Therapeutics of Integrative Medicine Ameliorate Immunological Disorders of the Nervous System: A Meta-Analysis

Objective: The objective of the study is to evaluate the therapeutic efficacy of integrative medicine in the treatment of immunological disorders of the nervous system. Methods: The Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines were adopted to conduct this study, which included randomized controlled trials with a confirmed diagnosis of multiple sclerosis(MS), myasthenia gravis(MG), and Guillain–Barre syndrome(GBS), all of which were treated with integrative medicine. The effective rate, recurrent frequency, and disease score were used as the markers of outcome variables for the meta-analysis. Results: A total of 48 randomized control trials were included. The effective rates of treatment with integrative medicine were noticeably higher than those with Western medicine alone for the three diseases. The recurrence frequency for MS and the recurrence rate for MG treated with integrative medicine were reduced more than those with Western medicine alone. The Extended Disability Status Scale, acetylcholine receptor antibody, and Hughes scores for MS, MG, and GBS, respectively, treated with integrative medicine were significantly lower than those with Western medicine alone. The risks of bias in the literature evaluation showed that the quality of the included studies was not high. Conclusions: Compared to treatment with Western medicine alone, integrative medicine might ameliorate immunological disorders of the nervous system.

High-speed ophthalmic optical coherence tomography(OCT)systems are of interest because they allow rapid,motion-free,and wide-field retinal ***-division multiplexing optical coherence tomography(SDMOCT)is a high-speed imaging technology that takes advantage of the long coherence length of micro electro mechanical vertical cavity surface emitting laser sources to multiplex multiple images along a single imaging *** demonstrate wide-field retinal OCT imaging,acquired at an effective A-scan rate of 800,000 A-scans/s with volumetric images covering up to 12.5 mm×7.4 mm on the retina and captured in less than 1 s.A clinical feasibility study was conducted to compare the ophthalmic SDM-OCT with commercial OCT systems,illustrating the high-speed capability of SDM-OCT in a clinical setting.

Whole-exome sequencing and microRNA profiling reveal PI3K/AKT pathway＇s involvement in juvenile myelomonocytic leukemia

Background： Clinical studies and genetic analyses have revealed that juvenile myelomonocytic leukemia （JMML） is caused by somatic and/or germline mutations of genes involved in the RAS/MAPK signalling pathway. Given the vastly different clinical prognosis among individual patients that have had this disease, mutations in genes of other pathways may be involved. Methods： In this study, we conducted whole-exome and cancer-panel sequencing analyses on a bone marrow sample from a 2-year old juvenile myelomonocytic leukemia patient. We also measured the microRNA profile of the same patient＇s bone marrow sample and the results were compared with the normal mature monocytic cells from the pooled peripheral blood. Results： We identified additional novel mutations in the Pi3K/AKT pathway and verified with a cancer panel targeted sequencing. We have confirmed the previously tested PTPNII gene mutation （exon 3 181G〉T） in the same sample and identified new nonsynonymous mutations in NTRKI, HMGA2, MLH3, MYH9 and AKT1 genes. Many of the microRNAs found to be differentially expressed are known to act as oncogenic MicroRNAs （onco^MicroRNAs or oncomiRs）, whose target genes are enriched in the PI3K/AKT signalling pathway. Conclusions： Our study suggests an alternative mechanism for JMML pathogenesis in addition to RAS/MAPK pathway. This discovery may provide new genetic markers for diagnosis and new therapeutic targets for JMML patients in the future.

Safety and Performance of Harmonic®HD 1000i Shears in Thoracoscopic Procedures: A Retrospective Study

Background: Ultrasonic energy devices are utilized for transection, incision, and hemostasis in traditional open and laparoscopic procedures. The Harmonic HD 1000i Shears, designed to deliver a precise amount of thermal energy during tissue transection and vessel sealing, has been utilized in many specialties. This study aimed to confirm real-world safety and performance of the Harmonic device in two thoracoscopic procedures: lobectomy and segmentectomy. Methods: The primary endpoint of this retrospective, observational, single-arm study was rate of post-operative blood transfusions related to study device or procedure. Secondary endpoints included occurrence of intra- and post-operative adverse events (AEs) or complications device- or procedure-related, and rate of required additional hemostatic measures. Adults included those who underwent thoracoscopic lobectomy or segmentectomy where HD 1000i shears were used while excluding those where additional advanced energy devices were used. The study was conducted at Severance Hospital, Yonsei University Health System, South Korea from May 1, 2018, to November 30, 2020. Results: Subjects included n = 766 lobectomies (mean age 63.79, 52% male) and n = 215 segmentectomies (mean age 63.19, 54% male). Estimated blood loss was 50 mL (0 min, 3200 max) and 20 mL (0 min, 800 max), intraoperative transfusion rate 0.001% and 0%, intraoperative complication/AE rate 1% and 2%, and post-operative complication/AE rate 9% and 4% in the lobectomy and segmentectomy groups, respectively. Median operative times were 108 min. (35 min, 395 max) for lobectomies and 105 min. (32 min, 574 max) for segmentectomies. Conclusion: Given the low rate of blood loss and intra- and post-operative complication/AE rates, HD 1000i can be used confidently for thoracoscopic pulmonary resection in adults.

