BACKGROUND Antiviral treatment of patients with chronic hepatitis B(CHB)in the grey zone of treatment comands risk management in order to optimize the health *** this sense,the identification of HBV mutants related with an increased risk of hepatocellular carcinoma(HCC)could be useful to identify subpopulations with potential indication of antiviral *** To analyze the prevalence/persistence of hepatitis B virus(HBV)preS and basal core promoter(BCP)/precore/core variants associated to HCC development in CHB patients in the grey *** Work was designed as a longitudinal retrospective study,including 106 plasma samples from 31 patients with CHB in the grey zone of treatment:Hepatitis B e antigen negative,HBV-DNA levels between 12-20000 IU/ml,normal or discordant transaminase levels during follow up and mild/moderate necroinflammatory activity in liver biopsy or Fibroscan(up to 9.5 kPa).Serum HBVDNA was tested using the Abbott Real Time HBV Assay and the BCP/precore/core and the hepatitis B surface antigen(HBsAg)coding regions were analyzed in positive samples by PCR/bulk-sequencing to identify the HCCrelated HBV *** High-risk HCC related mutants were detected in 24(77%)patients:19(61%)in the BCP/precore/core,and 7(23%)in the HBsAg coding region(2 preS1 and 5 preS2 deletions).The prevalence of preS deletions was genotype-dependent:3/5(60%)patients with preS2 deletions and 1/2 with preS1 deletions were infected with the HBV-E *** HBV-E was the most prevalent in sub-Saharan patients,a correlation between preS deletions and ethnicity was also found:6/8(75%)sub-Saharan vs 1/19(5%)Caucasian patients had preS deletions(P=0.00016).Remarkably,this correlation was maintained in those patients infected with HBV-A,a minor genotype in sub-Saharan patients:2/2 patients infected with HBV-A from West Africa vs 0/6 of Caucasian origin had preS *** HCC related variants were the major strains and persisted over time(up to 48 mo).Pa

Occult hepatitis B infection(OBI) is characterized by hepatitis B virus(HBV) DNA in serum in the absence of hepatitis B surface antigen(HBsAg) presenting HBsAg-negative and anti-HBc positive serological *** HBV status is associated in some cases with mutant viruses undetectable by HBsAg assays;but more frequently it is due to a strong suppression of viral replication and gene *** is an entity with world-wide *** failure to detect HBsAg,despite the persistence of the viral DNA,is due in most cases to the strong suppression of viral replication and gene expression that characterizes this"occult"HBV infection;although the mechanisms responsible for suppression of HBV are not well *** majority of OBI cases are secondary to overt HBV infection and represent a residual low viremia level suppressed by a strong immune response together with histological derangements which occurred during acute or chronic HBV *** evidence suggests that it can favour the progression of liver fibrosis and the development of hepatocellular carcinoma.

To the Editor:We sincerely applaud the work done by Gupta et al.,in which they analyzed a single-center experience regarding gallbladder per-foration(GBP)management[1].Currently,there is a lack of cohort studies to adequately describe the approach strategies and man-agement for this pathology,with international guidelines making ambiguous recommendations[2,3].Their study stratified GBP types and provided details regarding clinical presentation,comorbidities,preoperative diagnosis,etiology,imaging techniques,and surgical *** study reported a high conversion rate when a la-paroscopic approach was attempted for cholecystectomy.

