Long-term outcomes of pediatric liver transplantation in acute liver failure vs end-stage chronic liver disease:A retrospective observational study

BACKGROUND Children with acute liver failure(ALF)who meet the criteria are eligible for super-urgent transplantation,whereas children with end-stage chronic liver disease(ESCLD)are usually transplanted *** liver transplantation(PLT)in ALF and ESCLD settings has been well described in the literature,but there are no studies comparing the outcomes in these two *** To determine if there is a difference in post-operative complications and survival outcomes between ALF and ESCLD in *** This was a retrospective observational study of all primary PLTs performed at a single center between 2000 and *** and ESCLD groups were compared for pretransplant recipient,donor and operative parameters,and post-operative outcomes including graft and patient *** Over a 20-year study period,232 primary PLTs were performed at our center;195 were transplanted for ESCLD and 37 were transplanted for *** ALF recipients were significantly older(median 8 years vs 5.4 years;P=0.031)and heavier(31 kg vs 21 kg;P=0.011).Living donor grafts were used more in the ESCLD group(34 vs 0;P=0.006).There was no difference between the two groups concerning vascular complications and rejection,but there were more bile leaks in the ESCLD ***-transplant patient survival was significantly higher in the ESCLD group:1-,5-,and 10-year survival rates were 97.9%,93.9%,and 89.4%,respectively,compared to 78.3%,78.3%,and 78.3%in the ALF group(P=0.007).However,there was no difference in 1-,5-,and 10-year graft survival between the ESCLD and ALF groups(90.7%,82.9%,77.3%vs 75.6%,72.4%,and 66.9%;P=0.119).CONCLUSION Patient survival is inferior in ALF compared to ESCLD recipients;the main reason is death in the 1st year post-PLT in ALF *** the ALF children overcome the 1st year after transplant,their survival stabilizes,and they have good long-term outcomes.

Saffron(Crocus sativus L.) stigmas as a potential natural additive to improve oxidative stability attributes of sunflower(Helianthus annuus L.) oil stored under different conditions

The aim of our study was to explore the antioxidant potential of Moroccan saffron stigmas（SS） and examine their performance as a natural antioxidant in enhancing the stability and quality properties of edible sunflower oil（SO）, with a focus on understanding the critical importance of edible oil stability for predicting quality deterioration during storage. Bearing this in mind, our research was designed to compare the efficacy of three concentrations of dried SS（at 0.2%, 0.3%, and 0.6%） against tocobiol（a synthetic antioxidant） as a positive control（at 0.3% concentration）. This comparison was conducted under three distinct storage conditions namely accelerated ageing（60 ℃）, exposure to light at ambient temperature, and darkness, to evaluate their impact on preventing severe oxidation and extending oil shelf-life. Oxidation state evolution was evaluated through peroxide value（PV）, free fatty acid（FFA）, anisidine value（p-AV）, K270value（conjugated trienes）, total oxidation index（TOTOX）, iodine value（IV）, and fatty acid composition（FA）. Our results revealed notable differences in stability tracking parameters. Specifically, these parameters were higher in samples stored under accelerated conditions, followed by the samples stored in ambient light, while those stored in darkness showed the highest stability among the three storage conditions. Supplementation of sunflower oil with SS and tocobiol significantly enhanced its oxidation stability. Notably, SS exhibited exceptional effectiveness in stabilizing sunflower oil regardless of SS dose, with the highest efficacy observed at 0.6%. This was evidenced by the slower rate of oxidation parameters under various storage conditions, highlighting a superior antioxidant activity compared to both the non-enriched oil and tocobiol-enriched oil. Furthermore, saffron stigmas, used as a natural supplement,contributed to the preservation of polyunsaturated fatty acids, indicating its potential as a robust source of natural antioxidan

Strategies to Build Trust and COVID-19 Vaccine Confidence and Engagement among Minority Groups in Scotland

