Exercise catheterization for Hemodynamic Evaluation of Adults with coarctation of the Aorta

Background: coarctation of the aorta (coA) is associated with a generalized arteriopathy and long-term complicationsdespite repair. Data on invasive exercise hemodynamics in this population are lacking. Accordingly, wereviewed adults with coA undergoing exercise catheterization to assess 1. hemodynamic profile;2. feasibilityfor assessment of coA severity. Methods: Twenty patients undergoing exercise cardiac catheterization (12 armadduction and 8 supine cycle ergometry) at a quaternary care center between 2004 and 2021 were identified. Restingand exercise hemodynamic data were abstracted from the procedure logs. Results: Mean age was 43.6 ±12.0 years. Eleven patients (55%) had resting pulmonary arterial wedge pressure (PAWP) >15 mmHg;amongthose undergoing arterial catheterization, left ventricular end-diastolic pressure was >15 mmHg in 63%. Elevenpatients (55%) had pulmonary hypertension: 7 (35%) combined and 4 (20%) isolated post-capillary. At peak exercise,ΔPAWP/Δcardiac output (cO) ≥2 and Δmean pulmonary artery pressure/ΔcO ≥3 mmHg/l/min were foundin 7 (78%) and 6 (67%) patients, respectively;the composite of exercise PAWP ≥25 mmHg or ΔPAWP/ΔcO>2 was seen in 12 (86%). coA peak-to-peak gradients at baseline (n = 14) and during exercise (n = 9) were 12(3–16) and 16 mmHg (9–28), respectively. Only 2 patients had an increase in coA gradient to >20 mmHg withexercise. conclusions: Diastolic dysfunction and pulmonary hypertension were highly prevalent, with exerciseunmasking abnormal diastolic and pulmonary vascular reserve in some individuals. Most patients failed to showsignificant increases in coA peak-to-peak gradients with exercise. Further studies are warranted to establish thebest diagnostic method for coA severity assessment.

Background:The utility of neoadjuvant treatment for resectable pancreas cancer is yet to be determined,but has commonly included *** evaluated outcomes in patients with radiographically resectable pancreatic adenocarcinoma treated with neoadjuvant chemotherapy without ***:A retrospective review of patients in our institutional pancreatic cancer registry was performed,which identified 36 patients who received neoadjuvant chemotherapy alone for resectable pancreatic adenocarcinoma between 2012 and ***:Median age at diagnosis was 66.3 *** regimens included gemcitabine(n=17),gemcitabine/nab-paclitaxel(n=8),or 5-FU/leucovorin/irinotecan/oxaliplatin(FOLFIRINOX)(n=11).Surgical resection was performed in 69%of patients(n=25),with an R0 resection rate of 92%(n=23 patients).During chemotherapy,distant disease became apparent in 19%of patients(n=7),while no patients had evidence of local *** rates were similar between chemotherapy regimens(single agent=59%,multiple agent=79%).Median overall survival for all patients who received neoadjuvant chemotherapy was 30.3 and 34.4 months for those who underwent surgical *** was no difference in median survival for patients treated with gemcitabine(31.3 months)or multi-agent chemotherapy(29.7 months).conclusions:A short course of neoadjuvant chemotherapy without chemoradiation may improve patient selection prior to surgical resection for pancreas ***,local disease progression did not limit surgical resection in this small series.

Background: The US FDA has recently approved autologous cultured fibroblast cells (AcF) as treatment for cutaneous contour defects. AcF provides an alternative to synthetic fillers or fat grafting with the significant advantage of producing longer lasting effect. Methods: This was a prospective open label single group clinical study to demonstrate the clinical efficacy of AcF that we have replicated in our lab. The study enrolled 18 patients with nasolabial folds from 2 centres in Malaysia in 2011-2012. AcF at dose of 23 or 92 million cells were injected on 3 occasions at 4 weeks interval. Efficacy at 6- and 12-month post treatment was assessed using a standardized 7-point scale which was performed by the 2 investigators as well as a panel of 7 independent evaluators. Results: We obtained a successful outcome (primary endpoint), defined as improvement in the appearance of the nasolabial fold of at least 2 points on the 7-point scale, in 33% of patients as assessed by investigators at 12-month follow-up and in 22% of patients assessed by independent evaluators. Investigators also found 78% of patients having had a 1-point improvement from baseline while independent evaluators found 83% of patients having improved by as much. Patients were uniformly (100%) satisfied with the improvement in their appearance. No treatment-related adverse event was reported. Baseline score and follow-up duration had significant effect on treatment response. The greater the baseline severity and the longer the post-treatment follow-up, the better the response to AcF. Larger dose of cells (92 million cells) has little additional effect suggesting that the 1 mL dose (23 million cells) is near optimal. Older patients showed a trend towards poorer response but this was not statistically significant. conclusions: We have successfully replicated the laboratory method and clinical procedure to perform AcF treatment which was effective in improving the appearance of nasolabial folds in some patie

Herein, we reported the microemulsion polymerization in supercritical carbon dioxide. With the aid of an anionic phosphate fluorosurfactant （bis-[2-（F-hexyl）ethyl]phosphate sodium）, water-soluble/cO2-insoluble acryloxyethyltrimethyl ammonium chloride monomer and N,N＇-methylene-bisacrylamide cross-linker were solubilized into cO2 continuous phase via the formation of water-in-cO2 （w/c） microemulsion water pools. Initiated by a cO2-soluble initiator, 2,2＇-azo-bisisobutyronitrile （AIBN）, cross-linked poly（acryloxyethyltrimethyl ammonium chloride） particles were produced and stabilized in these w/c internal water pools. Nano-sized particles with sizes less than 20 nm in diameter and narrow particle size distributions were obtained.

Hardware/software co-verification is how to make sure that embedded system software works correctly with the hardware, and that the hardware has been properly designed to run the software successfully -before large sums are spent on prototypes or manufacturing. Aims Geographic patterns of the intensity of plant herbivory in relation to climate factors have garnered little general support and appear to be species ***,plant-herbivore interactions are also driven by resource availability,such as soil nutrient content,and it remains unclear whether broad-scale variation in soil factors is reflected in herbivore consumption rates across species’***,we know little of how intraspecific variation in tissue quality associates with edaphic and climatic factors,and how this variation controls herbivore *** resource availability hypothesis(RAH)predicts that plant individuals growing in low-resource environments will have lower leaf nutritional quality and more constitutive defenses,which will result in lower rates of leaf *** We collected leaves from the old-field dominant species,Solidago altissima L.,from 20 sites across 10 degrees of latitude in the Eastern USA to determine the percentage leaf area consumed by insect *** obtained soil and climate data for each site,as well as plant functional and defensive traits,including specific leaf area(SLA),leaf carbon:nitrogen(c:N),and trichome *** Findings Although we found no significant latitudinal trend of leaf consump-tion rate,there was strong evidence that leaf herbivory decreased with leaf c:N and trichome density,which themselves decreased with soil N,supporting our hypothesis that the RAH applies for intraspecific variation across spatial ***,high precipitation seasonality and soil nitrogen predicted decreased *** results suggest that spatial variation in herbivory can be driven by factors other than herbivore communities and climatic gradients,and that bottom-up processes,where plant traits and soil fertility control leaf consumption,must be incorporated into spatial predictions of herbivory.
