Hepatogenous diabetes: Is it a neglected condition in chronic liver disease?

Diabetes mellitus(DM) that occurs because of chronic liver disease(CLD) is known as hepatogenous diabetes(HD). Although the association of diabetes and liver cirrhosis was described forty years ago, it was scarcely studied for long time. patients suffering from this condition have low frequency of risk factors of type 2 DM. Its incidence is higher in CLD of viral, alcoholic and cryptogenic etiology. Its pathophysiology relates to liver damage, pancreatic dysfunction, interactions between hepatitis C virus(HCV) and glucose metabolism mechanisms and genetic susceptibility. It associates with increased rate of liver complications and hepatocellular carcinoma, and decreased 5-year survival rate. It reduces sustained virological response in HCV infected patients. In spite of these evidences, the American Diabetes Association does not recognize HD. In addition, the impact of glucose control on clinical outcomes of patients has not been evaluated. Treatment of diabetes may be difficult due to liver insufficiency and hepatotoxicity of antidiabetic drugs. Notwithstanding, no therapeutic guidelines have been implemented up to date. In this editorial, authors discuss the reasons why they think that HD may be a neglected pathological condition and call attention to the necessity for more clinical research on different fields of this disease.

Hepatocellular carcinoma(HCC) is one of the most common malignancies leading to high mortality rates in the general population; in cirrhotic patients, it is the primary cause of death. The diagnosis is usually delayed in spite of at-risk population screening recommendations, i.e., patients infected with hepatitis B or C virus. Hepatocarcinogenesis hinges on a great number of genetic and molecular abnormalities that lead to tumor angiogenesis and foster their dissemination potential. The diagnosis is mainly based on imaging studies such as computed tomography and magnetic resonance, in which lesions present a characteristic classical pattern of early arterial enhancement followed by contrast medium "washout" in late venous phase. On occasion, when imaging studies are not conclusive, biopsy of the lesion must be performed to establish the diagnosis. The Barcelona Clinic Liver Cancer staging method is the most frequently used worldwide and recommended by the international guidelines of HCC management. Currently available treatments include tumor resection, liver transplant, sorafenib and locoregional therapies(alcoholization, radiofrequency ablation, chemoembolization). The prognosis of hepatocarcinoma is determined according to the lesion's stage and in cirrhotic patients, on residual liver function. Curative treatments, such as liver transplant, are sought in patients diagnosed in early stages; patients in more advanced stages, were not greatly benefitted by chemotherapy in terms of survival until the advent of target molecules such as sorafenib.

The recurrence of hepatitis C after a liver transplant remains an important cause of graft loss and retransplant. Antiviral therapy with peginterferon plus ribavirin (pEG-INF/RBV) can achieve a sustained viral response and histological improvement in a high percentage of cases. However, this treatment is not exempt from important side effects or from the possibility of precipitating rejection, with the resulting graft loss. We report the case of a liver transplant patient who received treatment with pEG-INF/RBV and developed plasma cell hepatitis as the presenting form of rejection.

Background Little information exists about the role of anemia in patients with acute coronary syndromes(ACS)admitted to Intensive Cardiac Care Units(ICCU).The aim of this study was to assess the prevalence of anemia and its impact on management and outcomes in this clinical *** All consecutive patients admitted to eight different ICCUs with diagnosis of non-ST segment elevation ACS(NSTEACS)were prospectively *** was defined as hemoglobinpatients were *** age was 66.6 years.A total of 197 patients(31.3%)had *** angiography was performed in most patients(96.2%).patients with anemia were significantly older,with a higher prevalence of comorbidities,poorer left ventricle ejection fraction and higher GRACE score *** with anemia underwent less often coronary angiography,but underwent more often intraaortic counterpulsation,non-invasive mechanical ventilation and renal replacement *** ICCU and hospital stay were significantly longer in patients with *** the incidence of mortality(HR=3.36,95%CI:1.43–7.85,p=0.001)and the incidence of mortality/readmission were significantly higher in patients with anemia(HR=2.80,95%CI:2.03–3.86,p=0.001).After adjusting for confounders,the association between anemia and mortality/readmission remained significant(p=0.031).Conclusions Almost one of three NSTEACS patients admitted to ICCU had *** patients underwent coronary *** was independently associated to poorer outcomes at 6 months.

This paper analyzes the viability of a new microcogeneration system with a Stirling engine micro-CHp(combined heat and power)and renewable solar energy:thermal and photovoltaic system,using accumulation system for hot water and ion lithium batteries for electricity.A weather station gives real meteorological parameters for Mediterranean climate(Málaga,Spain).A control unit permits to have a full automated system,which works according to a *** controller also allows theorizing a demand profile curve of daily consumption,typical for this or other *** studies analyse different kinds of combined systems by simulating or making the real installation to obtain results for either microcogeneration or solar power,but not together,*** heating applications to obtain a constant consumption of hot *** new system shows a new combination of resources(natural gas in Stirling and solar energy systems),and the implementation allows doing experiments with different profiles of consumption to have real,non-theoretical,*** control system is self-governing:it connects the different supplies of energy(solar thermal,photovoltaic,Stirling or batteries)depending on the demand,this demand can be changed by simulating any profile:domestic(heat and power),business,low thermal demand,low electricity demand,etc.

Background:We used mixed models with random components to develop height-diameter(h-d) functions for mixed,uneven-aged stands in northwestern Durango(Mexico),considering the breast height diameter(d) and stand variables as ***:The data were obtained from 44 permanent plots used to monitor stand growth under forest management in the study ***:The generalized Bertalanffy-Richards model performed better than the other generalized models in predicting the total height of the species under *** the genera pinus and Quercus,the models were successfully calibrated by measuring the height of a subsample of three randomly selected trees close to the mean d,whereas for species of the genera Cupressus,Arbutus and Alnus,three trees were also selected,but they are specifically the maximum,minimum and mean d ***:The presented equations represent a new tool for the evaluation and management of natural forest in the region.