As countries continue to deal with the global COVID-19 pandemic and its consequences, policymakers recognize that science, technology, and innovation(STI) practices offer a means of addressing many of the health problems that arise from the ongoing pandemic. Such recognition has given rise to many STI policy initiatives across various areas of science and policy, leading to a better understanding of coronavirus and the development of COVID-19 vaccines,treatments, and diagnostics. However, the practical implementation of vaccine and treatment strategies within local communities extends well beyond the laboratory. This study explored how misinformation and trust amplify or attenuate coronavirus and COVID-19 vaccine perceptions of those from ethnic minority groups deemed more susceptible to the impacts of the virus. Primary data in this study were collected in Scotland through semistructured interviews with 26 expert and nonexpert members from Scotland’s minority ethnic communities. The study findings show that risk perception is fluid and dependent on the information and evidential environment in which people find themselves. Misinformation, fake news, conspiracies, and trust or distrust(from prior experiences and historic practices) influence the perception of coronavirus and how risk messages are received, including the acceptance of coronavirus *** article reflects on Scotland’s approach to building trust and COVID-19 vaccine confidence and engagement based on the findings of this study, identifying areas of strength and areas for further improvement or research. The authors believe, as shown by our research, that vaccine engagement will be more impactful if developed by and with the public,and reflects public values, concerns, and priorities.

Introduction: Lower extremity arterial disease in diabetic patients has distinct characteristics. Objectives: To study the hospital frequency of lower extremity arterial disease and identify associated factors in diabetic patients at the Libreville University Hospital Center (CHUL). Materials and Methods: This was a cross-sectional study conducted from July 1, 2023, to January 31, 2024, in the endocrinology department of the CHUL. All patients with type 2 diabetes over the age of 18 admitted to this department, regardless of the reason for hospitalization, who provided informed consent, were included. Socio-demographic data and cardiovascular risk factors were recorded. Personal and family cardiovascular history and functional symptoms were investigated. The physical examination included measuring blood pressure, heart rate, and the ankle-brachial index in all patients. Results: A total of 219 patients were included, of whom 75 had lower extremity arterial disease, representing a prevalence of 34.24%. It was compensated in 28 cases (37.33%) and decompensated in 39 patients (52%). In eight cases (10.67%), there was critical chronic ischemia. Cardiovascular risk factors associated with diabetes were physical inactivity (89.5%), hypertension (55.25%), overweight (49.77%), obesity (22.84%), and smoking (10.04%). In multivariate analysis, only hypertension (OR = 2.09;95% CI: 1.05 - 4.17;p = 0.035) and LDL cholesterol (OR = 2.75;95% CI: 1.10 - 6.85;p = 0.03) were significantly associated with lower extremity arterial disease in diabetics at the University Hospital of Libreville. Conclusion: Lower extremity arterial disease is common in diabetic patients at the University Hospital of Libreville. It is often asymptomatic, thus requiring systematic screening.[-rId11-]

Rationale for Decision-Making Processes in Enhancement of Community Participation for Sustainable Mangrove Management in Lamu, Kenya

Decision-making is the process of deciding between two or more options in order to take the most appropriate and successful course of action in order to achieve sustainable mangrove management. However, the distinctiveness of mangrove as an ecosystem, and thus the attendant socio-economic and governance ramifications, causes the idea of decision making to become relatively distinct from other decision making process As a result, the purpose of this research was to evaluate the impact that community engagement plays in the decision-making process as it relates to the establishment of governance norms for sustainable mangrove management in Lamu County. In this study, a correlational research design was applied, and the researchers employed a mixed techniques approach. The target population was 296 respondents. The research used questionnaires and interviews to collect data. A descriptive statistical technique was utilized to perform an inspection and analysis on the data that was gathered. The findings indicated that having awareness about governance standards is beneficial during the process of making decisions. In addition, the findings demonstrated that respondents had the impression that the decision-making process was not done properly. On the other hand, the participants pointed out the positive aspects of the decision-making process and agreed that the participation of both gender was essential for the sustainable management of mangroves. Based on these data, it appeared that full community engagement in decision-making is necessary for sustainable management of mangrove forests.

Barriers to Community Participation in Governance Standards Setting for Sustainable Mangrove Management in Lamu County

Mangrove management requires an all-inclusive community engagement for sustainability in protection and conservation of mangroves. Previously, community participation was not considered pivotal but the rapid degradation of mangroves has necessitated their participation. Despite much legislation enacted, there are still challenges hindering community participation in Lamu County and this paper seeks to highlight barriers perceived by the various stakeholders in mangrove management. The study used a mixed methods approach to answer both qualitative and quantitative research questions and a correlational research design to explore all variables. The study shows that only 60% of the respondents agreed that there is lack of enforcement in mangrove conservation through community participation. However, less than 50% agreed on the challenges of lack of good management in mangroves (46%), lack of implementation mechanisms on mangrove conservation (47%) and lack of consultation in decision-making processes for mangrove conservation (36%). This demonstrates that lack of enforcement is the greatest challenge to the implementation of standards for mangrove conservation.

