98 patients,2 sessions-25 patients,≥3 sessions-4 patients).Twenty-seven patients with high risk of bleeding(large or fundal or RCS+or Child C) received Histoacryl  injection as a primary prophylactic *** these patients,hepatitis B virus was the major etiology of cirrhosis,25 patients showed GOV1 or 2(92.6%)and F2 or F3 accounted for 88.9%(n=24).The rate of initial hemostasis was 98.4%and recurrent bleeding within one year occurred in 18.1%of *** hemostasis during episodes of rebleeding was achieved in 73.9%of *** survival was 50 mo (95%CI 30.5-69.5).Major complications occurred in 4 patients(3.1%).The rebleeding rate in patients with hepatocellular carcinoma or GOV2 was higher than in those with other *** of the 27 subjects who were treated prophylactically experienced treatment-related *** survival rates of the 127 patients at 6 mo,1,3,and 5 years were 92.1%,84.2%,64.2%,and 45.3%,*** 6 mo cumulative survival rate of the 27 patients treated prophylactically was 75%.CONCLUSION:Histoacryl  injection therapy is an effective treatment for gastric varices and also an effective prophylactic treatment of gastric varices which

This paper presents an innovative data-integration that uses an iterative-learning method,a deep neural network(DNN)coupled with a stacked autoencoder(sae)to solve issues encountered with many-objective history *** proposed method consists of a DNN-based inverse model with sae-encoded static data and iterative updates of supervised-learning data are based on distance-based clustering *** functions as an inverse model and results in encoded flattened data,while sae,as a pre-trained neural network,successfully reduces dimensionality and reliably reconstructs *** iterative-learning method can improve the training data for DNN by showing the error reduction achieved with each iteration *** proposed workflow shows the small mean absolute percentage error below 4%for all objective functions,while a typical multi-objective evolutionary algorithm fails to significantly reduce the initial population *** learning-based manyobjective history matching estimates the trends in water cuts that are not reliably included in dynamicdata *** confirms the proposed workflow constructs more plausible *** workflow would be a reliable alternative to overcome the less-convergent Pareto-based multi-objective evolutionary algorithm in the presence of geological uncertainty and varying objective functions.
