胃成形术治疗肥胖者的非酒精性脂肪性肝病

Nonalcoholic steatohepatitis (NASH), which is the most severe histologic form of nonalcoholic fatty liver disease (NAFLD), is emerging as the most common clinically important form of liver disease in obese patients. The prevalence of NASH may increase with the rise in the rate of obesity and metabolic syndrome in affluent communities. The aim of this work.is to describe clinical and histopathologic findings and correlate liver tissue damage to the length of duration of the obesity in the group of patients who underwent surgery as obesity treatment. Eighty-seven severely or morbidly obese patients underwent gastroplasty. Each patient was evaluated with complete clinical and laboratory medical assessment together with wedge liver biopsy tak.n from 59 unselected patients during the surgery. Patients were followed up for 41 months. Repeat liver biopsy was tak.n from 10 patients. Pathologic analysis recorded the presence and degree of steatosis, portal and lobular inflammation and fibrosis. Age, body mass index (BMI), and laboratory assessment correlated with pathologic data. Male patients showed more pronounced metabolic syndrome and fatty liver damage. Patients who become obese in childhood or as teenagers showed no differences in metabolic syndrome and NAFLD in mature age. There was statistically significant association between BMA, elevated transaminases, NAFLD, and fibrosis. Significant weight reduction was observed within first year after surgery,was slower in the second year, and stabilized within third year. Remark.ble improvement followed in biological mark.rs of metabolic syndrome. Ninety-six percent of initial liver biopsies had steatosis; 16%developed steatohepatitis and mild perivenular fibrosis. Significant improvement of the degenerative and inflammatory hepatic lesions in repeated biopsies and liver function readings was noted within 8 months after surgery. In the realm of data privacy protection, federated learning aims to collaboratively train a global model. However, heterogeneous data between clients presents challenges, often resulting in slow convergence and inadequate accuracy of the *** shared feature representations alongside customized classifiers for individual clients emerges as a promising personalized solution. Nonetheless, previous research has frequently neglected the integration of global k.owledge into local representation learning and the synergy between global and local classifiers, thereby limiting model performance. To tack.e these issues, this study proposes a hierarchical optimization method for federated learning with feature alignment and the fusion of classification decisions (FedFCD). FedFCD regularizes the relationship between global and local feature representations to achieve alignment and incorporates decision information from the global classifier, facilitating the late fusion of decision outputs from both global and local classifiers. Additionally, FedFCD employs a hierarchical optimization strategy to flexibly optimizemodel parameters. Through experiments on the Fashion-MNIST, CIFAR-10 and CIFAR-100 datasets, we demonstrate the effectiveness and superiority of FedFCD. For instance, on the CIFAR-100 dataset, FedFCD exhibited a significant improvement in average test accuracy by 6.83% compared to four outstanding personalized federated learning approaches. Furthermore, extended experiments confirmthe robustness of FedFCD across various hyperparameter values. © 2024 The Authors. Published by Tech Science Press.

This study explored a Bayesian belief network. (BBNs) approach, developing two distinct models for prioritizing the seven indicators related to the "rapid response to and mitigation of the spread of an epidemic" category within the context of both the specific category and the Global Health Security Index (GHS index). Utilizing data from the 2021 GHS index, the methodology involves rigorous preprocessing, the application of the augmented naive Bayes algorithm for structural learning, and k.fold cross-validation. k.y findings show unique perspectives in both BBN models. In the mutual value of information analysis, "link.ng public health and security authorities" emerged as the k.y predictor for the "rapid response to and mitigation of the spread of an epidemic" category, while "emergency preparedness and response planning" assumed precedence for the GHS index. Sensitivity analysis highlighted the critical role of "emergency preparedness and response planning" and "link.ng public health and security authorities" in extreme performance states, with "access to communications infrastructure" and "trade and travel restrictions" exhibiting varied significance. The BBN models exhibit high predictive accuracy, achieving 83.3% and 82.3% accuracy for extreme states in "rapid response to and mitigation of the spread of an epidemic" and the GHS index, respectively. This study contributes to the literature on GHS by modeling the dependencies among various indicators of the rapid response dimension of the GHS index and highlighting their relative importance based on the mutual value of information and sensitivity analyses.

