Prediction of esophageal and gastric histology by macroscopic diagnosis during upper endoscopy in pediatric celiac disease

AIMTo determine the sensitivity of macroscopic appearance for predicting histological diagnosis at sites other than duodenum in pediatric celiac disease (CD).METHODSEndoscopic and histologic findings in pediatric patients undergoing upper endoscopy for first-time diagnosis of CD at Stollery Children’s Hospital from 2010-2012 were retrospectively *** charts from 140 patients were reviewed. Esophageal and gastric biopsies were taken in 54.3% and 77.9% of patients, respectively. Endoscopic appearance was normal in the esophagus and stomach in 75% and 86.2%. Endoscopic esophageal diagnoses were eosinophilic esophagitis (EE) (11.8%), esophagitis (7.9%), glycogenic acanthosis (1.3%) and non-specific abnormalities (3.9%). Endoscopic gastric diagnoses were gastritis (8.3%), pancreatic rest (0.9%), and non-specific abnormalities (4.6%). Histology was normal in 76.3% of esophageal and 87.2% of gastric specimens. Abnormal esophageal histology was EE (10.5%), esophagitis (10.5%), glycogenic acanthosis (1.3%) and non-specific (1.3%). Gastritis was reported in 12.8% of specimens. Sensitivity and specificity of normal endoscopy for predicting normal esophageal histology was 86.2% and 61.1%, and for normal gastric histology was 87.4% and 21.4%.CONCLUSIONIn the absence of macroscopic abnormalities, routine esophageal and gastric biopsy during endoscopy for pediatric CD does not identify major pathologies. These findings have cost and time saving implications for clinical practice.

Objective:The main objective of this initiative was to present evaluation results from an inno-vative adaptation of the Advanced Life Support in Obstetrics(ALSO)training *** modified the traditional ALSO curriculum in our institution by adding hands-on training in laceration repairs and simulation scenarios on acute maternity ***:The modified ALSO provider course was designed to enhance cognitive and proce-dural skills of health care professionals in managing obstetric ***-nine participants attended this course and completed a posttraining *** statistics were used to de-scribe the participant-reported assessment scores for the ALSO course on three domains(subject knowledge,organization and clarity,and teaching effectiveness)for each of 12 course ***:Evaluation of the results showed a high rate of trainee satisfaction as evidenced by the mean assessment scores across all topics ranging from 4.80 to 4.98(out of 5.00).All trainees said they would refer others to the *** modified ALSO course effectively addressed the important needs of primary care physicians involved in maternity care,especially in underserved communities where specialized obstetric care is not readily *** simulation scenarios and workshops using simulated human tissue provide a better foundation before formal ***:Given the changing legal and regulatory climate,we expect that learning to treat complex obstetric situations on the job will become increasingly *** this in mind,both simulation scenarios and workshops using simulated human tissue will provide a better foundation before formal training.

Locoregionally advanced laryngeal and hypopharyngeal cancers(LA-LHCs)are traditionally treated with surgery followed by postoperative radiotherapy,which impairs speech and swallowing functions and reduces the quality of life.^(1,2)The use of induction chemotherapy(IC)as a larynxpreserving approach for LA-LHCs has been verified and refined.^(3)However,the short-term tumor response to IC varies,non-responders usually show poor survival and little benefit.

Introduction:The establishment of a functional trophectoderm(TE) epithelium is an essential prerequisite for blastocyst formation and *** coactivator yes-associated protein(yap),a downstream effector of the hippo signaling pathway,is required for specification of both the TE and epiblast lineages in ***,the biological role of yap in porcine blastocyst development is not ***,we took pig early embryo as the research object,aiming to reveal the dynamic expression rule of yap in pig early embryos,and to explore the effect of yap gene knockdown on pig preimplantation embryo development,so as to provide a basis for elucidating the role and mechanism of HIPPO signaling pathway on pig early embryo ***:Firstly,oocytes and early embryos were collected and yap expression was detected by real-time quantitative PCR(RT-qPCR) and immunofluorescence(IF).Secondly,three yap-targeting siRNA was designed,the expression of yap was knocked down by microinjection,and the treatment of embryos with ***,yap knockdown embryos in morula stage were collected,and RT-qPCR and IF techniques were used to detect the expression of related proteins such as lineage differentiation and tight ***,yap knockdown in a single blastomere at the 2-cell stage with ***:Maternal derived yap protein is localized to both the cytoplasm and nuclei prior to the morula stage and is then predominantly localized to the TE nuclei in ***,maternal yap knockdown severely impeded blastocyst formation and perturbed the allocation of the first two *** treatment of embryos with verteporfin,a pharmacological inhibitor of yap,faithfully recapitulated the phenotype observed in yap deleted ***,we found that maternal yap regulates multiple genes which are important for lineage commitment,tight junction assembly,and fluid *** with the effects on tight junction ge