Phytochemical screening, antioxidant and antibacterial properties of daphne mucronata

OBJECTIVE:To aim at preliminary phytochemical screening of Daphne mucronata and evaluate its antioxidant and antibacterial ***:Preliminary phytochemical screening was conducted for the crude extracts using standard *** properties of crude methanolic extracts,n-hexane,chloroform,ethyl acetate and aqueous fractions of leaves,roots and bark were evaluated following standard *** antibacterial activities were checked against Acinetobacter baumannii(***),Escherichia coli(***),Pseudomonas aeruginosa(***),Staphylococcus aureus(***),Morganella morganii(***)and vancomycin resistant ***(VRSA).RESULTS:Carbohydrates,saponins,steroids,phenols/tannins,flavonoids and glycosides were present in different parts of Daphne *** extracts showed good antioxidant activity with EC50 values of 157.82-361.61μg/m *** extracts of roots showed good activity against ***(86.95%),***(85.18%)and ***(84.61%).Methanolic extracts of bark were active against ***(65.21%),***(65.21%)and ***(62.96%).Methanolic extracts of leaves showed good activity against ***(78.26%),***(77.78%),***(74.07%),***(73.07%),***(69.56%),VRSA(68%)and Proteus vulgaris(60%).The n-hexane fraction of roots was effective against ***(78.26%).Chlorofom fraction of roots showed moderate activity against ***(60.86%)and ***(61.53%).Ethyl acetate fraction of roots showed moderate activity against ***(69.56%),***(62.96%)and ***(69.23%).CONCLUSION:This study illustrates that Daphne mucronata possesses good antioxidant and antibacterial *** plant could be further exploited as potential natural antioxidant and as a new source of antimicrobials for treatment of various infections.

The Alternative Therapeutic in Pediatric Cancer: What We Know?

Scientific research has brought about enormous national advancements in cancer management. Currently, Morocco provides several therapeutic tools for cancer, among them chemotherapy, surgery and radiotherapy. In Morocco, there is not enough published data on the use of alternative medicine in pediatric oncology. Owing to the increased interest and lack of data on this practice, we conducted this study. It aims to evaluate the uptake, the types of therapies used, the factors influencing, the reasons for this choice and the cost of AM used by patients. An investigation through a questionnaire was carried out on thirty-four children following for cancer within the pediatric hematology and oncology department (PHO) of Marrakech. The survey was conducted over a period of one month and a half between July 25 and September 5, 2022. Data were collected through a questionnaire. From the medical files, we collected data regarding type of cancer, date of admission to PHO department and the treatment received. We asked about the type of AM used: herbs, honey, spiritual AM, zamzam water (water from a “sacred” source in Mecca). Using alternative therapies is a common practice in pediatric oncology with various reasons, among them, the declining socioeconomic level, the diversity, the cultural background, the psychological and the functional status of patients and sometimes the dissatisfaction with conventional medicine.

Effect of drying technologies on the biochemical properties of Stolephorus commersonnii

Objectives:Two different methods,namely traditional sundrying and solar dryer drying were employed to investigate the effect of drying method on the quality of Indian anchovies,Stolephorus *** and Methods:Fifty fish samples of Stolephorus commersonnii were purchased for the drying *** dried fishes were bought from the local *** solar drying the samples were kept in on the solar cabinet dryer KSD 25 designed and fabricated by the Central Institute of Fisheries Technology(CIFT).International standard AOAC methods were used for quality analysis of dried fish ***:The method of drying by the solar dryer is more hygienic and faster than the traditional sun-drying *** biochemical compositions and sensory evaluation of the products obtained showed significant variation between the two drying *** dryer-dried fish has got good organoleptic score than the traditional sundried *** overall quality of fish dried in the solar dryer was better than the traditionally sundried ***:The study clearly indicates that solar drying can be used as an environment-friendly method for fish processing which enhances food security.
