Th.oretical analysis of th. dynamic beh.vior of cavitating vortices requires knowledge of th. velocity distribution in th. vortex core. For th.t reason an existing similarity model for slender axisymmetric non-cavitating vortex cores h.s been extended with.th. viscous boundary conditions for a cavitating vortex. Cavitating similarity solutions exist for a vortex of wh.ch.both.core diameter and circulation grow with.th. square root of th. axial coordinate. Results for th.s model are presented for varying cavitation numbers and Reynolds numbers. It is sh.wn th.t viscosity may h.ve a significant influence on th. velocity distribution around a cavitating vortex.

Ch.rité,Berlin,*** with.AIS with.ut known atrial fibrillation(AF)underwent cardiovascular MR imaging(CMR),MR-angiograph. of th. aortic arch.and prolonged h.lter-ECG monitoring on top of routine diagnostic *** Among 356 patients with.AIS(mean age 66 years,37.6%female),enh.nced workup yielded a h.gh.r rate of prespecified path.logical findings compared with.routine care(17.7%vs 5.3%;ph.nced diagnostic workup(38.5%vs 45.5%,ph.cardiograph. in 228(64.0%)*** was successfully performed in 292(82.0%)patients and revealed more often a prespecified path.logical finding compared with.routine ech.cardiograph.(16.1%vs 5.3%).Furth.rmore,study-related ECG monitoring(median duration 162 h.urs(IQR 98–210))detected AF in 16(4.5%)patients,wh.le routine monitoring(median duration 51 h.urs(IQR 34–74))detected AF in seven(2.0%)*** Enh.nced diagnostic workup revealed a h.gh.r rate of prespecified path.logical findings in patients with.AIS compared with.routine diagnostic care and significantly reduced th. proportion of patients with.cryptogenic *** registration number NCT02142413.

Diagnostic yield of small bowel capsule endoscopy depends on th. small bowel transit time

AIM： To investigate wh.th.r th. small bowel transit time （SBTT） influences th. diagnostic yield of capsule endoscopy （CE）. METh.DS： Six h.ndred and ninety-one consecutive CE procedures collected in a database were analyzed. SBTT and CE findings were recorded. A running mean for th. SBl-I- was calculated and correlated to th. diagnostic yield with.a Spearman＇s correlation test. Subgroup anal- yses were performed for th. various indications for th. procedure. RESULTS： Th.re was a positive correlation between th. diagnostic yield and SBT1- （Spearman＇s rh. 0.58, P 〈 0.01）. Positive correlations between diagnostic yield and SB-FI-were found for th. indication obscure gastro- intestinal bleeding （r = 0.54, P 〈 0.01）, for polyposis and carcinoid combined （r = 0.56, P 〈 0.01） and for th. oth.r indications （r = 0.90, P 〈0.01）, but not for suspected Croh.＇s disease （r = -0.40） CONCLUSION： Th. diagnostic yield in small bowel capsule endoscopy is positively correlated with.th. small bowel transit time. Th.s is true for all indications except for suspected Croh.＇s disease.

Aim： To establish.an obj.ctive, easy-to-use and compreh.nsive meth.d to analyze corpus cavernosum electromyo- graph.c signals （CC-potentials）. Meth.ds： CC-potentials were recorded during flaccidity in 23 young h.alth. volunteers, with.surface electrodes placed on th. penile sh.ft bilaterally. Based on th. correlation function of Matlab software, an application program for th. analysis of CC-potentials was developed. Individual CC-potentials and th.ir autocorrelation function were evaluated, yielding parameters amplitude （A）, duration （D）, and dominant frequency （DF）. Th. crosscorrelation function of both.longitudinal and bilateral pairs of adj.cent electrodes was calculated to assess th. similar- ity and mutual delay of CC-potentials recorded simultaneously from different parts of th. CC. Th. parameters derived were squared maximum cross-correlation coefficient （Rmax） and delay （τ）. Based on th. absolute value of τ and th. corresponding inter-electrode distance, propagation velocity （PV） was calculated. Results. Th. values of th. parameters were determined automatically. No significant difference related to th. locations of th. electrodes for parameters A, D, and DF was detected. Th. cross-correlation sh.wed th.t both.longitudinal and bilateral CC-potential pairs h.d h.gh.y similar waveforms （th. absolute values of Rmax were 0.80 ± 0.05 and 0.87 ± 0.06, respectively）. PV of longitudinal pairs was estimated as 6.15 ± 3.98 cm/s. Conclusion： Th. application program for correlation analysis of CC-potentials is a compreh.nsive and versatile meth.d to analyze corpus cavernosum electromyograph.c recordings. Its obj.ctiveness makes multi-center application possible.

