Retrospective analysis of hepatitis C infected patients treated through an integrated care model

AIm To determine if our health system's integrated model reflects sustained virologic response(SVR) outcomes similar to those in clinical trial data, maximizes adherence, and averts drug *** Subjects with chronic hepatitis C had their medical records reviewed from November 1st, 2014 through march 1st, 2016. Patients eligible for treatment were entered into an integrated care model therapy algorithm. The primary outcome was SVR12 based on intention to treat(ITT) analysis. Inclusion criteria consisted of both treatment na?ve and experienced patients over the age of 18 who were at least twelve weeks post-therapy completion with any genotype(GT) or mETAVIR score. Secondary outcomes included adherence, adverse events, and number of drug interaction *** At the time of analysis, 133 patients had reached twelve weeks post therapy with ITT. In the ITT analysis 70 patients were GT 1a, 26 GT 1b, 23 could not be differentiated between GT 1a or 1b, 8 GT 2, 4 GT 3, and 2 patients with multiple genotypes. The ITT treatment regimens consisted of 97 sofosbuvir(SOF)/ledipasvir(LDV), 8 SOF/LDV and ribavirin(RBV), 7 SOFand Simeprevir(SmV), 6 3D and RBV, 1 3D, 11 SOF and RBV, and 1 SOF, peg interferon alpha, and RBV. The overall SVR12 rate was 93% in the ITT analysis with a total of 6 patients relapsing. In patients with cirrhosis, 89% obtained SVR12. All 33 patients who were previous treatment failures achieved SVR12. Drug-drug interactions were identified in 56.4% of our patient population, 69 of which required interventions made by the pharmacist. The most common side effects were fatigue(41.4%), headache(28.6%), nausea(18.1%), and diarrhea(8.3%). Geographical distribution of the incidence of gastric cancer in Bhutan

AIm: To estimate the prevalence of gastric cancer(GC) in a cohort of patients diagnosed with GC and to compare it with patients diagnosed with all other types of gastro-intestinal(GI) cancer during the same ***: Between 2008 and 2013,five-year period,the medical records of all GI cancer patients who underwent medical care and confirm diagnosis of cancer were reviewed at the National Referral Hospital,Thimphu which is the only hospital in the country where surgical and cancer diagnosis can be made. Demographic information,type of cancer,and the year of diagnosis were ***: There were a total of 767 GI related cancer records reviewed during the study period of which 354(46%) patients were diagnosed with GC. There were 413 patients with other GI cancer including; esophagus,colon,liver,rectum,pancreas,gall bladder,cholangiocarcinoma and other GI tract cancers. The GC incidence rate is approximately 0.9/10000 per year(367 cases/5years per 800000 people). The geographic distribution of GC was the lowest in the south region of Bhutan 0.3/10000 per year compared to the central region 1.4/10000 per year,Eastern region 1.2/10000 per year,and the Western region 1.1/10000 per year. moreover,GC in the South part was significantly lower than the other GI cancer in the same region(8% vs 15%; OR = 1.8,95%CI: 1.3-3.1,P = 0.05). Among GC patients,38% were under the age of 60 years,mean age at diagnosis was 62.3(± 12.1) years with male-to-female ratio 1:0.5. The mean age among patients with all other type GI cancer was 60 years(± 13.2) and male-tofemale ratio of 1:0.7. At time of diagnosis of GC,342(93%) were at stage 3 and 4 of and by the year 2013; 80(23%) GC patients died compared to 31% death among patients with the all other GI cancer(P = 0.08).CONCLUSION: The incidence rate of GC in Bhutan is twice as high in the United States but is likely an underestimate rate because of unreported and undiagnosed cases in the villages. Timings of early crustal activity in southern highlands of mars:Periods of crustal stretching and shortening

