The incidence of hepatocellular carcinoma(HCC)is increasing in the United States,and 50%-75%o patients with HCC will develop metastatic disease Orbital metastases from HCC are extremely *** report the case of a 52-year-old male with known metastatic HCC,who presented with severe proptosis and *** orbital mass was identified on magnetic resonance imaging(MRI)and confirmed to have hypermetabolic activity on positron emission tomography/computed *** received a palliative course of external beam radiation therapy to the right *** modulated radiation therapy (IMRT)was used to allow sparing of critical norma tissues in close proximity to the *** month after completion of IMRT to 58 Gray in 30 fractions delivered over 6 wk,the patient had a complete clinical,radiologic(MRI)and symptomatic response The patient continues to have local control in the orbi 1.7 years after therapy *** critical norma structures were kept below the tolerance dose using IMRT,and no toxicities were observed.

创建了一个剪切破坏的钢筋混凝土梁-柱节点试验试件的大型数据库。所有的试件(对于普通的节点至少有最小的横向增强约束)承受循环横向荷载,没有面外受力构件(横向的梁或板)。数据库中的主要类别包括节点剪切破坏的类型(有连接、无连接、纵向梁增强的屈曲)以及联结的类型(内、外和屈曲节点)。对确定关键点(体现了最显著的刚度变化)上的剪应力和/或应变性能的影响参数进行了评估。混凝土裂缝以及横向和纵向梁增强节点的屈曲,从全部和局部性能到节点剪切破坏(最大的试验楼层剪力)的初始点,都引发了刚度上很明显的变化。通过材料性能、节点板的几何特征、增强约束、柱的轴向荷载和增强粘结条件,对节点剪力性能的影响参数进行分组。研究发现,对于不同的连接类型和破坏模式次序,节点剪力性能的最重要影响参数有些不同。然而,在数据库所有数据中,混凝土压应力是节点剪力性能最普遍的控制参数。此外,通过简单的方程研究了节点剪切裂缝的应力和应变(表现在第一个明显刚度变化的点)。最后,用AC352R-02推荐的设计校正方法对数据库中的构件进行了验证。

Objective： Patients with poorly controlled diabetes have more medical complications and are more difficult to manage. The objective of the present study was to evaluate the clinical outcomes of successful implementation of an employer initiated community pharmacist-based disease management program for diabetic patients with poorly controlled diabetes. Methods： Employees with poorly controlled diabetes （glycosylated hemoglobin （A1 C） level 〉 7.5%） were identified fi＇om a large diabetes disease management program, in a rural setting in Texas, US. A longitudinal retrospective study was conducted, analyzing clinical indicators in the diabetes patients following the community pharmacist-based disease management program. The program involved a comprehensive drug therapy assessment and individualized disease management education. Primary outcome measured in the present study was A1C levels, assessed at the baseline visit and at the end of the intervention. Results： A total of 64 patients with poorly controlled diabetes were identified. Significant improvement in mean clinical outcome scores was achieved for A1C levels （p = 0.0011）. At the end of the 1 year longitudinal intervention, targeted body mass index and A1C goals were attained by 35.9% （p 〈 0.001） and 15.6% patients, respectively. The 10 patients reaching goal levels post intervention were in the group that had baseline A1C of 7.5 to 9%. However, patients with 〉 9% A1C levels at baseline had a significant reduction （mean 2.1, p 〈 0.001） post intervention. Conclusion： The community pharmacist-based diabetes disease management program improved A1C levels of patients with poorly controlled diabetes.

Encapsulation of DASP （4-[4-（dimethylamino）styryl]-l-octylpyridinium bromide） with amylose, and conformational transitions of amylose upon complexation with the dyes were investigated using UV-vis, fluorescence and two-photon absorption properties in various Dimethyl Sulfoxide （ DMSO）/H20 mixtures. Encapsulation of the dye was achieved by dilution of DMSO stock solutions of the chromophores （1 x 10^-3 M） and amylose （1 x 10^-1 M） with DMSO, followed by a dropwise addition of H20 with continuous stirring at room temperature. At less than 40% aqueous DMSO solutions, the chromophores had a stable encapsulation with amylose and showed over 100 times the fluorescence intensity of non-encapsulated chromophores. Both of the dye solution with and without amylose also exhibit 2PA （two-photon absorption）, as determined by nonlinear absorption measurements using 10 ns laser pulses at 800 nm. The respective values of two-photon cross-sections are 13.3 x 10.20 for encapsulated dye solution and 2.53 x 10.20 cma/GW for non encapsulated dye solution. The data clearly indicate that the contribution of the encapsulated dye dominates the two-photon absorption cross-section of the non encapsulated dye at 800 nm.

Recently published Medical Physics Practice Guideline 5.a. (MPPG 5.a.) by American Association of Physicists in Medicine (AAPM) sets the minimum requirements for treatment planning system (TPS) dose algorithm commissioning and quality assurance (QA). The guideline recommends some validation tests and tolerances based primarily on published AAPM task group reports and the criteria used by IROC Houston. We performed the commissioning and validation of the dose algorithms for both megavoltage photon and electron beams on three linacs following MPPG 5.a. We designed the validation experiments in an attempt to highlight the evaluation method and tolerance criteria recommended by the guideline. It seems that comparison of dose profiles using in-water scan is an effective technique for basic photon and electron validation. IMRT/VMAT dose calculation is recommended to be tested with some TG-119 and clinical cases, but no consensus of the tolerance exists. Extensive validation tests have provided the better understanding of the accuracy and limitation of a specific dose calculation algorithm. We believe that some tests and evaluation criteria given in the guideline can be further refined.
