The paper tackles the problem of robust fault detection using Takagi-Sugeno fuzzy models. A model-based strategy is employed to generate residuals in order to make a decision about the state of the process. Unfortunately, such a method is corrupted by model uncertainty due to the fact that in real applications there exists a model-reality mismatch. In order to ensure reliable fault detection the adaptive threshold technique is used to deal with the mentioned problem. The paper focuses also on fuzzy model design procedure. The bounded-error approach is applied to generating the rules for the model using available measurements. The proposed approach is applied to fault detection in the DC laboratory engine.

Carbon materials are widely used for supercapacitor applications thanks to their high surface area, good rate capability, and excellent cycling stability. However, the development of high energy density carbon supercapacitors still remains a challenge. In this work, hollow Co3O4 nanoboxes have been embedded into three-dimensional macroporous laser-scribed graphene （LSG） to produce composite electrodes with improved electrochemical performance. Here, Co3O4 provides high capacity through fast and reversible redox reactions, while LSG serves as a conductive network to maintain high power. The open nanobox morphology is a unique solution for extracting the maximum capacity from Co3O4, resulting in electrodes whose surfaces, both internal and external, are accessible to the electrolyte. The electrochemical performance of the composite material is promising with a volumetric capacity of 60.0 C/cm^3 and a specific capacity of 542.3 C/g, corresponding to 682.0 C/g of the constituent Co3O4. With a low equivalent series resistance of 0.9 Ω, the Co3O4/LSG electrode is able to maintain 113.1% of its original capacity after 10,000 cycles. This work provides new insights into the design of high-performance carbon/metal oxide nano- composites for next-generation energy storage devices.

Antithrombin(AT) is a natural anticoagulant with antiinflammatory properties that has demonstrated value in sepsis, disseminated intravascular coagulation and in burn and inhalation injury. With high doses, AT maydecrease blood loss during eschar excision, reducing blood transfusion requirements. There are no human randomized, placebo-controlled studies, which have tested the true benefit of this agent in these conditions. Two main forms of AT are either plasma-derived AT(ph AT) and recombinant AT(rh AT). Major ovine studies in burn and smoke inhalation injury have utilized rh AT. There have been no studies which have either translated the basic rh AT research in burn trauma, or determined the tolerance and pharmacokinetics of rh AT concentrate infusions in burn patients. Advantages of rh AT infusions are no risk of blood borne diseases and lower cost. However, the majority of human burn patient studies have been conducted utilizing ph AT. Recent Japanese clinical trials have started using ph AT in abdominal sepsis successfully. This review examines the properties of both ph AT and rh AT, and analyzes studies in which they have been utilized. We believe that it is time to embark on a randomized placebo-controlled multi-center trial to establish the role of AT in both civilian and military patients with burn trauma.

Necrotizing Soft Tissue Infections of the Male and Female Breast: A Literature Review

Background: Within the spectrum of necrotizing soft tissue infections (NSTI), necrotizing fasciitis (NF) has been characterized by tissue and fascial necrosis with systemic toxicity. The objective was to determine and summarize the frequency and characteristics of breast NF and NSTI in the literature. Methods: Cases were obtained through PubMed, Google Scholar, Google, and from published article reference sections. One hundred twenty-three cases were gleaned from 96 articles that reported NF and NSTI of the breast (1924 through 2021). Results: NF was reported in 70 and NSTI in 53 cases (111 women and 12 men). Patients presented with swollen, painful breasts, erythema, bullae, crepitus, necrosis, gangrene, fever, tachycardia, and neutrophilia. Fifty-nine of 123 (48.4%) patients were septic on admission. The most frequent microorganisms were β hemolytic Group A Streptococcus, and Staphylococcus aureus. Treatment consisted of antibiotics, mastectomy and debridement with flaps, skin grafts or primary and secondary closure. Forty-four (63.0%) of the NF cases had chest wall involvement;of these, 18 (14.6%) involved the breast secondarily, P P = 0.007. Conclusions: Men and women with breast NF and NSTI presented with similar signs and symptoms and required the same emergent treatment as provided for NF and NSTI of the more common sites. As a time-sensitive disease, patients treated within 12 hours of admission had a better survival. Patients with NF were more likely to have sepsis on admission, a higher mortality, and fascial chest wall/muscle involvement than patients with NSTI.

Background: Myanmar’s National Health Plan 2017-2021 set out concrete steps to strengthen the health system on the way to achieving the goal of universal health coverage by 2030. Ensuring these steps include the 7.7 million older adults in Myanmar’s population by 2030 will require novel strategies that improve health and reduce financial burden of health expenditures. An examination of the relationship between social networks and selected health outcomes was undertaken to determine whether these networks can safeguard older adults’ continued contributions to family, society and their own health. Methods: Secondary analysis of a nationally representative study of adults aged 60 years and older. Ordered logistic regression analyses with weighted data were used to examine the relationship between a social network variable and a number of health outcomes. Findings: Stronger social networks were associated with better health outcomes like better memory (OR 1.2), lower reported walking difficulties (OR 0.58), better self-reported health (R 1.15). People with higher social ties were also more like to receive regular assistance and provide care for other household members. Interpretation: Social networks play a role in mediating health outcomes and interactions with the health care systems. Social network interventions should be explored as part of policy mechanisms to ensure universal health coverage for older adults in Myanmar.

Performance of the health system in China and Asia as measured by responsiveness

Objectives: To examine differences in health system responsiveness across different sectors in China and to compare to other Asian countries. Methods: The World Health Survey was implemented in a nationally representative sample in China and 10 additional Asian countries from 2002-2003. Face-to-face interviews were conducted to gather health care utilization and health systems responsiveness data. Results: Overall health system responsiveness in China was better for the inpatient than the outpatient health system. Differences were seen by domain, with prompt attention and respectful treatment performing better than the other domains. Differences in responsiveness were seen by socio- demographic characteristics, with women and younger respondents rating inpatient systems, whereas men and higher educated respondents rated outpatient systems, more responsive. Conclusions: As populations age, health care systems will come under more pressures – responsiveness can be used by governments to guide policy and system improvement efforts when resources are limited. In China, reforms might prioritize outpatient system responsiveness.
