Single-atom heterogeneous catalysts based on distinct carbon nitride scaffolds

Carbon nitrides integrating macroheterocycles offer unique potential as hosts for stabilizing metal atoms due to their rich electronic structure. To date, only graphitic heptazine-based polymers have been ***, we demonstrate that palladium atoms can be effectively isolated on other carbon nitride scaffolds including linear melem oligomers and poly(triazine/heptazine imides). Increased metal uptake was linked to the larger cavity size and the presence of chloride ions in the polyimide structures. Changing the host structure leads to significant variation in the average oxidation state of the metal, which can be tuned by exchange of the ionic species as evidenced by X-ray photoelectron spectroscopy and supported by density functional theory. Evaluation in the semi-hydrogenation of 2-methyl-3-butyn-2-ol reveals an inverse correlation between the activity and the degree of oxidation of palladium, with oligomers exhibiting the highest activity. These findings provide new mechanistic insights into the influence of the carbon nitride structure on metal stabilization.

Chromitite bodies hosted in peridotites typical of suboceanic mantle(***)are found in the northern and central part of the Loma Caribe peridotite,in the Cordillera Central of the Dominican *** chromitites are massive pods of small size(less than a few meters across)and veins that intrude both dunite and ***,they are high-Cr chromitites[Cr#=Cr/(Cr+Al)atomic ratio=0.71-0.83]singularly enriched in TiO2(up to 1.25 wt.%),Fe2 O3(2.77-9.16 wt.%)as well as some trace elements(Ga,V,Co,Mn,and Zn)and pGE(up to 4548 ppb in whole-rock).This geochemical signature is unknown for chromitites hosted in oceanic upper mantle but akin to those chromites crystallized from mantle plume derived ***,the melt estimated to be in equilibrium with such chromite from the Loma Caribe chromitites is similar to basalts derived from different source regions of a heterogeneous Caribbean mantle *** mantle plume is responsible for the formation of the Caribbean Large Igneous province(CLIp).Dolerite dykes with back-arc basin basalt(BABB)and enriched mid-ocean ridge basalt(E-MORB)affinities commonly intrude the Loma Caribe peridotite,and are interpreted as evidence of the impact that the Caribbean plume had in the off-axis magmatism of the back-arc basin,developed after the Caribbean island-arc extension in the Late *** propose a model in which chromitites were formed in the shallow portion of the back-arc mantle as a result of the metasomatic reaction between the supra-subduction zone(SSZ)peridotites and upwelling plume-related melts.

Optimizing clarithromycin-containing therapy for Helicobacter pylori in the era of antibiotic resistance

The efficacy of triple therapy for Helicobacter pylori infection has dramatically declined over the last decade,largely related to increasing clarithromycin resistance *** a microbiological standpoint,bismuth quadruple therapy is the ideal replacement since it combines drugs for which resistance does not impair its ***,several obstacles such as availability,complexity or tolerance prevent a general implementation of bismuth quadruple therapy,so nonbismuth quadruple regimens remain the best firstline treatment in clinical practice in many geographical *** review the rationale and efficacy of several optimization tools(increasing the length of duration,high-dose acid suppression,probiotics),which have been largely evaluated over the last 5 years to increase the effectiveness of standard triple ***,we update available evidence on the effectiveness of several non-bismuth quadruple therapies(sequential,concomitant,hybrid,miscellaneous therapy),which have gained interest *** also revise evidence on the efficacy of the aforementioned optimization tools for non-bismuth quadruples schemes and,finally we provide a novel regionalized therapeutic algorithm,based on novel formulas recently developed for predicting the outcome of non-bismuth quadruple regimens,upon local antibiotic resistance rates.

Nature and evolution of the lower crust under central Spain:Inferences from granulite xenoliths(Calatrava Volcanic Field-Spanish central system)