Conversion-type reaction anode materials with high specific capacity are attractive candidates to improve lithium ion batteries(LIBs), yet the rapid capacity fading and poor rate capability caused by drastic volume change and low electronic conductivity greatly hinder their practical applications. To circumvent these issues, the successful design of yolk@shell Fe2 O3@C hybrid composed of a columnar-like Fe2O3 core within a hollow cavity completely surrounded by a thin, self-supported carbon(C) shell is presented as an anode for high-performance LIBs. This yolk@shell structure allows each Fe2O3 core to swell upon lithiation without deforming the carbon shell. This preserves the structural and electrical integrity against pulverization, as revealed by in situ transmission electron microscopy(TEM) measurement. Benefiting from these structural advantages, the resulting electrode exhibits a high reversible capacity(1013 m Ah g-1 after80 cycles at 0.2 A g-1), outstanding rate capability(710 m Ah g-1 at 8 A g-1) and superior cycling stability(800 m Ah g-1 after 300 cycles at 4 A g-1). A Li-ion full cell using prelithiated yolk@shell Fe2 O3@C hybrid as the anode and commercial Li CoO2(LCO) as the cathode demonstrates impressive cycling stability with a capacity retention of 84.5% after 100 cycles at 1 C rate, holding great promise for future practical applications.

Forest succession in post-agricultural Larix olgensis plantations in northeast China

In order to understand the management of regional vegetation,numerical classification and ordination are widely used to investigate community distribution and vegetation *** particular,two-way indicator-species analysis programs(TWINSPAN)classifies plots and species into different ***-trended correspondence analysis(DCA)and canonical correspondence analysis(CCA)reflects the relationship between community and site *** with Larix olgensis *** is a suitable restoration strategy on post-agricultural fields in the Lesser Khingan *** results of this study show how these plantations develop over time to establish a reliable pathway model by measuring and clarifying the succession ***-eight *** plantations along a 48-year chronosequence of afforestation were investigated with a quadrat sampling *** composition,community structure attributes of diversity,and site conditions were *** were classified by TWINSPAN into five successional stages:immature,juvenile,mid-aged,nearmature and *** were validated by DCA and CCA *** conditions such as soil and litter thickness,soil organic matter,soil density,and pH were *** stages varied in community composition and species population,accompanied by time from afforestation and a gradient of site *** gradient showed changes in vegetation occurrence and diversity coinciding with changes in soil *** study showed that *** plantations had marked predominance in growth and were associated with improved soil fertility and the formation of a stable plant community.

球床式高温气冷堆采用了球形燃料元件,燃料区域由石墨基体和弥散在其中的包覆燃料颗粒构成,其外有与石墨基体相同材料的包壳;燃料球堆叠成填充率约为0.61的球床式堆芯活性区。在堆芯物理计算中,必须考虑其特殊的双重非均匀性结构对共振计算的影响。此外,由于石墨起到了中子慢化和结构材料的重要作用,其截面参数的准确性对共振计算和临界计算均有很大影响。本文采用蒙特卡罗中子输运计算程序SCALE/KENO-Ⅵ和Serpent-2,对比分析了ENDF/B Ⅶ.0和ENDF/B Ⅶ.1版本核数据库对不同燃料模型的有效增殖因数(keff)及反应率的影响,并进一步比较了不同双重非均匀性处理方法对计算结果的影响。结果表明,由于石墨吸收率增大,使用ENDF/B Ⅶ.1版本核数据库所得keff小于使用ENDF/B Ⅶ.0版本核数据库的结果,且计算模型中石墨材料越多,计算结果相差越大:对于包覆颗粒模型差别约为200pcm,对于燃料元件约为700pcm,对于堆芯单元约为1 600pcm。SCALE/KENO-Ⅵ程序使用DOUBLEHET模型进行多群蒙特卡罗计算所得结果与连续能量模型计算结果吻合良好,且计算效率高,对燃料球模型而言可节省约85%的计算时间。
