Strategies to prevent necrotising enterocolitis

Necrotizing enterocolitis （NEC） is the most common gastrointestinal emergency and carries with it both morbidity, short and long term, as well as mortality.1,2 It is a world-wide problem in preterm infants with a highly variable incidence varying from 2.6% to 28%.1 It affects 1.0%-7.7% of NICU admissions and is primarily a disease of premature infants with over 90% of cases occurring in infants bom prematurely. In the National Institute of Child Health and Human Development Neonatal Research Network, NEC was noted to occur in 11.5% in infants weighing 401-750 g, 9% in 751-1000 g,6% in 1001-1250 g and 4% in 1251-1500 g.

How does the neonatal immune system decide which of the millions and millions of microbes it encounters during and soon after birth are commensal and which are pathological?This is not a trivial *** vigorous responses to commensal bacteria cause unnecessary collateral tissue damage and loss of the potentially beneficial microbial flora needed for host *** the other hand,insufficient immune defense against invading,harmful,microbes is also very problematic,if not lethal.1 While the mechanisms for self-tolerance have been the subject of extensive investigations since the mid-1950s,such is not the case for selective unresponsiveness to commensal microbial flora residing on the skin or the mucosa of the gastrointestinal track.

Bacopa monnieri extracts prevent hydrogen peroxide- induced oxidative damage in a cellular model of neuroblastoma IMR32 cells

Neurodegenerative diseases are the consequences of imbalance between the production of oxidative stress and its nullification by cellular defense mechanisms. Hydrogen peroxide(H_2O_2), a precursor of deleterious reactive oxygen species, elicits oxidative stress, resulting in severe brain injuries. Bacopa monnieri is well known for its nerve relaxing and memory enhancing properties. The present study was designed to evaluate the protective effects of extracts from Bacopa monnieri against H_2O_2 induced oxidative stress using a cellular model, neuroblastoma IMR32 cell line. The protective potential of methanolic, ethanolic, and water extracts of B. monnieri(BM-MEx, BM-EEx, and BM-WEx) was evaluated using MTT assay. Although, all the B. monnieri extracts were found to protect cells against H_2O_2-mediated stress but BM-MEx showed significantly greater protection. UPLC analysis of BM-MEx revealed various polyphenols, including quercetin, catechin, umbelliferone, and caffeic acid predominance. Further, BM-MEx was found to possess considerable greater neuroprotective potential in comparison to the standard polyphenols such as quercetin, catechin, umbelliferone, and caffeic acid. The levels of antioxidant enzymes were significantly elevated after the pretreatment of BM-MEx and quercetin. The expression levels of oxidative stress markers, such as NF200, HSP70, and mortalin, were significantly alleviated after the pretreatment of BM-MEx as shown by immunofluorescence and RT-PCR. In conclusion, the present study demonstrated the protective effects of BM-MEx, suggesting that it could be a candidate for the development of neuropathological therapeutics.

mpaired flow-mediated vasodilatation in Asian Indians with erectile dysfunction

Endothelial dysfunction is the postulated link between coronary artery disease （CAD） and erectile dysfunction （ED）. Brachial artery flow-mediated vasodilatation （FMD） is a non-invasive surrogate marker for endothelial function assessment. Despite Asian Indians representing a considerable global CAD burden, data on FMD and ED in these patients are lacking. Of the 225 patients undergoing coronary angiography, 72% had ED （assessed using the International Index of Erectile Function （IIEF-5） questionnaire）; ED was moderate to severe in 61% of the patients. ED patients had a higher incidence of severe and diffuse angiographic CAD, a greater number of coronary vessels involved and a lower mean brachial artery FMD （6.40%±4.60% vs. 9.10%±4.87%, P〈0.001） compared to non-ED patients. A progressive reduction in FMD was noted with increasing severity of ED. Impaired FMD （ ≤ 5.5%） was twice as common in ED patients （52% vs. 24% without ED）. Patients with impaired FMD had higher ED prevalence （85% vs. 62%） and lower mean I IEF-5 scores compared to those with normal FMD. Impaired FMD was a significant ED predictor independent of other risk factors （odds ratio, 2.33; 95% confidence interval： 0.59-9.23; P=0.03）. An inverse correlation between FMD and ED severity was observed （r=-0.22; P=0.004）. ED is common among Asian Indians with angiographically documented CAD. Patients with ED have impaired FMD independent of other risk factors, suggesting that endothelial dysfunction is the underlying pathophysiology. Urologists and cardiologists need to be aware of the association between ED, CAD and endothelial dysfunction.
