Can I-124 PET/CT Predict Pathological Uptake of Therapeutic Dosages of Radioiodine (I-131) in Differentiated Thyroid Carcinoma?

Introduction: The aim of the study was to assess the role of I-124 PET/CT in the prediction of uptake of a therapeutic dosage I-131 during follow-up of patients with differentiated thyroid cancer (DTC). Methods: The results of 34 I-124 PET/CT scans performed in our hospital between 2007 and 2012 were retrospectively evaluated. All scans were made in patients under follow up, replacing the diagnostic I-131 or I-123 scintigraphy. In all cases thyroglobulin (Tg) was stimulated (by recombinant thyroid stimulating hormone (rhTSH) or thyroid hormone withdrawal). A dosage of 40 MBq I-124 was used, with scans at 24 hours and 96 hours after administration. Results were compared to subsequent I-131 post-treatment scans (6 cases) and a combination of follow up, stimulated Tg and other imaging tools results available to assess presence of recurrence. Results: Recurrence of DTC was found in 14/34 cases. I-124 PET/CT correctly detected recurrence in 2 cases, with false negative results in 12 cases. In 1 case a false positive I-124 PET/CT result was recorded. 19 true negative results were found. For I-124 PET/CT this meant a sensitivity of 14% and a specificity of 95%. Positive predictive value was 67%, negative predictive value 61%. Post-treatment I-131 uptake (6 cases) was correctly predicted in 1 case, with false negative results in 4 cases and 1 true negative result. Conclusions: In this study I-124 PET/CT did not reliably detect recurrent differentiated thyroid carcinoma. More importantly it failed to predict I-131 uptake on post-treatment scintigraphy in a significant number of cases, which would lead to under-treatment.

Objectives: In grade III-IV breast cancer, dissemination of disease needs to be assessed. Until now this was done by conventional imaging (liver ultrasonography, chest X-ray and bone scintigraphy), but evidence favoring the use of FDG-PET/CT is accumulating. Methods: Patients with high-risk breast cancer, who had received conventional imaging and FDG-PET/CT, were included. Patients were staged and assigned a treatment after 1) conventional imaging and 2) FDG-PET/CT, both by a multidisciplinary oncology team. Equivocal FDG-PET/CT findings were histologically confirmed. Results: 16 patients were included (mean age 59 years). TNM-stage changed in 5 patients (31%) after FDG-PET/CT. In 3 patients (19%) unknown distant metastases were detected by FDG-PET/CT. An adjustment of treatment took place in 4 patients (25%). Conclusions: Our case series emphasizes the role of FDG-PET/CT in the staging of high-risk breast carcinoma, especially in the assessment of distant metastases. We suggest replacing conventional imaging with FDG-PET/CT.

Competing Patterns in Bernoulli Sequence of Trials

Consider performing a sequence of Bernoulli trials (each resulting in either a success, denoted S, or a failure F, with a probability of p and q := 1 - p respectively) until one of m specific strings (or patterns) of consecutive outcomes is generated. This can be seen as a game where m players select one such pattern each and the one whose pattern occurs first wins. We present symbolic formulas for the m probabilities of winning, and for the mean number of trials and the corresponding standard deviation to complete this game. Several numerical examples are presented, including a search for optimal strategy.

Controlling the decision to fight or flee： the roles of biogenic amines and nitric oxide in the cricket

Aggression is a common behavioral strategy employed by animals to secure limited resources, but must be applied with restraint to limit potential costs including injury. How animals make the adaptive decision to fight or flee is barely known. Here, we review our work on crickets that reveals the roles of biogenic amines, primarily octopamine (the insect analog of noradrenaline) and nitric oxide (NO). Using aminergic drugs, we found that amines are not essential for actually initiating aggression. However, octopamine is necessary for mediating the aggression-promoting effects of potentially rewarding experiences including stimulation with a male antenna, physical exertion, winning, and resource possession. Hence, octopamine can be considered as the motivational component of aggression. Imposed handicaps that impede aggressive signaling revealed that the agonistic actions of an opponent perceived during fighting act to reduce aggression, and that crickets make the decision to flee the moment the accumulated sum of such aversive experiences exceeds some critical level. Treatment with nitridergic drugs revealed that the impact of the opponent's aggressive actions is mediated by NO. NO acts to suppress aggression by promoting the tendency to flee and is primarily responsible for the depressed aggressiveness of subordinates after social defeat. Octopamine and dopamine can each restore aggression in subordinates, but only dopamine is necessary for normal recovery. The role of serotonin remains unclear, and is discussed. We conclude that octopamine and NO control the decision to fight or flee by mediating the effects of potentially rewarding and aversive experiences, respectively.