Extensional and compressional structures are globally abundant on mars. Distribution of these structures and their ages constrain the crustal stress state and tectonic evolution of the planet. Here in this paper, we report on our investigation over the distribution of the tectonic structures and timings of the associated stress fields from the Noachis-Sabaea region. Thereafter, we hypothesize possible origins in relation to the internal/external processes through detailed morphostructural mapping. In doing so, we have extracted the absolute model ages of these linear tectonic structures using crater size-frequency distribution measurements, buffered crater counting in particular. The estimated ages indicate that the tectonic structures are younger than the mega impacts events(especially Hellas) and instead they reveal two dominant phases of interior dynamics prevailing on the southern highlands, firstly the extensional phase terminating around3.8 Ga forming grabens and then compressional phase around 3.5-3.6 Ga producing wrinkle ridges and lobate scarps. These derived absolute model ages of the grabens exhibit the age ca. 100 ma younger than the previously documented end of the global extensional phase. The following compressional activity corresponds to the peak of global contraction period in Early Hesperian. Therefore, we conclude that the planet wide heat loss mechanism, involving crustal stretching coupled with gravitationally driven relaxation(i.e.,lithospheric mobility) resulted in the extensional structures around Late Noachian(around 3.8 Ga). Lately cooling related global contraction generated compressional stress ensuing shortening of the upper crust of the southern highlands at the Early Hesperian period(around 3.5-3.6 Ga).

Cancer risk in primary sclerosing cholangitis: Epidemiology,prevention, and surveillance strategies

Primary sclerosing cholangitis(PSC) is a rare cholestatic liver disease characterized by progressive fibroinflammatory destruction of the intra-and/or extrahepatic biliary ducts. While its features and disease course can be variable,most patients with PSC have concurrent inflammatory bowel disease and will eventually develop liver cirrhosis and end-stage liver disease, with liver transplantation representing the only potentially curative option. Importantly,PSC is associated with a significantly increased risk of malignancy compared to the general population, mainly cholangiocarcinoma, gallbladder carcinoma,hepatocellular carcinoma, and colorectal cancer, with nearly 50% of deaths in patients with PSC being due to cancer. Therefore, robust surveillance strategies are needed, though uncertainty remains regarding how to best do so. In this review, we discuss the epidemiology, prevention, and surveillance of cancers in patients with PSC. Where evidence is limited, we present pragmatic approaches based on currently available data and expert opinion.

DPm dispersion inside a single straight entry using dynamic mesh model

Three-dimensional simulations of diesel particulate matter （DPm） distribution inside a single straight entry for the Load-Haul-Dump loader （LHD）-truck loading and truck hauling operations were conducted by using ANSYS FLUENT computational fluid dynamics software. The loading operation was performed for a fixed period of 3 min. The dynamic mesh technique in FLUENT was used to study the impact of truck motion on DPm distribution. The resultant DPm distributions are presented for the cases when the truck were driving upstream and downstream of the loading face. Interesting phenomena were revealed in the study including the piston effect, layering of DPm in the roof region, and backflow of diesel exhaust against ventilation. The results from the simulation can be used to determine if the areas inside the face area and straight entry exceed the current U.S. regulatory requirement for DPm concentration （〉160 pg/m3）. This research can guide the selection of DPm reduction strategies and improve the working practices for the underground miners.

Quaternion Quasi-Chebyshev Non-local means for Color Image Denoising

Quaternion non-local means(QNLm) denoising algorithm makes full use of high degree self-similarities inside images to suppress the noise, so the similarity metric plays a key role in its denoising *** this study, two improvements have been made for the QNLm: 1) For low level noise, the use of quaternion quasi-Chebyshev distance is proposed to measure the similarity of image patches and it has been used to replace the Euclidean distance in the QNLm algorithm. Since the quasi-Chebyshev distance measures the maximal distance in all color channels, the similarity of color images measured by quasi-Chebyshev distance can capture the structural similarity uniformly for each color channel; 2) For high level noise, quaternion bilateral filtering has been proposed as the preprocessing step in the QNLm algorithm. Denoising simulations were performed on 110 images of landscape, people, and architecture at different noise levels. Compared with QNLm, quaternion non-local total variation(QNLTV), and non-local means(NLm)variants(NLTV, NLm after wavelet threshold preprocessing, and the color adaptation of NLm), our novel algorithm not only improved PSNR/SSIm(peak signal to noise rate/structural similarity) and figure of merit values by an average of 2.77 dB/8.96% and 0.0491 respectively, but also reduced processing time.