This study presents a comparative analysis of two image enhancement techniques, Continuous Wavelet Transform (CWT) and Fast Fourier Transform (FFT), in the context of improving the clarity of high-quality 3D seismic data obtained from the Tano Basin in West Africa, Ghana. The research focuses on a comparative analysis of image clarity in seismic attribute analysis to facilitate the identification of reservoir features within the subsurface structures. The findings of the study indicate that CWT has a significant advantage over FFT in terms of image quality and identifying subsurface structures. The results demonstrate the superior performance of CWT in providing a better representation, mak.ng it more effective for seismic attribute analysis. The study highlights the importance of choosing the appropriate image enhancement technique based on the specific application needs and the broader context of the study. While CWT provides high-quality images and superior performance in identifying subsurface structures, the selection between these methods should be made judiciously, tak.ng into account the objectives of the study and the characteristics of the signals being analyzed. The research provides valuable insights into the decision-mak.ng process for selecting image enhancement techniques in seismic data analysis, helping researchers and practitioners mak. informed choices that cater to the unique requirements of their studies. Ultimately, this study contributes to the advancement of the field of subsurface imaging and geological feature identification.

User identity link.ge (UIL) refers to identifying user accounts belonging to the same identity across different social media platforms. Most of the current research is based on text analysis, which fails to fully explore the rich image resources generated by users, and the existing attempts touch on the multimodal domain, but still face the challenge of semantic differences between text and images. Given this, we investigate the UIL task.across different social media platforms based on multimodal user-generated contents (UGCs). We innovatively introduce the efficient user identity link.ge via aligned multi-modal features and temporal correlation (EUIL) approach. The method first generates captions for user-posted images with the BLIP model, alleviating the problem of missing textual information. Subsequently, we extract aligned text and image features with the CLIP model, which closely aligns the two modalities and significantly reduces the semantic gap. Accordingly, we construct a set of adapter modules to integrate the multimodal features. Furthermore, we design a temporal weight assignment mechanism to incorporate the temporal dimension of user behavior. We evaluate the proposed scheme on the real-world social dataset TWIN, and the results show that our method reaches 86.39% accuracy, which demonstrates the excellence in handling multimodal data, and provides strong algorithmic support for UIL. © 2024 The Authors. This study examines the turbidite dynamics and hydrocarbon reservoir formation in Ghana’s Tano Basin, which is located in coastal West Africa. Through an exploration of geological processes spanning millions of years, we uncover k.y factors shaping hydrocarbon accumulation, including source rock.richness, temperature, pressure, and geological structures. The research offers valuable insights applicable to exploration, management, and sustainable resource exploitation in coastal West Africa. It facilitates the identification of exploration targets with higher hydrocarbon potential, enables the anticipation of reservoir potential within the Tano Basin, and assists in tailoring exploration and management strategies to specific geological conditions of the Tano Basin. Analysis of fluvial channels sheds light on their impact on landscape formation and hydrocarbon exploration. The investigation into turbidite systems unveils intricate interactions involving tectonics, sea-level fluctuations, and sedimentation patterns, influencing the development of reservoirs. An understanding of sediment transport and depositional settings is essential for efficient reservoir management. Geomorphological features, such as channels, submarine canyons, and distinct channel types, are essential in this situation. A detailed examination of turbidite channel structures, encompassing canyons, channel complexes, convex channels, and U-shaped channels, provides valuable insights and aids in identifying exploration targets lik. basal lag, channel levees, and lobes. These findings underscore the enduring significance of turbidite systems as conduits for sediment transport, contributing to enhanced reservoir management and efficient hydrocarbon production.
