New Era: Endoscopic treatment options in obesity–a paradigm shift

The prevalence of obesity continues to rise,and along with it comes a multitude of health-related *** healthcare community has consistently struggled with providing treatment options to obese patients,in part due to the reluctance of patients in pursuing the more effective(yet invasive)surgical approaches such as sleeve gastrectomy and Rou-en-Y gastric *** the other hand,the less invasive approach such as lifestyle/behavioral interventions and pharmacotherapy(Orlistat,Phenteramine,Phentermine/Topiramate,Locaserin,Naltrexon/Buproprion,and Liraglutide)have very limited efficacy,especially in the morbidly obese *** our best efforts,the epidemic of obesity continues to rise and pose enormous costs on our healthcare system and *** endoscopy is an evolving field generated to combat this epidemic through minimally invasive *** procedures can be performed in an ambulatory setting,are potentially reversible,repeatable,and pose less complications than their invasive surgical *** modalities are designed to alter gut metabolism by means of space occupation,malabsorption,or *** this review we will discuss different bariatric endoscopic options(such as intragastric balloons,endoscopic sleeve gastroplasty,endoscopic aspiration therapies and gastrointestinal bypass sleeves),their advantages and disadvantages,and suggest a new paradigm where providers may start incorporating this modality in their treatment approach for obese patients.

In this paper we propose and test a staged knapsack model to create a supplier portfolio for *** first two stages in the model use judgement criteria related to supplier characteristics and supplier fit,gathered from managers to create a portfolio that optimizes supplier rank and fit *** the third stage we optimize cost by considering landed costs for each supplier in the portfolio to arrive at the final set of *** results indicate that the criteria used,and the staged refinement of the supplier pool resulted in a better fit than just considering the landed costs of the final pool of suppliers without rank and fit score *** proposed method will enable purchasing managers to strategically create a supplier portfolio that will suit the retailer’s needs more effectively.

Dramatic decline of observed atmospheric CO_(2) and CH_(4) during the COVID-19 lockdown over the Yangtze River Delta of China

The temporal variation of greenhouse gas concentrations in China during the COVID-19 lockdown in China is analyzed in this work using high resolution measurements of near surface ΔCO_(2),ΔCH_(4) andΔCO concentrations above the background conditions at Lin’an station(LAN),a regional background station in the Yangtze River Delta *** the pre-lockdown observational period(IOP-1),bothΔCO_(2) andΔCH_(4) exhibited a significant increasing trend relative to the 2011-2019 climatological *** reduction ofΔCO_(2),ΔCH_(4) andΔCO during the lockdown observational period(IOP-2)(which also coincided with the Chinese New Year Holiday)reached up to 15.0 ppm,14.2 ppb and 146.8 ppb,respectively,and a reduction ofΔCO_(2)/ΔCO probably due to a dramatic reduction from industrial emissions.ΔCO_(2),ΔCH_(4) and ΔCO were observed to keep declining during the post-lockdown easing phase(IOP-3),which is the synthetic result of lower than normal CO_(2) emissions from rural regions around LAN coupled with strong uptake of the terrestrial ***,the trend reversed to gradual increase for all species during the later easing phase(IOP-4),with ΔCO_(2)/ΔCO constantly increasing from IOP-2 to IOP-3 and finally IOP-4,consistent with recovery in industrial emissions associated with the staged resumption of economic *** average,ΔCO_(2) declined sharply throughout the days during IOP-2 but increased gradually throughout the days during *** findings showcase the significant role of emission reduction in accounting for the dramatic changes in measured atmosphericΔCO_(2) and ΔCH_(4) associated with the COVID-19 lockdown and recovery.

An update–tissue engineered nerve grafts for the repair of peripheral nerve injuries

Peripheral nerve injuries（PNI） are caused by a range of etiologies and result in a broad spectrum of disability. While nerve autografts are the current gold standard for the reconstruction of extensive nerve damage, the limited supply of autologous nerve and complications associated with harvesting nerve from a second surgical site has driven groups from multiple disciplines, including biomedical engineering, neurosurgery, plastic surgery, and orthopedic surgery, to develop a suitable or superior alternative to autografting. Over the last couple of decades, various types of scaffolds, such as acellular nerve grafts（ANGs）, nerve guidance conduits, and non-nervous tissues, have been filled with Schwann cells, stem cells, and/or neurotrophic factors to develop tissue engineered nerve grafts（TENGs）. Although these have shown promising effects on peripheral nerve regeneration in experimental models, the autograft has remained the gold standard for large nerve gaps. This review provides a discussion of recent advances in the development of TENGs and their efficacy in experimental models. Specifically, TENGs have been enhanced via incorporation of genetically engineered cells, methods to improve stem cell survival and differentiation, optimized delivery of neurotrophic factors via drug delivery systems（DDS）, co-administration of platelet-rich plasma（PRP）, and pretreatment with chondroitinase ABC（Ch-ABC）. Other notable advancements include conduits that have been bioengineered to mimic native nerve structure via cell-derived extracellular matrix（ECM） deposition, and the development of transplantable living nervous tissue constructs from rat and human dorsal root ganglia（DRG） neurons. Grafts composed of non-nervous tissues, such as vein, artery, and muscle, will be briefly discussed.

