Abstract:Background:The pTNM staging system is widely recognized as the most effective prognostic indicator for *** latest update of this staging system introduced a new pathological staging system(ypTNM)for patients receiving neoadjuvant chemoradiotherapy(NACRT).However,whether the prognostic value of the ypTNM staging system for rectal cancer is similar to that of the pTNM staging system remains *** study was conducted to compare the ypTNM and pTNM staging systems in terms of their prognostic value for patients with nonmetastatic rectal cancer undergoing *** and Methods:This study was conducted at a large teaching *** January 2014 and December 2022,542 patients with rectal cancer were analyzed(median follow-up period,60 months;range,6–105 months).Of them,258 and 284 were included in the pTNM and ypTNM groups,*** probability of treatment weighting(IPTW)was performed to account for the effects of *** proportional-hazards regression was performed for the between-group comparison of overall survival(OS).Results:The crude model revealed that OS was similar between the two groups(p=0.607).After performing IPTW,we found that patients with the same ypTNM-and pTNM-classified stages had similar overall survival(hazard ratio=1.15;95%CI=0.76–1.73;p=0.5074).Conclusions:For patients with rectal cancer who have received preoperative NACRT,the prognostic value of ypTNM staging appears to be similar to that of pTNM staging,mostly because of the downstaging effect of neoadjuvant chemoradiotherapy。

In this paper, the long-term statistical properties of wave height in an idealized square harbor with a partial opening are studied. The incident waves are propagated into the harbor numerically by the finite/infinite element method using three different wave models: (1) monochromatic wave train, (2) long-crested random wave train, and (3) short-crested random wave train. This study shows that for a given incident wave, the wave height in the harbor is affected by the wave model used. For long-term estimation of wave height exceedance probability, it is recommended that the waves be propagated into the harbor using the random wave model, and that wave heights be computed by use of the Rayleigh probability distribution.

AIM： To assess the efficacy and safety of a compound containing alginic acid plus antacid （Topaal） compared to equal-strength antacid （Nacid） in patients with endoscopy-negative reflux disease （ENRD）. METHODS： A total of 121 patients with ENRD were randomized to receive Topaal （65 patients） or Nacid （56 patients） for 6 weeks, with a consultation every 3 weeks. The primary end-point assessment was the change in the severity of heartburn as evaluated using a visual analog scale （VAS） at 6 weeks. The secondary end-point assessments were the VAS at 3 weeks, the change of frequency of the reflux symptom, the change of quality of life and the adverse effects. RESULTS： Demographics of randomized subjects in each treatment group were comparable except that the Topaal group included more males. The baseline characteristics between the groups were similar. After 6 weeks of treatment, the reduction of VAS of heartburn was more prominent in the Topaal group （-6.29 cm vs -4.11 cm）. At the 3m week, Topaal group showed greater reduction of VAS for heartburn （P= 0.0016）, regurgitation （P=0.0006）, vomiting （P=0.0373）, and belching （P〈0.0001）. The patients of the Topaal group had lower frequency of heartburn （P=0.0015） and pain （P= 0.0163） at the end of the 6-week treatment period. From the doctor＇s point of view, the Topaal group also showed significant reduction in the severity of heartburn （P= 0.0020）, regurgitation （P=0.0081）, vomiting （P= 0.0182）, and belching （P= 0.0018） at the end of the treatment. The improvement of the quality of life was more remarkable in the Topaal group at the end of the 6-week treatment period （P〈 0.0001）. For the adverse effect, there was no difference in both the groups. CONCLUSION： Topaal is more effective than Nacid for the treatment of symptoms presented by patients with ENRD.

AIM: To examine the effects of Helicobacter pylori(Hpylori) infection on the invasiveness of gastric cancer cells,and to elucidate its mechanism. METHODS: Gastric carcinoma cells, MKN-45, were incubated with CagA-positive H pylori, and cell invasion was determined by Matrigel *** expression of matrix metallopr-oteinase-9 (MMP-9),vascular endothelial growth factor (VEGF), and cyclooxygenase-2 (COX-2) were assessed by Western-blot analysis, and transcriptional activation of the COX-2 promoter was examined by measuring luciferase and β-galactosidase activities. Lastly,the proteinDNA interaction was confirmed by an electrophoretic mobility shift assay. RESULTS: The current studies showed that: (1) incubation of CagA-positive H pylori with MKN-45 cells significantly promotes gastric cancer cells invasion, and this effect is attenuated by pre-treatment with NS-398, a COX-2 inhibitor, or PDTC,a nuclear factor κB (NF-κB) inhibitor;(2) the induction of MKN-45 cells invasion by Hpylori is associated with increases in COX-2, MMP-9, and VEGF protein expression, and co-incubation of NS-398 or PDTC significantly reduces these effects;(3) H pylori infection transactivates COX-2 promoter activity and increases the binding of NF-κB to this promoter. CONCLUSION: Our data demonstrate that H pylori infection promotes gastric epithelial cells invasion by activating MMP-9 and VEGF expression. These effects appear to be mediated through a NF-κB and COX-2 mediated pathway, as COX-2 or NF-κB inhibitor significantly attenuate the invasiveness of gastric cancer cells and the expressions of MMP-9 and VEGF protein.

