Recent trends in liver transplantation for alcoholic liver disease in the United States

AIM To examine temporal changes in the indications for liver transplantation(LT) and characteristics of patients transplanted for alcoholic liver disease(ALd).METHOdS We performed a retrospective cohort analysis of trends in the indication for LT using the United Network for Organ Sharing(UNOS) database between 2002 and 2015. Patients were grouped by etiology of the liver disease and characteristics were compared using χ~2 and t-tests. Time series analysis was used identifying any year with a significant change in the number of transplants per year for ALd, and before and after eras were modeled using a general linear model. Subgroup analysis of recipients with ALd was performed by age group, gender, UNOS region and etiology(alcoholic cirrhosis, alcoholic hepatitis and hepatitis C-alcoholic cirrhosis dual listing).RESULTS Of 74216 liver transplant recipients, ALd(n = 9400, 12.7%) was the third leading indication for transplant after hepatitis C and hepatocellular carcinoma. Transplants for ALd, increased from 12.8%(553) in 2002 to 16.5%(1020) in 2015. Time series analysis indicated a significant increase in the number of transplants per year for ALd in 2013(P = 0.03). There were a stable number of transplants per year between 2002 and 2012(linear coefficient 3, 95%CI:-4.6, 11.2) an increase of 177 per year between 2013 and 2015(95%CI: 119, 234). This increase was significant for all age groups except those 71-83 years old, was observed for both genders, and was incompletely explained by a decrease in transplants for hepatitis C and ALd dual listing. All UNOS regions except region 9 saw an increase in the mean number of transplants per year when comparing eras, and this increase was significant in regions 2, 3, 4, 5, 6, 8, 10 and *** There has been a dramatic increase in the number of transplants for ALd starting in 2013.

d-MELd risk capping improves post-transplant and overall mortality under markov microsimulation

AIM: To hypothesize that the product of calculated Model for End-Stage Liver disease score excluding exception points and donor age(d-MELd) risk capping ± Rule 14 could improve post liver transplant and overall survival after ***: Probabilities derived from the United Network for Organ Sharing database between 2002 and 2004 were used to simulate potential outcomes for all patients listed for transplantation. The Markov simula-tion was then modified by screening matches using a 1200 or 1600 d-MELd risk cap ± allowing transplants for Model for End-Stage Liver disease(MELd) ≤ 14(Rule 14). The differential impact of the rule changes was ***: The Markov simulation accurately reproduced overall and post transplant survival. A 1200 d-MELd risk cap improved post-transplant survival. Both the 1200 and 1600 risk caps improved overall survival for waitlisted patients. The addition of Rule 14 further improved post transplant and overall survival by redistribution of donor livers to recipients in higher MELd subgroups. The mechanism for improved overall and post-transplant survival after listing was due to shifting a larger percentage of transplants to the moderate MELd score subgroup(MELd 15-29) while also ensuring that high MELd recipients have livers of high quality to achieve excellent post transplant ***: A 1200 d-MELd risk cap + Rule 14 provided the greatest overall benefit primarily by focusing liver transplantation towards the moderate MELd recipient.

This paper discusses a class of interval alignment （IA） scheduling policies, which are particularly effective for the systems that do not have Markovian structure. The numerical results show that IA policies effectively smooth part flows, improve performance and decrease average Work-in-Process （WlP） by adding intermediate delays to the system, The boundary of IA policy is proven and the applications of IA policy in the system with multiple stream arrivals have been discussed. With the combination of release policy, it is practical to implement IA to multiple stream arrival system.

Promises and paradoxes of regulatory T cells in inflammatory bowel disease

Since their discovery two decades ago,Cd4+Cd25+Foxp3+ regulatory T cells(Tregs) have become the subject of intense investigation by immunologists. Unlike other T cells,which promote an immune response,Tregs actively inhibit inflammation when activated by their cognate antigen,thus raising hope that these cells could be engineered into a highly targeted,antigenspecific,immunosuppressant therapy. Although Tregs represent less than 10% of circulating Cd4+T cells,they have been shown to play an essential role in preventing or limiting inflammation in a variety of animal models and human diseases. In particular,spontaneous intestinal inflammation has been shown to occur in the absence of Tregs,suggesting that there may be a Treg defect central to the pathogenesis of human inflammatory bowel disease(IBd). However,over the past decade,multiple groups have reported no qualitative or quantitative deficits in Tregs from the intestines and blood of IBd patients to explain why these cells fail to regulate inflammation in Crohn's disease and ulcerative colitis. In this review,we will discuss the history of Tregs,what is known about them in IBd,and what progress and obstacles have been seen with efforts to employ them for therapeutic benefit.

The subeconomic Mount Novit Zn-Pb-Ag deposit is located approximately 20 km south of Mount Isa,*** contrast to the nearby Mount Isa,Hilton and George Fisher Zn-Pb-Ag deposits,mineralisation at Mount Novit is situated to the west of the regional-scale Mount Isa Fault and is hosted in the Moondarra Siltstone as opposed to the Urquhart ***-grade(d veining and consists of pyrrhotite,pyrite and sphalerite with lesser ***-grade(>10 wt.%Zn+Pb)mineralisation occurs as a matrix supported breccia dominated by sphalerite and pyrrhotite with galena,pyrite,and ***-situ U-Pb geochronology was completed on apatite and two textural varieties of ***-grained(dral to anhedral monazite is located within highly foliated biotite alteration directly adjacent Zn-Pb-Ag mineralisation and yields a mean weighted^(207)Pb/^(206)Pb age of 1527±18 Ma(MSWd=1.06).This age is consistent with the formation of highly foliated biotite alteration during d;deformation of the Isan *** from the same fabric yields a lower intercept age of1443±29 Ma(MSWd=1.30).Consistent with previous studies,this age is interpreted to represent the age of a major thrusting event along the Mount Isa Fault that resulted in the cooling of the Mount Novit area below~375℃.Coarse-grained monazite is coeval with Zn-Pb-Ag mineralisation and yields a mean weighted^(207)Pb/^(206)Pb age of 1457±11 Ma(MSWd=0.28).Sphalerite from Mount Novit has low concentrations(d Ga and a relatively high concentration of In(5 to>10 ppm),possibly reflecting the leaching of the metals from an underlying basement *** GGIMFis geothermometer(Frenzel et al.,2016)produced a mean formation temperature of 345±52℃.The timing and temperature of Zn-Pb-Ag mineralisation is consistent with the age and cooling temperature of apatite presented in this *** on these correlations,we

A new direction for Alzheimer's research

despite decades of research, at present there is no curative therapy for Alzheimer＇s disease. Changes in the way new drugs are tested appear to be necessary. Three changes are presented here and will be discussed. The first change is that Alzheimer＇s disease must be considered a disease of four major pathological processes, not one. The four processes are： 1） vascular hy- poperfusion of the brain with associated mitochondrial dysfunction, 2） destructive protein inclusions, 3） uncontrolled oxidative stress, and 4） proinflammatory immune processes second- ary to microglial and astrocytic dysfunction in the brain. The second change recommended is to alter the standard cognitive measurement tools used to quantify mental decline in test patients. Specifically the dementia Severity Rating Scale （dSRS） should supersede Mini-Mental State Examination （MMSE） and other popular tests, and a measurement scale developed in research should be used to produce a linear and non-irregular baseline. Finally, accepting the concept that four etiologies cause Alzheimer＇s disease leads to the last necessary change, that new thera- pies must be employed directed against all four causes, likely as a combination. There are drugs ready to be employed in such a combinations which are available and used clinically for other purposes so can be used ＂offlabel＂ and one such combination is suggested.
