Patients and physicians understand the im.ortance of self-care following spinal cord injury (SCI), yet m.ny individuals with SCI do not adhere to recom.ended self-care activities despite logistical supports. Neurobehavioral determ.nants of SCI self-care behavior, such as im.ulsivity, are not widely studied, yet understanding them.could inform.efforts to im.rove SCI self-care. We explored associations between im.ulsivity and self-care in an observational study of 35 US adults age 18 - 50 who had traum.tic SCI with paraplegia at least six m.nths before assessm.nt. The prim.ry outcom. m.asure was self-reported self-care. In LASSO regression m.dels that included all neurobehavioral m.asures and dem.graphics as predictors of self-care, dispositional m.asures of greater im.ulsivity (negative urgency, lack of prem.ditation, lack of perseverance), and reduced m.ndfulness were associated with reduced self-care. Outcom. (m.gnitude) sensitivity, a latent decision-m.king param.ter derived from.com.utationally m.deling successive choices in a gam.ling task, was also associated with self-care behavior. These results are prelim.nary;m.re research is needed to dem.nstrate the utility of these findings in clinical settings. Inform.tion about associations between im.ulsivity and poor self-care in people with SCI could guide the developm.nt of interventions to im.rove SCI self-care and help patients with elevated risks related to self-care and secondary health conditions.

This is a com.entary on the often-observed phenom.non of observing task-based brain signaling differences between clinical populations and healthy control participants in the absence of any behavioral decrem.nts in the clinical group. We offer several explanations for why brain-based differences am.d norm.tive perform.nce m.y be of interest to researchers and clinicians. First, neural processing in the clinical group m.y not be as efficient as that in the control group. Second, differences in activation could reveal im.ortant differences in the cognition behind the (norm.tive) behavior. Third, differences in activation m.y be prognostic biom.rkers of injury or decline. In addition, we contend that sim.lar behavior between groups is im.ortant in properly interpreting brain data. Finally, we offer caveats and future directions to further clarify brain m.chanism. underlying behavior in clinical populations.

Despite the advent of relatively reliable m.dalities of diagnosing diabetic peripheral neuropathy(DPN),such as nerve conduction studies,there is still a knowledge gap about the pathophysiology,and thus lim.ted available in-terventions for sym.tom.control and curtailing disease *** pharm.-cologic aspect of m.nagem.nt is m.inly centred on pain control,however,there are several im.ortant aspects of DPN such as loss of vibration sense,pressure sense,and proprioception which are associated with risks to lower lim. health,which pharm.cotherapy does not ***,published evidence suggests non-pharm.cologic interventions such as glycaem.c control through dietary m.dification and exercise need to be com.ined with other m.asures such as psychotherapy,to reach a desired,however m.dest *** is em.rging as an im.ortant treatm.nt m.dality for several chronic m.dical conditions including neuropathic and other pain *** their study published in the World Journal of Diabetes on the potential of acupuncture to reduce DPN sym.tom. and enhance nerve conduction param.ters,Hoerder et al have been able to dem.nstrate that acupuncture im.roves sensory function and that this effect is likely sustained two m.nths after treatm.nt *** previous studies also support these findings,larger m.lti-center random.zed control trials including a sham.controlled arm.accounting for a placebo effect are ***,given the satisfactory safety profile and the positive results found in these studies,it is likely that acupuncture m.y becom. an im.ortant aspect of the repertoire of effective DPN m.nagem.nt.