Insular marine biotas are often richer in faunal diversity than those from the open sea in the same geo- graphical region. The existence of particular island effects were tested under polar conditions by compar- ing infaunal benthic assemblages on Peter I Island with those of similar latitudes in the open ocean at the Bellingshausen Sea and also from the coast of the Antarctic Peninsula. Sampling was carried out aboard the R/V Hesperides during the expedition named BENTART-2003 from 24 January to 3 March 2003. Benthic samples were collected at 18 stations ranging from 90 to 2 044 m depth, using an USNEl-type box corer （BC） dredge. Representatives of 32 higher taxa of invertebrates were found. Univariate and multivariate analyses revealed various patterns in the data. First, significant differences in polychaete abundance were detected between the stations located in the open sea and the remaining sites （island plus mainland sites）. Bivalve abundances were also distinct between island and mainland sampling sites. Sediment columns taken from the island seafloor exhibited the highest rate of bioturbation by the infauna. These findings confirm the model that islands develop distinct assemblages characterized by the enhancement of the benthic commu- nities even in cold waters. Several abiotic factors were measured simultaneously at the seafloor and along the water column to investigate faunal distribution patterns. Significant correlations were found between the benthic assemblages and a combination of two environmental variables：＂island effect＂（measured as a categorical variable） and the redox state of sediments. Richer and more complex benthic assemblages were found in Peter I Island＇s sea bottom, whereas the more depauperate bottoms remained in the open sea.

Single-atom heterogeneous catalysts based on distinct carbon nitride scaffolds

Carbon nitrides integrating macroheterocycles offer unique potential as hosts for stabilizing metal atoms due to their rich electronic structure. To date, only graphitic heptazine-based polymers have been ***, we demonstrate that palladium atoms can be effectively isolated on other carbon nitride scaffolds including linear melem oligomers and poly(triazine/heptazine imides). Increased metal uptake was linked to the larger cavity size and the presence of chloride ions in the polyimide structures. Changing the host structure leads to significant variation in the average oxidation state of the metal, which can be tuned by exchange of the ionic species as evidenced by X-ray photoelectron spectroscopy and supported by density functional theory. Evaluation in the semi-hydrogenation of 2-methyl-3-butyn-2-ol reveals an inverse correlation between the activity and the degree of oxidation of palladium, with oligomers exhibiting the highest activity. These findings provide new mechanistic insights into the influence of the carbon nitride structure on metal stabilization.

The use of unmanned aerial vehicles(UAV)for forest monitoring has grown significantly in recent years,providing information with high spatial resolution and temporal *** with multispectral sensors allow the use of indexes such as the normalized difference vegetation index(NDVI),which determines the vigor,physiological stress and photo synthetic activity of *** study aimed to analyze the spectral responses and variations of NDVI in tree crowns,as well as their correlation with climatic factors over the course of one *** study area encompassed a 1.6-ha site in Durango,Mexico,where Pinus cembroides,Pinus engelmannii,and Quercus grisea *** images were acquired with UAV and information on meteorological variables was obtained from NASA/POWER *** ANOVA explored possible differences in NDVI among the three *** correlation was performed to identify the linear relationship between NDVI and meteorological *** differences in NDVI values were found at the genus level(Pinus and Quercus),possibly related to the physiological features of the species and their *** grisea had the lowest NDVI values throughout the year which may be attributed to its sensitivity to relative humidity and *** the use of UAV with a multispectral sensor for NDVI monitoring allowed genera differentiation,in more complex forest analyses hyperspectral and liDAR sensors should be integrated,as well other vegetation indexes be considered.

From conventional two-stage hepatectomy to AlPPS:Fifteen years of experience in a hepatobiliary surgery unit

Phytoclimatic Dynamics of Mediterranean Forests under Climate Change. A Case Study in a Southern European <i>Pinus sylvestris</i>l. Stand

Some effects of climate change on the composition and competitive capacity of southern European Pinus sylvestris l. forests in the Mediterranean basin were evaluated. The variation over the period 1910-2008 through 30-year mobile averages of a Phytoclimatic Suitability Index (PSI) of the main tree species of the forest cover are used to indicate the competitive hierarchy of Pinus sylvestris and Fagus sylvatica l. The methodology was applied at a specific location on the Spanish south-facing slopes of the Pyrenees mountain range in the Iberian Peninsula, where the increase in the average temperature was 1.4?C in the period of observation. The results indicated that the apparent equilibrium between the two species studied changed from the 1934-1963 average. Due to the loss of competitive capacity of Scots pine with respect to European beech, particularly from the years 1970-1999, the model predicted an inversion of the situation as it was up until now, so that beech had a higher PSI than pine. The phytoclimatic approach proposed here offers new methodological horizons for the study of the effects of climate change on the future of the forests.
