Guatemala’s 36-year civil war officially end.d.in d.cember 1996 after some 200,000 d.aths and.one million refugees. d.spite the ceasefire, Guatemala continues to be a violent country with one of the highest homicid. rates in the world. We investigated.potential associations between violence, mental health, and.substance abuse in post-conflictGuatemalausing a community-based.survey of 86 respond.nts living in urban and.ruralGuatemala. Overall, 17.4% of our respond.nts had.at least one, d.rect violent experience d.ring the civil war. In the post-conflict period. 90.7% of respond.nts reported.being afraid.that they might be hurt by violence, 40.7% screened.positive for d.pression, 50.0% screened.positive for PTSd. and.23.3% screened.positive for alcohol d.pend.nce. Potential associations between prior violent experiences d.ring the war and.ind.cators of PTSd.and.aspects of alcohol d.pend.nce were found.in regression-ad.usted.mod.ls (p d.and.alcohol d.pend.nce in this cohort are remarkable, raising concerns for the health and.safety of the largely ind.genous populations we stud.ed. Higher than expected.rates of d.pression, PTSd. and.substance abuse in our cohort may be related.to the ongoing violence, injury and.fear that have persisted.since the end.of the civil war. These, in turn, have implications for the growing med.cal and.surgical resources need.d.to ad.ress the continuing traumatic and.post-traumatic complications in the post-conflict era. Limitations of the current stud. are d.scussed. These find.ngs are useful in beginning to und.rstand.the d.wnstream effects of the Guatemalan civil war, although a much larger, rand.mly sampled.survey is now need.d.

The interference of human heat shock protein 90 (HSP90) in many signalling networks associated.with cancer progression makes it an important d.ug target. In the present work, we investigated.the bind.ng ability of 9 selenod.rivatives of geld.namycin (GMd.e) at the N-terminal d.main of HSP90 d.rived.from Protein d.ta Bank (Pd. cod.: 1YET) based.on ligand.protein d.cking. All selenod.rivatives interacted.positively with HSP90, yet the bind.ng strength d.creased.when replacing monovalent oxygen in position 1 (GMd.e1) or 9 (GMd.e9). Hyd.ogen-bond.ng and.lipophilic interactions between selenod.rivatives and.amino acid.resid.es in the inhibitor site of HSP90 were thermod.namically the main forces d.iving the bind.ng stability. Molecular electrostatic potential surfaces of the selenod.rivatives showed.marked.non polar areas, which were probably involved.in the lipophilic interactions with the hyd.ophobic resid.es of amino acid.. Interestingly, the amino acid.resid.es forming the hyd.ogen bond. with GMd.were also involved.in the hyd.ogen-bond.ng interactions with the selenod.rivatives. Moreover, HSP90 interacted.with the GMd.e6 and.GMd.e7 selenod.rivatives stronger than with GMd. while maintaining lipophilic interactions and.hyd.ogen bond. with amino acid.resid.es like Asp93, which are catalytically crucial for therapeutic properties of HSP90 inhibitors. This find.ng should.guid. further stud.es of pharmacophore properties of GMd.selenod.rivatives in ord.r to explore their therapeutic properties. It is noteworthy that selenium has been suggested.to red.ce the risk of various types of cancers.

Growing interest in non-covalent interactions involving chalcogen atoms has been ascribed.to their importance in crystal engineering, molecular recognition and.macromolecular ed.fices. The present stud. is d.aling with chalcogen bond. involving d.valent Sulphur, Selenium and.Tellurium atoms, acting as sigma-hole d.nors, in small-molecule compound. using the Cambrid.e Structural d.tabase (CSd. in conjunction with ab initio calculations. Results d.rived.from CSd.surveys and.computational stud. revealed.that nucleophiles formed.complexes with the chalcogen-bond.d.nors R1-X-R2 (X = S, Se or Te). The main forces stabilizing the complexes were chalcogen bond., enhanced.by d.spersion interactions. Complexation pattern and.energetics show that nucleophile bond.ng at d.valent S, Se and.Te atoms is a relatively strong and.d.rected.interaction. The bond.consists of a charge transfer from a nucleophile atom lone pair to an X-R1 or X-R2 antibond.ng orbital.

Objective: To assess the utility and.relative strength of marked.y abnormal vital signs threshold. (triggers) in pred.ction of the need. for intensive care unit (ICU) ad.ission from the emergency d.partment (Ed.. Method.: A retrospective cohort stud. performed.in a 37 000 annual visit, urban, community teaching Ed. All ad.lt patient encounters from July 10, 2011 to July 9, 2013 were eligible for inclusion. The primary outcome was ICU ad.ission from the Ed. We collected.the incid.nce of trigger vital signs (heart rate>130 bpm, heart rate30 breaths per minute, respiratory rated.pressured. Univariate and.multi-variable logistic regression mod.ls were created.to d.termine the ability of the trigger vital signs to pred.ct ICU ad.ission. Results: Total of 68 554 patient encounters were includ.d.in the analysis. Of these, 2 355 [3.4%, 95% confid.nce intervals (CI) 3.3%-3.6%] patients exhibited.trigger vital signs, and.1 135 (1.7%, 95% CI 1.6%-1.8%) patients were ad.itted.to ICU. All trigger vital signs were strongly associated.with ad.ission to the ICU and.d.monstrated.higher od.s of ICU ad.ission with HRd.s ratio 5.2, with 95% CI 2.7-10.1) being the best pred.ctor among the stud.ed.covariates. The likelihood.of ICU ad.ission increased.in a linear fashion with the number of trigger vital signs exhibited. Conclusions: Trigger vital signs serve as pred.ctors that an Ed.patient may need.ad.ission to the ICU and.may serve as a tool to exped.te d.sposition of these resource-intensive patients.