So far,the nature and evolution of the lower crust under central Spain have been constrained mainly on the basis of a heterogeneous suite of granulite xenoliths from the Spanish Central System(SCS).In recent years,ultramafic volcanics from the Calatrava Volcanic Field(CVF)have also provided deep-seated crustal xenoliths which have not been studied in *** data,combining mineral,whole-rock and isotopic geochemistry with U–pb–Hf isotope ratios in zircons from the CVF and SCS xenoliths,highlight the felsic composition of the lower crust under central Iberia.A number of the Calatrava xenoliths represents Variscan igneous protoliths,which are a minor population in the SCS,and were likely formed by crystallisation of intermediate and felsic melts in the lower crust during the Variscan orogeny(leucodiorite protolith age of 314±3 Ma and leucogranite protolith age of 308±2.5 Ma).U–pb data of metamorphic zircons show that granulite-facies metamorphism mainly occurred from 299 to 285 Ma in both *** ages are slightly younger than those of granitic intrusions that could be genetically related to the granulitic residue,which points to a main role of U–pb isotope resetting in lower crustal zircons during HT or UHT *** zircon U–pb–Hf isotopic ratios support the idea that the lower crust in central Iberia consists mainly of Ordovician–Neoproterozoic metaigneous and metasedimentary rocks associated with the Cadomian continental arc of northern *** rocks provide evidence of mixing between juvenile magmas and an enriched crustal component,ultimately extracted from an Eburnean *** more evolved components present in detrital zircons are likely related to recycling of Archean crust derived from North Africa cratonic terranes.

Long-term outcomes after stenting as a"bridge to surgery"for the management of acute obstruction secondary tocolorectal cancer

Obstructive symptoms are present in 8% of cases at the time of initial diagnosis in cases of colorectal cancer. Emergency surgery has been classically considered the treatment of choice in these patients. However, in the majority of studies, emergency colorectal surgery is burdened with higher morbidity and mortality rates than elective surgery, and many patients require temporal colostomy which deteriorates their quality of life and becomes permanent in 10%-40% of cases. The aim of stenting by-pass to surgery is to transform emergency surgery into elective surgery in order to improve surgical results, obtain an accurate tumoral staging and detection of synchronous lesions, stabilization of comorbidities and performance of laparoscopic surgery. Immediate results were more favourable in patients who were stented concerning primary anastomosis, permanent stoma, wound infection and overall morbidity, having the higher surgical risk patients the greater benefit. However, some findings laid out the possible implication of stenting in long-term results of oncologic treatment. perforation after stenting is related to tumoral recurrence. In studies with perforation rates above 8%, higher recurrences rates in young patients and lower disease free survival have been shown. On the other hand, after stenting the number of removed lymph nodes in the surgical specimen is larger, patients can receive adjuvant chemotherapy earlier and in a greater percentage and the number of patients who can be surgically treated with laparoscopic surgery is larger. Finally, there are no consistent studies able to demonstrate that one strategy is superior to the other in terms of oncologic benefits. At present, it would seem wise to assume a higher initial complication rate in young patients without relevant comorbidities and to accept the risk of local recurrence in old patients(> 70 years) or with high surgical risk(ASA Ⅲ/Ⅳ).

patient Safety in the Use of Mechanical Restraints: Regulatory Compliance among Hospitals in the City of Buenos Aires and proposal for Its Improvement

Introduction: Mechanical or physical restraint is an exceptional therapeutic resource to immobilize a subject and thus guarantee the safety of the patient and/or third parties in the face of high-risk behaviors, but it entails multiple crossings (bioethical, philosophical, medical, psychological, legal). Framed in the so-called “safety culture” developed by the WHO, based on the protocol for its implementation of the CABA and attentive to its frequent use in CABA by different hospital services (medical clinic, geriatrics, intensive care and medical guards) we consider it necessary its study in terms of compliance with the risks it entails and its management. Objectives: Identify regulatory compliance with the GCABA Mechanical restraint (MR) protocol from a patient safety perspective, as well as describe the clinical and medicolegal aspects, and propose the usefulness of a tool for its management and control. Methodology: Observational, descriptive, transversal and prospective work through the analysis of Clinical Records with indication of MR using a rubric-type form. 177 cases were analyzed between September-November 2023 from three hospitals of the Government of the City of Buenos Aires, statistical parameters were applied and graphs were made. Results: Only 12.99% complied with the protocol. In the mental health specialized hospital compliance was almost 5 times greater than in the general one, and in the emergency services compliance was 12 times greater than in Inpatient services. We found that the start or end time of MR was not recorded and only 43% described the causes/justifications for the indication (mostly in Emergency and Specialized hospitals), with the MR average time being shorter in Emergency. Conclusions: Only 1.3 out of 10 patients reliably completed the protocol and it was mostly in the mental health specialized hospital and the emergency services.