Analysis on variety and characteristics of maghemite

Maghemite (γ-Fe2O3) is a very common mineral at the earth’s surface and also an important material for making music and video tapes. Maghemite is usually synthesized from magnetite under oxidizing conditions after a few hours or a few days below a temperature of 300°C. The magnetic property of thermal instability and the chemical action after heating is an important character for maghemite. That is, it will become hematite in certain proportion after being heated above 250°C. Maghemite is therefore actually unable to have its Curie temperature measured. But late using synthetic sample, maghemite was further found partially thermal stable with a measurable Curie temperature ~645°C. During our thermally magnetic experiments for a set of synthetic magnetite, we found that extra fined grain size (pseudo single domain (PSD) and small multi-domain (MD), mainly 1-10 μm) magnetite was formed to a completely thermally stable maghemite. This maghemite can also be produced by heating the same powder up to 700°C in an oven and keeping this temperature for 10 min, then cooling it down. When the generated maghemite by these two ways is heated from room temperature to 700°C, it shows almost fully reversible, or thermally stable. We used X-ray powder diffraction and Mssbauer spectroscopy to confirm the identity of this maghemite and compared its magnetic hysteresis, high temperature magnetization, low temperature thermal demagnetization, and low temperature susceptibility with those of the original preheated magnetite. Such quickly oxidized maghemite by heating to high temperature implies some types of maghemite formed in certain natural condition can carry a thermal remnant magnetization (TRM). Four types of maghemite were characterized and discussed according to their thermal stability. Aluminum metal matrix composites （Al-MMCs） are difficult to machine. The reinforcement of aluminum using ceramic particles accelerates tool wear. Moreover, demanded machining accuracies or properties of the surface layer are difficult to achieve. In the present study, the effect of silicon carbide reinforcement particles on the surface layer of the workpiece was investigated using multiple cutting conditions for dry turning. Three differently reinforced Al-MMCs regarding the volume percent-age （17% and 30%） and the particle size （0.6 μm and 3 μm） and their non-reinforced matrix were considered as the workpiece materials. The reinforcement and the cutting condition affect the results of turning. A greater particle volume percent improves the surface roughness and decreases the tensile stress in the surface. The smaller particle size caused a lower tensile stress in the surface. A general effect of the particle size on the workpiece roughness can not be concluded. The most important cutting parameter for the surface layer of the workpiece is the feed. Greater feeds decrease the tensile stress in the surface, but deteriorate the surface quality.

Background The optimal management of ipsilateral extracranial internal carotid artery(ICA)stenosis during endovascular treatment(EVT)is *** compared the outcomes of two different strategies:EVT with vs without carotid artery stenting(CAS).Methods In this observational study,we included patients who had an acute ischaemic stroke undergoing EVT and a concomitant ipsilateral extracranial ICA stenosis of≥50%or occlusion of presumed atherosclerotic origin,from the Dutch Multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands(MR CLEAN)Registry(2014-2017).The primary endpoint was a good functional outcome at 90 days,defined as a modified Rankin Scale score≤*** endpoints were successful intracranial reperfusion,new clot in a different vascular territory,symptomatic intracranial haemorrhage,recurrent ischaemic stroke and any serious adverse *** Of the 433 included patients,169(39%)underwent EVT with *** 123/168(73%)patients,CAS was performed before intracranial *** 42/224(19%)patients who underwent EVT without CAS,a deferred carotid endarterectomy or CAS was *** with and without CAS were associated with similar proportions of good functional outcome(47%vs 42%,respectively;adjusted OR(aOR),0.90;95%CI,0.50 to 1.62).There were no major differences between the groups in any of the secondary endpoints,except for the increased odds of a new clot in a different vascular territory in the EVT with CAS group(aOR,2.96;95%CI,1.07 to 8.21).Conclusions Functional outcomes were comparable after EVT with and without *** during EVT might be a feasible option to treat the extracranial ICA stenosis but randomised studies are warranted to prove non-inferiority or superiority.