The opportunity cost of androgen suppression in locally advanced prostate cancer

The use of androgen suppression ther- apy （AST） and radiotherapy for locallyadvanced prostate cancer has become the standard of care worldwide. At the same time, it has become clear that AST carries significant risk for side effects. Recently, Denham and colleagues have reported ini- tial quality of life （QoL） results from the TROG 03.04 RADAR trial. The authors identify clinically meaningful decrements in patient-reported QoL for those treated with 18 months of AST vs. 6 months but only marginal differences at 36 months. Once survival data becomes available, these data will help to frame any benefits seen for longer courses of AST.

A pathophysiology-based approach to the management ot early priapism

Priapism is a rare condition that involves persistent penile erection for greater than 4 h. Distinct variants exist, each with unique characteristics. Ischemic priapism is a painful medical emergency that may occur as a result of veno-occlusion leading to hypoxia and tissue death. Recurrent bouts of ischemic priapism, or stuttering priapism, require treatment for individual attacks as well as long-term prevention. Non-ischemic priapism is associated with trauma and may be managed conservatively. Recent advances into the pathophysiology of priapism have allowed the development of treatment algorithms that specifically target the mechanisms involved. In this review, we outline the basics of smooth muscle contraction and describe how derangement of these pathways results in priapism. A pathophysiological approach to the treatment of priapism is proposed with duration-based algorithms presented to assist in management.

Unintended targeting of Dmp1-Cre reveals a critical role for Bmpr1a signaling in the gastrointestinal mesenchyme of adult mice

Cre/loxP technology has been widely used to study cell type-specific functions of genes. Proper interpretation of such data critically depends on a clear understanding of the tissue specificity of Cre expression. The Dmpl- Cre mouse, expressing Cre from a 14-kb DNA fragment of the mouse Dmpl gene, has become a common tool for studying gene function in osteocytes, but the presumed cell specificity is yet to be fully established. By using the Ai9 reporter line that expresses a red fluorescent protein upon Cre recombination, we find that in 2-month-old mice, Dmpl-Cre targets not only osteocytes within the bone matrix but also osteoblasts on the bone surface and preosteoblasts at the metaphyseal chondro-osseous junction. In the bone marrow, Cre activity is evident in certain stromal cells adjacent to the blood vessels, but not in adipocytes. Outside the skeleton, Dmpl-Cre marks not only the skeletal muscle fibers, certain cells in the cerebellum and the hindbrain but also gastric and intestinal mesenchymal cells that express Pdgfra. Confirming the utility of Dmpl-Cre in the gastrointestinal mesenchyme, deletion of Bmprla with Dmpl-Cre causes numerous large polyps along the gastrointestinal tract, consistent with prior work involving inhibition of BMP signaling. Thus, caution needs to be exercised when using Dmpl-Cre because it targets not only the osteoblast lineage at an earlier stage than previously appreciated, but also a number of non-skeletal cell types.

Single-atom spintronics

Recent work on magnetic quantum point contacts (QPCs) was discussed. Complete magnetoresistance loops across Co QPCs as small as a single atom was measured. The remarkable feature of these QPCs is the rapid oscillatory decay in magnetoresistance with the increase of contact size. In addition, stepwise or quantum magnetoresistance loops are observed, resulting from varying transmission probability of the available discrete conductance channels because the sample is cycled between the ferromagnetic (F) and antiferromagnetic (AF) aligned states. Quantized conductance combined with spin dependent transmission of electron waves gives rise to a multi-channel system with a quantum domain wall acting as a valve, i.e., a quantum spin-valve. Behavior of a few-atom QPC is built on the behavior of a single-atom QPC and hence the summarization of results as ‘single-atom spintronics’. An evolutionary trace of spin-dependent electron transmission from a single atom to bulk is provided, the requisite hallmarks of artefact-free magnetoresistance is established across a QPC – stepwise or quantum magnetoresistance loops and size dependent oscillatory magnetoresistance.

Gastrointestinal manifestations of systemic mastocytosis

Systemic mastocytosis (SM) is a rare disease with abnormal proliferation and infiltration of mast cells in the skin, bone marrow, and viscera including the mucosal surfaces of the digestive tract. Gastrointestinal (GI) symptoms occur in 14%-85% of patients with systemic mastocytosis. The GI symptoms may be as frequent as the better known pruritis, urticaria pigmentosa, and flushing. In fact most recent studies show that the GI symptoms are especially important clinically due to the severity and chronicity of the effects that they produce. GI symptoms may include abdominal pain, diarrhea, nausea, vomiting, and bloating. A case of predominantly GI systemic mastocytosis with unique endoscopic images and pathologic confirmation is herein presented, as well as a current review of the GI manifestations of this disease including endoscopic appearances. Issues such as treatment and prognosis will not be discussed for the purposes of this paper.