云计算、人工智能、互联网大数据、高价值现实应用等前沿技术的快速发展,为人类社会带来了积极的变革。然而,它同样伴随着灾难性的、日益加剧的网络攻击。因此,学术界和工业界需要利用机器学习的突破性优势来设计和优化有效的入侵检测系统(IDS),以帮助解决这些安全漏洞。目前有两种标准类型的IDS:基于签名IDS和基于异常的IDS基于签名的IDS,特别是Snort入侵检测系统,利用规则的概念进行工作。但创建Snort规则的传统方式非常可能出错且成本高昂;此外,尽管过去几年基于异常的IDS呈指数级增长,但由于高维数据集特征导致的低准确性,因此,可优化、高效且可信赖的IDS系统仍然是一个挑战。另一方面,单个ML分类器无法检测所有类型的新型攻击,昂贵的标记训练数据集、对数据集的错误使用、显著的误报率(FAR)以及过度的模型构建和测试都对目前基于异常的IDS(方案)提出了巨大挑战。最后,存在上述问题的IDS方案中产生的大量警报也尚未得出有效且可靠的解决方案。因此,本论文提出了四种有前景的解决方案来应对这些安全和隐私挑战。首先,这项研究提出了一种Snort自动规则生成器(SARG),它利用网络数据包内容自动生成可靠和高效的Snort规则,而无需太多人工干预,并且,我们评估了生成的Snort规则的有效性和可靠性,这些规则产生了有效和高效的结果;其次,本研究提出了一种轻量级且有前途的混合特征选择(HFS)方法,该方法具有集成分类器,可有效选择相关特征并提供一致的攻击分类;另外,我们使用遗传算法、基于规则的引擎的综合优势以及在没有类的情况下特征之间的相关性概念来有效地选择相关的特征子集,这大大降低了模型复杂性并增强了学习算法的泛化性;最后,本研究提出了一种具有包容性且高效的集成分类器,该分类器使用K-means、One-Class SVM、DBSCAN和期望最大化(KODE)作为优化分类器,持续地在恶意实例和常规实例之间进行分类。因此,HFS-KODE使用10倍交叉验证、CICIDS2017、NSL-KDD、UNSW-NB15数据集和各种指标记录了良好的结果。例如,HFS-KODE的性能准确率分别为99.99%、99.73%和99.997%,CIC-IDS2017、NSL-KDD和UNSW-NB15的检测率分别为99.75%、96.64%和99.93%,仅基于11、8、12个选定的特征。同样,HFS-KODE在CIC-IDS2017、UNSW-NB15和NSL-KDD中的FAR分别为0.09、0.11和0.16。除此之外,本研究提出了一种安全事件关联器(SEC),它可以在没有先验知识的情况下有效地接受原始事件(警报),并生成一组更易于管理的警报,以便于分析和解释。实验结果表明,提出的SEC可以有效地分析和管理当前IDS生成的大量无关警报。最后,SARG-SEC和HFS-KODE的实验结果显示出良好的性能,并且与其他相关方法相比更具有竞争优势。

AIM： To examine the gene expression profile of gastric cancer （GC） by combination of laser capture microdissection （LCM） and microarray and to correlate the profiling with histological subtypes. METHODS： Using LCM, pure cancer cells were procured from 45 cancerous tissues. After procurement of about 5 000 cells, total RNA was extracted and the quality of RNA was determined before further amplification and hybridization. One microgram of amplified RNA was converted to cDNA and hybridized to cDNA microarray. RESULTS： Among 45 cases, only 21 were qualified for their RNAs. A total of 62 arrays were performed. These included 42 arrays for cancer （21 cases with dyeswab duplication） and 20 arrays for non-tumorous cells （10 cases with dye-swab duplication） with universal reference. Analyzed data showed 504 genes were differentially expressed and could distinguish cancerous and non-cancerous groups with more than 99% accuracy. Of the 504 genes, trefoil factors 1, 2, and 3 were in the list and their expression patterns were consistent with previous reports. Immunohistochemical staining of trefoil factor 1 was also consistent with the array data. Analyses of the tumor group with these 504 genes showed that there were 3 subgroups of GC that did not correspond to any current classification system, including Lauren＇s dassification. CONCLUSION： By using LCM, linear amplification of RNA, and cDNA microarray, we have identified a panel of genes that have the power to discriminate between GC and non-cancer groups. The new molecular classification and the identified novel genes in gastric carcinogenesis deserve further investigations to elucidate their clinicopathological significance.
