Long-term differences in urinary, bowel and sexual function among men treated with surgery versus radiation for prostate cancer

surgery and radiation have both been shown to increase the longterm diseasespecific survival rate for men with din icaUy localized prostate cancer. Although both modalities have demonstrated favor able effects on cancer control, questions regarding quality of life （QoL） and func tional outcomes remain incompletely answered. To date, no randomized prospective trials have been performed comparing the two treatment modalities and so indirect compar isons of longterm functional outcomes have served as a substitute to aid in patient coun seling and decisionmaking. As there is a pau city of longterm data comparing functional outcomes after radical prostatectomy and external beam radiation therapy, a recent art icle by Resnick et al.,1 has attempted to pro vide additional information about this topic in terms of continence, erectile function and bowel function. Utilizing the Prostate Cancer Outcomes study cohort, a populationbased cohort of men diagnosed with prostate cancer in the pro statespecific antigen （PsA） era, the authors compared rates of urinary incontinence, erect ile dysfunction, and bowel urgency at 2, 5 and 15 years after primary therapy. They showed that men undergoing prostatectomy had higher rates of incontinence and erectile dys function at 2 and 5 years, but these rates were similar to those in the radiotherapy group at 15 years. More specifically, men were approxi mately five times more likely to have urinary continence issues if they underwent prostatec tomv versus radiotherapy and almost three anda half times more likely to develop erectile dys function in the short to intermediateterm following primary treatment. As expected, rates of bowel urgency were higher in the radio therapy group at 2 and 5 years, but not sig nificantly different from the surgery group at 15 years. This paper reviews the progress on nano-aperture vertical-cavity surface-emitting lasers （VCsELs）. The design, fabrication, and polarization control of nano-aperture VCsELs are reviewed. With the nanoaperture evolving from conventional circular and square aperture to unique C-shaped, H-shaped, I-shaped, and bowtie-shaped aperture, both the near-field intensity and near-field beam confinement from nanoaperture VCsELs are significantly improved. As a high-intensity compact light source with sub-100- nm spot size, nano-aperture VCsELs are promising to realize many new near-field optical systems and applications.

Current recommendations of managing HBV infection in preconception or pregnancy

Hepatitis B remains a leading cause of cirrhosis, hepatocellular carcinoma and liver transplantation worldwide. Management of chronic hepatitis B during pregnancy is challenging. Transmission of hepatitis B to infants still occurs perinataily although immunoprophylaxis is widely available for infants born to mothers with chronic hepatitis B infection. The emerging data suggest that initiation of antiviral therapy in the beginning of the third trimester in highly viremic mothers can prevent immunoprophylaxis failure in their infants. The available drug safety data show that lamivudine, telbivudine and tenofovir are generally safe to be used during the pregnancy. In order to minimize the fetal exposure to the antiviral medication, antiviral therapy during the pregnancy should be limited to a selected group of patients with cirrhosis, high hepatitis B viral load, or prior history immunoprophylaxis failure. An elective Caesarean section may reduce the risk of perinatal transmission. For those females planning for pregnancy or in early stage of pregnancy, communication and follow-up among obstetrician, gastroenterologist, and primary care physician are important. In this article, we will review the features of hepatitis B infection before, during and after the pregnancy; the risk factors that increase mother-tochild transmission; safety data on antiviral drug use during pregnancy; and the potential role of Caesarean section in selected cases.

Implicit Continuous User Authentication for Mobile Devices based on Deep Reinforcement Learning

Estimating REY content of eastern Kentucky coal samples based on their associated ash elements

Coal and coal combustion byproducts can be considered as an alternative source of rare earth elements and yttrium （REY）. The study of relationships between REY and other main coal properties could have several advantages such as estimating REY content of coal particles and designing beneficial extraction method. In this investigation, inter-correlations between REY content with coal parameters （proximate and ash elements） for a wide range of eastern Kentucky coal samples （708 records） were explored. Results demonstrate that zircon and monazite are the main source of heavy and light rare earth elements （HREE and LREE）, respectively. Zr has the highest correlation with Y and Th has the strength relationship with Ce and La. In general, LREE have higher interaction with coal ash content in comparison with HREE. Results indicated that REY can be estimated quite satisfactorily by using their associated elements in coal ash.