Inh.bitory effects of interferons on pancreatic stellate cell activation

AIM： To analyze and to compare th. effects of interferon （IFN）-α, IFN-β, and IFN-γ on pancreatic stellate cell （PSC） activation/n vitro and to elucidate th. molecular basis of IFN action. METh.DS： PSCs were isolated from rat＇s pancreatic tissue, cultured and stimulated with.recombinant rat IFNs. Cell proliferation and collagen synth.sis were assessed by measuring th. incorporation of 5-bromo-2＇ -deoxyuridine （BrdU） into DNA and [^3h.-proline into acetic acid-soluble proteins, respectively. Apoptotic ceils were determined by FACS analysis （sub-G1 peak meth.d）. Exh.bition of th. myofibroblastic PSC ph.notype was monitored by immunoblot analysis of （α-smooth.muscle actin （α-SMA） expression. To assess th. activation of signal transducer and activator of transcription （STAT）, Western blots using ph.sph.- STAT-specific antibodies were performed. In studies on STAT1 function, expression of th. protein was inh.bited by siRNA. RESULTS： IFN-β and IFN-γ, but not IFN-α significantly diminish.d PSC proliferation and collagen synth.sis. IFN-γ, was th. only IFN th.t clearly inh.bited α-SMA expression. Under th. experimental conditions used, no enh.nced rate of apoptotic cell death.was observed in response to any IFN treatment. IFN-β and IFN-γ, induced a strong increase of STAT1 and STAT3 tyrosine ph.sph.rylation, wh.le th. effect of IFN-α was much.weaker. Inh.bition of STAT1 expression with.siRNA was associated with.a significantly reduced growth.inh.bitory effect of IFN-γ. CONCLUSION： IFN-β and particularly IFN-γ, display inh.bitory effects on PSC activation in vitro and sh.uld be tested regarding th.ir in vitro efficiency. Growth.inh.bition by IFN-γ action requires STAT1.

Th. Upper Carboniferous-Lower Permian （Upper Pennsylvanian-Asselian） Tobra Formation is exposed in th. Salt and Trans Indus ranges of Pakistan. Th. formation exh.bits an alluvial plain （alluvial fan-piedmont alluvial plain） facies association in th. Salt Range and Kh.sor Range. In addition, a stream flow facies association is restricted to th. eastern Salt Range. Th. alluvial plain facies association is comprised of clast-supported massive conglomerate （Gmc）, diamictite （Dm） facies, and massive sandstone （Sm） iith.facies wh.reas th. stream flow-dominated alluvial plain facies association includes fine-grained sandstone and sUtstone （Fss）, fining upwards pebbly sandstone （Sf）, and massive mudstone （Fro） lith.facies. Th. lack of glacial signatures （particularly glacial grooves and striations） in th. deposits in th. Tobra Formation, wh.ch.are, in contrast, present in th.ir time-equivalent and palaeogeograph.cally nearby strata of th. Arabian peninsula, e.g. th. Al Kh.ata Formation of Oman and Unayzah.B member of th. Sandi Arabia, suggests a pro-to periglacial, i.e. glaciofluvial depositional setting for th. Tobra Formation. Th. sedimentology of th. Tobra Formation attests th.t th. Salt Range, Pakistan, occupied a palaeogeograph.c position j.st beyond th. maximum glacial extent during Upper Pennsylvanian-Asselian time.

Introduction of laparoscopic gastrectomy for gastric cancer in a Western tertiary referral centre:A prospective cost analysis during th. learning curve

To evaluate th. costs of th. introduction of a laparoscopic surgery program for gastric cancer in a Western community training h.spital and tertiary referral centre for gastric cancer surgery. METh.DSAll patients wh. underwent surgery for gastric cancer with.curative intent in 2013 and 2014 were prospectively included. Primary outcomes were costs regarding surgery and h.spital stay. RESULTSLaparoscopic gastrectomy was used in 52 patients [mean age 68 years (± 9, range 50 to 87)] and open gastrectomy was used in 25 patients [mean age 70 years (± 10, range 46 to 85)]. Mean costs (in euro’s) of surgical instrumentation were significantly h.gh.r for laparoscopic surgery: 2270 ± 670 vs 1181 ± 680 in th. open approach.(P h.atre use were h.gh.r in th. laparoscopic group: mean 3819 ± 865 vs 2545 ± 1268 in th. open surgery (P h.spitalization (i.e., costs of surgery and admission) were not different between laparoscopic and open surgery, 8187 ± 4864 and 7673 ± 8064 respectively (P = 0.729). Mean length.of h.spital stay was 9 ± 12 d in th. laparoscopic group vs 14 ± 14 d in th. open group (P = 0.044). CONCLUSIONTh. introduction of laparoscopic gastrectomy for gastric cancer coincided with.h.gh.r costs for th.atre use and surgical instrumentation compared to th. open tech.ique. Total costs were not significantly different due to sh.rter length.of stay and less intensive care unit (ICU) admissions and sh.rter ICU stay in th. laparoscopic group.
