AIM:To evaluate the in vitro effect of amoxicillin and clarithromycin on the cag pathogenicity island(cag PAI).METHODs:One hundred and forty-nine clinical isolates of Helicobacter pylori(***)cultured from gastric biopsies from 206 Colombian patients with dyspeptic symptoms from a high-risk area for gastric cancer were included as study *** susceptibility was determined by the agar dilution *** isolates at baseline and in amoxicillin and clarithromycin serial dilutions were subjected to genotyping(cagA,vacA alleles s and m),Glu-Pro-Ile-Tyr-Ala(EPIYA)polymerase chain reaction and random amplified polymorphic DNA(RAPD).Images of the RAPD amplicons were analyzed by Gel-Pro Analyzer *** analyses was done using sPss 15.0statistical package,where each of the fingerprint bands were denoted as *** were designed by following Ward’s clustering method and the estimation of distances between each pair of *** isolates was calculated with the squared Euclidean ***:Resistance rates were 4%for amoxicillin and 2.7%for clarithromycin with 2%double *** evidenced a high prevalence of the genotype cagA-positive/vacA *** 3’region of cagA gene was successfully amplified in 92.3%(12/13)of the baseline resistant isolates and in 60%(36/60)of the resistant isolates growing in antibiotic *** observing the distribution of the number of EPIYA repetitions in each dilution with respect to baseline isolates,it was found that in 61.5%(8/13)of the baseline isolates,a change in the number of EPIYA repetitions lowered antibiotic *** gain and loss of EPIYA motifs resulted in a diversity of *** subclones after bacterial adjustment to changing conditions product of antibiotic *** PCR evidenced the close clonal relationship between baseline isolates and isolates growing in antibiotic ***:Antibiotic pressure does not induce loss of the cag pathoge

A trophic state increase relates to surface water bodies nutrient enrichment,due to the chemical products used such as fertilizers in agriculture and residues from cattle raising *** research consists of nutrient transport numerical modeling to analyze the Betancíreservoir trophic state in Colombia;Water samples were collected to analyze total nitrogen,total phosphorus,ammonia,nitrates,nitrites,phosphates,chlorophyll-a,dissolved oxygen,BOD,COD,suspended solids,and water *** water quality model MOHID studio was implemented and after its calibration,scenarios of increase and decrease of nutrients and inflows to the reservoir through its main tributaries were simulated to reproduce the agricultural activity changes in the basin and the effects that would have on the reservoir's trophic state dynamics;Therefore,the Carlson Trophic status Index was calculated for each *** results show that variations in total nitrogen concentration and increased inflows present short-term consequences on the reservoir's trophic *** the incoming total nitrogen con-centrations by 100%causes the reservoir to change from a light eutrophic to a hypereutrophic *** results of this research provide a starting tool to water resources integrated management in reservoirs.

Helicobacter pylori (H pylori) infection is the main cause of gastritis, gastroduodenal ulcer disease, and gastric cancer. After more than 20 years of experience in H pylori treatment, in my opinion, the ideal regimen to treat this infection is still to be found. Currently, apart from having to know first-line eradication regimens well, we must also be prepared to face treatment failures. Therefore, in designing a treatment strategy we should not focus on the results of primary therapy alone, but also on the final (overall) eradication rate. The choice of a "rescue" treatment depends on which treatment is used initially. If a clarithromycin- based regimen was used initially, a subsequent metronidazole-based treatment (quadruple therapy) may be used afterwards, and then a levofloxacin- based combination would be a third "rescue" option. Alternatively, it has recently been suggested that levofloxacin-based rescue therapy constitutes an encouraging second-line strategy, representing an alternative to quadruple therapy in patients with previous PPI-clarithromycin-amoxicillin failure, with the advantage of efficacy, simplicity and safety. In this case, a quadruple regimen may be reserved as a third-line rescue option. Finally, rifabutin-based rescue therapy constitutes an encouraging empirical fourth- line strategy after multiple previous eradication failures with key antibiotics such as amoxicillin, clarithromycin, metronidazole, tetracycline, and levofloxacin. Even after two consecutive failures, several studies have demonstrated that H pylori eradication can finally be achieved in almost all patients if several rescue therapies are consecutively given. Therefore, the attitude in H pylori eradication therapy failure, evenafter two or more unsuccessful attempts, should be to fight and not to surrender.
