Anaerobic Co-Digestion of Fish Processing Waste with Cow Manure and Waste of Market (Rests of Fruits and Vegetables): A Lab Scale Batch Test

The aim of this work was to use fish processing waste (FW) as main substrate for anaerobic digestion. To enhance the biogas production of FW, co-digestion was done with two other substrates: cow dung (CD) and waste of market (MW). Batch test was carried out in an 1 L glass digester in a temperature controlled chamber at 38°C. The following mixtures were carried out: FW with CD respectively at different ratios 100:0% (A), 80:20% (B) and 60:40% (C);FW with MW at the following ratio 80:20% (D);FW with CD and MW respectively at these ratios 80:10:10% (F) and 60:20:20% (G). The biogas produced was measured using a milligas counter® and the volume of gas was recorded. The gas composition was determined using gas chromatography. With a pH stable for raw substrates and mixtures, TS and VS (%TS) contents for FW were respectively 31.01% and 91.55%. Between 3 to 13 days of experimentation, the highest flow rate was observed. The percentage of methane was more important for mixtures B and D, 61% and 59% respectively. pH and VOA/TIC were stable at the end of the batch test for all mixtures, meaning that the organic matter was already well digested. The highest values of Volatile Solid Removal (VSR) were found for mixtures C, D, F and G. Therefore, the promising mixtures for next experimentations in large scale are B and D.

BACKGROUND Nonalcoholic fatty liver disease(NAFLD)describes the hepatic manifestations of metabolic syndrome,which is estimated to affect 25%of adults,and currently represents the second most common indication for liver transplant in the United States.Studies have shown that patients with NAFLD are at an increased risk for heart failure,arrhythmia,and coronary artery disease(CAD),which may impact outcomes of liver transplantation.However,it remains unclear whether the presence of cardiac disease affects survival prior to liver transplant.If so,this would represent an important opportunity to optimize cardiac status and improve outcomes before liver transplant.AIM To identify cardiac factors that impact survival to liver transplantation in patients with NAFLD and on the transplant waitlist.METHODS The aim of this study was to identify cardiac risk factors that limit survival to transplant in patients with NAFLD.We performed a retrospective analysis of patients with NAFLD listed for liver transplant at a tertiary academic medical center in the United States from January 2015 to January 2021,identified through United Network of Organ Sharing registry.Exclusion criteria included a concurrent etiology of liver disease and removal from the transplant list due to chemical dependency,lack of social support,improvement in liver disease,or being lost to followup.We manually reviewed patient charts including electrocardiogram,echocardiogram,and cardiac catheterization reports as well as physician notes to identify cardiac disease states(i.e.,heart failure,arrhythmia,valvular disease and CAD)and other related diagnoses.We performed a survival analysis by Cox proportional hazards regression model to analyze the association between cardiac factors at the time listed for transplant and death or clinical deterioration prior to transplant.RESULTS Between January 2015 and January 2021,265 patients with nonalcoholic fatty liver disease were listed for liver transplant at our institution.Our patient sample had a median age of 63 and

Polypore fungi of Caucasian alder as a source of antioxidant and antitumor agents

Macroscopic fungi on Caucasian alder wood(Alnus subcordata)were identified and tested as a source of betulin and betulinic acid(the most important metabolites of the Betulaceae family)to evaluate levels of phenols,flavonoids and antioxidant activity.Ganoderma applanatum,Lenzites betulina,Trichaptum biforme,Rigidoporus ulamrius,Fomes fomentarius,Schizophyllum commune,Auricularia mesenterica,and Trametes versicolor were among those identified,and they differed significantly in the level of betulin and betulinic acid and phenols,flavonoids and antioxidant properties in fungal tissues extracted with methanol and with ethanol(p≤0.01).G.applanatum had the most betulin(3.642%)and S.commune the most betulinic acid(1.413%).All tested fungi had high antioxidant activity,and L.betulina had the highest(97.775%).The highest amounts of phenol(719.993 mg mL-1)and flavonoids(361.403 mg mL-1)were found in the ethanolic extract from G.applanatum.Considering the results of this study and the low cost and convenient access to these fungi,they should be good sources for producing different drugs.

Review of recent guidelines for the management of severesepsis and septic shock

Severe sepsis and septic shock affect millions of patients and are major causes of mortality worldwide.Advancements in treatment and disease management led to a decline in in-hospital mortality from 27.8%(1979–1984)to 17.9%(1995 to 2000).In this article,we systemically review recent guidelines for the management of severe sepsis and septic shock published in 2008 by the International Surviving Sepsis Campaign Guidelines Committee.The 2008 Surviving Sepsis guidelines recom-mend protocolized resuscitation with goals to maintain central venous pressure≥8–12 mmHg,mean arterial pressure≥65 mmHg,urine output≥0.5 mL$kg–1$h–1 and central venous oxygen saturation≥70%(or mixed venous≥65%).Furtherﬂuid administration,transfusion of packed red blood cells to achieve a hematocrit of≥30%and/or infusion of dobutamine max 20µg$kg–1$min–1 are advised if venous O2 saturations remain below 70%.In patients with decreased ventricular compliance or mechanical ventilation,a target central venous pressure of 12–15 mmHg is recommended.Intravenous antibiotic administration within theﬁrst hour of recognizing severe sepsis and septic shock is essential,while use of corticosteroids in sepsis is controversial.The mechanisms by which activated protein C improves clinical outcomes in sepsis are unknown.Therapy with activated protein C is approved for patients with severe sepsis and an increased risk of death[Acute Physiology and Chronic Health Evaluation II(APACHE II)>25].Bicarbonate therapy is discouraged.Intravenous insulin should be used to control hyperglycemia in patients with severe sepsis following stabilization in the intensive care unit.

