Background:This study investigates the effects of exercise training on exerkines in patients with type 2 diabetes mellitus to determine the optimal exercise ***:A systematic search for relevant studies was performed in 3 *** controlled trials investigating the effects of exercise training on at least one of the following exerkines were included:adiponectin,apelin,brain-derived neurotrophic factor,fetuin-A,fibroblast growth factor-21,follistatin,ghrelin,interleukin(IL)-6,IL-8,IL-10,IL-15,IL-18,leptin,myostatin,omentin,resistin,retinol-binding protein 4,tumor necrosis factor-α,and ***:Forty randomized controlled trials were selected for data extraction(n=2160).Exercise training induces changes in adiponectin,fetuin-A,fibroblast growth factor-21,IL-6,IL-10,leptin,resistin,and tumor necrosis factor-a levels but has no significant effects on apelin,IL-18,and ghrelin compared to *** exercise training favored large and positive changes in pooled exerkines(i.e.,an overall effect size calculated from several exerkine s)(Hedge’s g=1.02,95%confidence interval(95%CI):0.76-1.28),which in turn were related to changes in glycated hemoglobin(mean difference(md)=-0.81%,95%CI:-0.95%to-0.67%),fasting glucose(md=-23.43 mg/dL,95%CI:-30.07 mg/dL to-16.80 mg/dL),waist circumference(md=-3.04 cm,95%CI:-4.02 cm to-2.07 cm),and body mass(md=-1.93 kg,95%CI:-2.00 kg to-1.86 kg).Slightly stronger effects were observed with aerobic,resistance,or high-intensity interval protocols at moderate-to vigorous-intensity and with programs longer than 24 weeks that comprise at least 3 sessions per week and more than 60 min per ***:Exercise training represents an anti-inflammatory therapy and metabolism-improving strategy with minimal side effects for patients with type 2 diabetes mellitus.

The conversion of somatic cells into pluripotent stem cells via overexpression of reprogramming factors involvesepigenetic remodeling. DNA methylation at a significant proportion of CpG sites in induced pluripotent stem cells(iPSCs) differs from that of embryonic stem cells (ESCs). Whether different sets of reprogramming factors influencethe type and extent of aberrant DNA methylation in iPSCs differently remains unknown. In order to help resolvethis critical question, we generated human iPSCs from a common fibroblast cell source using either the Yamanakafactors (OCT4, SOX2, KLF4 and cMYC) or the Thomson factors (OCT4, SOX2, NANOG and LIN28), and determinedtheir genome-wide DNA methylation profiles. In addition to shared DNA methylation aberrations present in all ouriPSCs, we identified Yamanaka-iPSC (Y-iPSC)-specific and Thomson-iPSC (T-iPSC)-specific recurrent ***, not only were the genomic locations of the aberrations different but also their types: reprogrammingwith Yamanaka factors mainly resulted in failure to demethylate CpGs, whereas reprogramming with Thomsonfactors mainly resulted in failure to methylate CpGs. Differences in the level of transcripts encoding DNMT3b andTET3 between Y-iPSCs and T-iPSCs may contribute partially to the distinct types of aberrations. Finally, de novoaberrantly methylated genes in Y-iPSCs were enriched for NANOG targets that are also aberrantly methylated insome cancers. Our study thus reveals that the choice of reprogramming factors influences the amount, location,and class of DNA methylation aberrations in iPSCs. These findings may provide clues into how to produce humaniPSCs with fewer DNA methylation abnormalities.

Patient and Physician Preferences for Treating Adjuvant Melanoma: A Discrete Choice Experiment

Objective: To evaluate and compare patient and physician preferences for the benefits and risks of currently available adjuvant melanoma treatments. Methods: Patients with stage II/III melanoma and oncologists in the USA were recruited from 6 clinical sites and an online panel to complete a survey. Preferences were assessed using a paired comparison discrete choice experiment that allowed for opt-out (i.e. no treatment). The treatments comprised 7 attributes, each with 3 levels associated with pegylated interferon, high-dose interferon, and ipilimumab. Attributes included efficacy outcomes, dosing regimen, and risks of moderate to severe toxicities. In addition, open-ended maximum acceptable risk (MAR) questions assessed tradeoffs between toxicity risk and efficacy. Results: 142 patients (45 stage II;97 stage III) chose a treatment in 78% of the choice tasks, while physicians (N = 127) chose treatment 79% of the time. The rankings of relative attribute importance were concordant between the patients and physicians for the top 4: 10-year survival in metastatic melanoma, fatigue risk, 3-year recurrence-free survival (RFS), and depression risk. Patients and physicians valued the difference in 21% survival versus no survival benefit about 3 and 4 times as much, respectively, as reducing diarrhea risk from 41% to 1% or reducing depression risk from 40% to 1%. The MAR of severe diarrhea and of a life-threatening event increased as the chance of 3-year RFS increased, with patients reporting higher risks than physicians. Conclusion: Patients and physicians were concordant in their preferences in adjuvant melanoma, preferring treatment versus none and judging potential efficacy to outweigh risks of toxicities.

Searching for the neutrinoless double beta decay(NLDBD)is now regarded as the topmost promising technique to explore the nature of neutrinos after the discovery of neutrino masses in oscillation *** X-III(particle and astrophysical xenon experiment III)will search for the NLDBD of136Xe at the China Jin Ping Underground Laboratory(CJPL).In the first phase of the experiment,a high pressure gas Time Projection Chamber(TPC)will contain 200 kg,90%136Xe enriched gas operated at10 *** pitch micro-pattern gas detector(Microbulk Micromegas)will be used at both ends of the TPC for the charge readout with a cathode in the *** signals can be used to reconstruct the electron tracks of the NLDBD events and provide good energy and spatial *** detector will be immersed in a large water tank to ensure～5 m of water shielding in all *** second phase,a ton-scale experiment,will consist of five TPCs in the same water tank,with improved energy resolution and better control over backgrounds.

AIM: To optimize diagnosis and treatment guidelines for this geographic region, a panel of gastroenterologists, epidemiologists, and basic scientists carried out a structured evaluation of available ***: Relevant questions were distributed among the experts, who generated draft statements for consideration by the entire panel. A modified three-round Delphi technique method was used to reach consensus. Critical input was also obtained from representatives of the concerned medical community. The quality of the evidence and level of recommendation supporting each statement was graded according to United States Preventive Services Task Force ***: A group of ten experts was established. The survey included 15 open-ended questions that were distributed among the experts, who assessed the articles associated with each question. The levels of agreement achieved by the panel were 50% in the first round, 73.3% in the second round and 100% in the third round. Main consensus recommendations included:(1) when available, urea breath and stool antigen test(HpSA) should be used for non-invasive diagnosis;(2) detect and eradicate Helicobacter pylori(H. pylori) in all gastroscopy patients to decrease risk of peptic ulcer disease, prevent o retard progression in patients with preneoplastic lesions, and to prevent recurrence in patients treated for gastric cancer;(3) further investigate implementation issues and health outcomes of H. pylorieradication for primary prevention of gastric cancer in high-risk populations;(4) prescribe standard 14-d triple therapy or sequential therapy for first-line treatment;(5) routinely assess eradication success post-treatment in clinical settings; and(6) select second- and third-line therapies according to antibiotic susceptibility ***: These achievable steps toward better region-specific management can be expected to improve clinical health outcomes.