Polyplex interaction strength as a driver of potency during cancer immunotherapy

many experimental cancer vaccines are exploring toll-like receptor agonists （TLRas） such as CpG, a DNA motif that agonizes toll-like receptor 9 （TLR9）, to trigger immune responses that are potent and molecularly-specific. The ability to tune the immune response is especially important in the immunosuppressive microenvironments of tumors. Because TLR9 is located intracellularly, CpG must be internalized by immune cells for functionality. Polyplexes can be self- assembled through electrostatics using DNA （anionic） condensed by a positively charged carrier. These structures improve cell delivery and have been widely explored for gene therapy. In contrast, here we use cationic poly （^-amino esters）（PBAEs） to assemble polyplexes from CpG as an adjuvant to target and improve immune stimulation in cells and mouse models. Polyplexes were formed over a range of PBAE：CpG ratios, resulting in a library of complexes with increasingly positive charge and stronger binding as PBAE：CpG ratio increased. Although higher PBAE：CpG ratios exhibited improved CpG uptake, lower ratios of PBAE：CpG--which condensed CpG more weakly, activated DCs and tumorspecific T cells more effectively. In a mouse melanoma model, polyplexes with lower binding affinities improved survival more effectively compared with higher binding affinities. These data demonstrate that altering the polyplex interaction strength impacts accessibility of CpG to TLRs in immune cells. Thus, physiochemical properties, particularly the interplay between charge, uptake, and affinity, play a key role in determining the nature and efficacy of the immune response generated. This insight identifies new design considerations that must be balanced for engineering effective immunotherapies and vaccines.

Real-time testing provides a viable experimental technique to evaluate the performance of structural systems subjected to dynamic ***-hydraulic actuators are often utilized to apply calculated displacements from an integration algorithm to the experimental structures in a real-time *** compensation of actuator delay is therefore critical to achieve stable and reliable experimental *** advances in compensation methods based on adaptive control theory enable researchers to accommodate variable actuator delay and achieve good actuator control for real-time ***,these adaptive methods all require time duration for actuator delay *** show that a good actuator delay estimate can help optimize the performance of the adaptive compensation *** rate of adaptation also requires that a good actuator delay estimate be acquired especially for the tests where the peak structural response might occur at the beginning of the *** paper presents a model reference adaptive control based method to identify the parameter of a simplified discrete model for servo-hydraulic dynamics and the resulting compensation *** are conducted using both numerical analysis and experimental results to evaluate the effectiveness of the proposed estimation method.

利用数字图像相关技术开展工业纯钛5种不同高宽比CT试样的疲劳裂纹扩展率试验,分别采用传统应力强度因子范围ΔK和基于CJP模型理论框架的新应力强度因子范围ΔKCJP对数据进行分析,得到的da/d N-ΔK和da/d N-ΔKCJP曲线均有效反映了试验数据分布规律,具有良好的拟合效果。相较于传统参量ΔK,ΔKCJP直接由裂尖位移场求解,无需进行修正即可用于材料疲劳裂纹扩展率描述,具有与CT试样高宽比无关的特性;作为ΔKCJP的两个组成参量,ΔKF、ΔKR与传统ΔK数据间均呈现出某种总体变化趋势,分别可用二次多项式和线性方程表征。构造了ΔKCJP与ΔK的关联函数,并由此推导出ΔKCJP表达式,为da/d N-ΔKCJP曲线在材料与结构的失效分析和安全评价中的应用提供了有效途径,拓宽了CJP模型的工程应用范围。