Lung cancer is one of the most common human cancers and the number one cancer killer in the United States.In general,lung cancer includes small cell lung cancer(SCLC)and non-small cell lung cancer(NSCLC),but NSCLC accounts for approximately 90%of lung cancer.The early diagnosis and therapy of lung cancer still presents a big challenge because validated screening tools,which can improve current early detection to reduce mortality from lung cancer,do not exist.Over the last decade,molecular genetic abnormalities have been described in NSCLC,including chromosomal aberrations,overexpression of oncogenes,and deletion and/or muta-tions in tumor suppressor genes.These molecular markers in NSCLC demonstrated close associations with the development of lung cancer such as Ras,the epidermal growth factor receptor(EGFR,or c-erbB-1),HER2(c-erbB-2),c-Met,and Bcl-2.Therefore,this information may be applied for early cancer detection,classiﬁcation,novel targeted therapy,and prognosis in NSCLC.Recent clinical data have revealed that targeted therapy might be the second-line therapy as an alternative approach.Currently,the targeted therapies are mainly focused on two lung cancer pathways,the EGFR and the vascular endothelial growth factor(VEGF)pathways.Some clinical trials are very encouraging,but some of them are not.However,these trials have not identiﬁed a subgroup of NSCLC with biomarkers.Therefore,it is very important to select NSCLC patients with biomarkers to match targeted agents so that we can further identify effectiveness of targeted therapy in the future.

BACKGROUND Given the high prevalence of cardiovascular and pulmonary abnormalities associated with sickle cell anemia(SCA),the clinical impact caused in addition to compromising the quality of life of patients and the overcharge that it represents to the public health system,this study systematized and evaluated scientific publications on pulmonary complications and cardiovascular diseases in sickle cell patients from 1920 to 2020.This compilation aims to provide knowledge for health professionals and managers in order to draw attention to the importance of chronic diseases in SCA patients and in addition to providing elements that provide improvements in management of useful resources that contribute to improve the quality and increase the life expectancy of these patients.AIM To systematically compile information about cardiopulmonary changes in patients with SCA.METHODS A systematic literature review was performed based on the PRISMA recommendation including scientific articles indexed in the Scientific Electronic Library Online databases of the United States National Library of Medicine and Biblioteca Virtual de Saúde.The search period was delimited between 1990 and 2020 and selected in Portuguese,English and Spanish.Three sets of descriptors were used for each database including research carried out with human beings.After reading the articles,those useful for this review were extracted using a collection instrument designed for this purpose.RESULTS The final selection included 27 studies.The year with the highest number of publications was 2016 with 5 studies(18.51%),followed by 2017 with 4(14.81%).The type of study most carried out in the period was cohort 10(37.03%)followed by cross-sectional and case-control with 8 studies in each(29.62%).Regarding the language of publication,the distribution was as follows:25(92.59%)in English,1(3.70%)in Spanish and 1(3.70%)in Portuguese.CONCLUSION The findings of the present study suggest that cardiopulmonary alterations represent a serious clinical repercussion of SCA

AIM:To determine the prevalence of a family history suggestive of Lynch syndrome (LS) among patients with colorectal cancer (CRC) followed in a coloproctology outpatient clinic in Southern Brazil.METHODS:A consecutive sample of patients with CRC were interviewed regarding personal and family histories of cancer.Clinical data and pathology features of the tumor were obtained from chart review.RESULTS:Of the 212 CRC patients recruited,61 (29%) reported a family history of CRC,45 (21.2%) were diagnosed under age 50 years and 11 (5.2%) had more than one primary CRC.Family histories consistent with Amsterdam and revised Bethesda criteria for LS were identified in 22 (10.4%) and 100 (47.2%) patients,respectively.Twenty percent of the colorectal tumors had features of the high microsatellite instability phenotype,which was associated with younger age at CRC diagnosis and with Bethesda criteria (P < 0.001).Only 5.3% of the patients above age 50 years had been previously submitted for CRC screening and only 4% of patients with suspected LS were referred for genetic risk assessment.CONCLUSION:A significant proportion of patients with CRC were at high risk for LS.Education and training of health care professionals are essential to ensure proper management.

Intermodal Transport of Ethanol in Brazil:A Simulation Using Partial Equilibrium

The search for renewable energy has put ethanol on another level,making Brazil the second largest producer in the world.The aim of this study was to analyze ethanol transportation alternatives in the face of the current logistics in Brazil.It was proposed a partial equilibrium model analyzing three scenarios:the first concerned a base scenario(scenario 1),the second estimated a 15%decrease in railroad freight(scenario 2),and the third included new national pipeline projects(scenario 3).Scenario 3 showed the highest trading volumes with a 0.20%increase compared with the base scenario,indicating transport systems that prioritize multimodality.It was also highlighted that the multimodality assumptions of scenarios 2 and 3 resulted in competitiveness gains in the international market,because all the routes destined for foreign markets are multimodal.
