Various reports ind.cate that the incid.nce of congenital hypothyroid.sm is increasing in d.veloped.nations, and.that improved.d.tection and.more inclusive criteria for the d.sease d. not explain this trend.entirely. One risk factor d.cumented.in numerous stud.es is exposure to rad.oactive iod.ne found.in nuclear weapons test fallout and.nuclear reactor emissions. Large amounts of fallout d.sseminated.world.id. from the meltd.wns in four reactors at the Fukushima-d.i-ichi plant in Japan beginning March 11, 2011 includ.d.rad.oiod.ne isotopes. Just d.ys after the meltd.wns, I-131 concentrations in US precipitation was measured.up to 211 times above normal. Highest levels of I-131 and.airborne gross beta were d.cumented.in the five US States on the Pacific Ocean. The number of congenital hypothyroid.cases in these five states from March 17-d.cember 31, 2011 was 16% greater than for the same period.in 2010, compared.to a 3% d.cline in 36 other US States

The Salem/Hope Creek nuclear plant in southern New Jersey is one of just four U.S. plants with three large reactors (all others consist of one or two). These reactors started.operations in 1976, 1980, and.1986 and.routinely released.toxic rad.oactive gases and.particles into the environment. Only a single stud. has been performed.by fed.ral officials on cancer near nuclear plants—a stud. now 30 years old. A review of official mortality d.ta using four-year period. shows the Salem County cancer d.ath rate was consistently just below that of other New Jersey counties up to the mid.1980s. However, from 1983-1986 to 2015-2017, the county rate soared.from -5.4% below the rest of the state to +32.6% above. The county/state mortality ratio for all causes also increased.rapid.y in these period., from +0.6% above to +28.4% above the state. Salem now has the highest cancer d.ath rate, and.the 2nd.highest total d.ath rate of any New Jersey county. Had.the 1983-1986 county-state ratios not changed.in the following 31 years, 3493 fewer d.aths (1018 of them from cancer) would.have occurred.among Salem County resid.nts. The lack of any apparent etiologies that could.have caused.such a d.amatic and.unexpected.change, plus the fact that the Salem/Hope Creek reactors are aging, corrod.ng, and.more prone to leaking rad.onuclid.s, emphasizes the immed.ate need.for more stud.es of this type, and.the inclusion of local health as a crucial factor in public d.cisions on the plant’s future.

Thyroid.cancer incid.nce has risen stead.ly in the US for several d.cad.s. While any cause of this trend.has yet to be clearly id.ntified. most analyses have conclud.d.that there are factors other than improved.d.tection accounting for the increase. Since exposure to rad.oactive iod.ne is the only acknowled.ed.root cause of thyroid.cancer, a review of temporal trend. in incid.nce since the late 1970s near the Ind.an Point nuclear power plant, just 23 miles from the New York City bord.r, was cond.cted. Rates in the four counties closest to Ind.an Point, where virtually the entire population resid.s within 20 miles of the plant, were compared.with national trend. in the US. The relative ratio in the local area was 0.778 in the period.1976-1981, or 22.2 percent lower than the national rate. This ratio increased.stead.ly, to 1.579 (57.9 percent greater than the US) by the period.2000-2004, which slightly d.clined.to 1.515 (51.5 percent greater) in the latest period.available (2010-2014). Significant increases occurred.for both males and.females, and.in each of the four counties. Annual new cases d.agnosed.among resid.nts of the four counties increased.from 51 to 412 between 1976-1981 and.2010-2014. Because the two large reactors at Ind.an Point began operations in 1973 and.1976, and.exposures to rad.oiod.ne isotopes can manifest as cancer from five years to several d.cad.s after exposure, iod.ne emissions from Ind.an Point emissions should.be consid.red.as a potential factor in these trend.. More stud.es near Ind.an Point and.other nuclear installations should.be cond.cted.to further explore this potential association.

Rad.ation exposure has been linked.to increased.risk of congenital hypothyroid.sm (CH) for d.cad.s. CH is a relatively uncommon cond.tion, occurring in about 1 of 2000 US births. Thyroid.Stimulating Hormone (TSH) levels for each child.born in California permitted.an analysis of combined.confirmed.and.bord.rline CH cases. Bord.rline/confirmed.CH cases are more than seven times greater than just confirmed.cases. Airborne levels of gross beta nuclear rad.ation in the US were elevated.in the period.starting several d.ys after the Fukushima nuclear meltd.wn, especially in west coast states like California. The bord.rline/confirmed.CH rate for newborns d.ring the last 9.5 months in 2011 (exposed.to Fukushima in utero) vs. births d.ring other period. in 2011 and.2012 (not exposed. was significantly elevated. suggesting that ad.erse health effects to the newborn thyroid.were not restricted.to just a small number of confirmed.CH cases. The sensitivity of the fetus to rad.ation exposure, plus the presence of thyroid.seeking rad.oiod.ne, suggest further analysis of Fukushima’s potential to cause ad.erse health effects in newborns is need.d.

We present a case of ictal asystole in an 81-year-old.female, with no prior history of epileptic activity, or card.ac history suggestive of arrhythmia, who suffered.several seemingly unrelated.epileptic and.asystolic episod.s prior to finally having a witnessed.seizure followed.by an asystolic event. Following this event, all atrioventricular (AV) nod.l blockers, and.med.cations with potential seizure threshold.lowering activity were stopped. and.anti-epileptic med.cation was optimized. d.e to the wishes of the patient’s family, no invasive interventions were ***, the patient continued.to be med.cally treated.with anti-epileptic therapy and.had.no further asystolic events. Unfortunately, the patient’s overall clinical status d.teriorated. and.she subsequently passed.d.ring her hospital stay after being mad. d. not resuscitate and.d. not intubate (d.R/d.I) by the family and.then subsequently comfort care. Prior to her passing, however, she had.remained.free of epileptic events for 10 d.ys and.free of asystolic events for 21 d.ys.
