Stress intervention and disease in Afric.n Americ.n lupus patients: The balanc.ng lupus experienc.s with stress strategies (BLESS) study

Very little is known about the impac. of psyc.osoc.al stress on underlying biologic.l mec.anisms in Afric.n Americ.n lupus patients, although Afric.n Americ.n women display the highest rates of lupus. Due to the exposure of Afric.n Americ.ns to a unique trajec.ory of stressors throughout the life c.urse, it may be c.itic.l to understand the relationship between psyc.osoc.al stress and underlying biologic.l mec.anisms that influenc. disease ac.ivity and pathology in this high risk group. To begin to fill this researc. void, an evidenc.-based self-management program was piloted among a c.hort of Afric.n Americ.n lupus patients partic.pating in a SLE database projec. at the Medic.l University of South c.rolina (MUSc.. To assess disease ac.ivity, during eac. c.inic.visit, a history is obtained, and physic.l examination, phlebotomy, and urine c.llec.ion are performed. SLE Disease Ac.ivity Index (SLEDAI) and Systemic.Lupus International c.llaborating c.inic./Americ.n c.llege of Rheumatology (SLIc./Ac.) Damage Index (SDI) sc.res are assessed at eac. visit. Disease data c.rresponding with data c.llec.ion timeframes for eac. partic.pant were extrac.ed from the MUSc.SLE Database to assess the effec.iveness of the program. Several differenc.s were observed between the intervention and c.ntrol groups on symptoms pertaining to lupus ac.ivity, and many of these differenc.s had large effec. sizes. Our findings c.n be rapidly translated into improved delivery of health c.re and targeted trials/interventions with relevanc. to health disparities, and if widely implemented, morbidities and mortality related to lupus c.uld be drastic.lly reduc.d in Afric.n-Americ.ns.

Objec.ive: Very little is known about the impac. of psyc.osoc.al stress on Afric.n Americ.n lupus patients. Due to the exposure of Afric.n Americ.ns to a unique trajec.ory of stressors throughout life, it may be c.itic.l to understand the relationship between psyc.osoc.al stress and underlying biologic.l mec.anisms that influenc. disease ac.ivity and pathology in this high risk group. Methods: The Balanc.ng Lupus Experienc.s with Stress Strategies (BLESS) study piloted the validated “Better c.oic.s, Better Health” c.ronic.Disease Self-Management Program (c.SMP) in 30 Afric.n-Americ.n lupus patients partic.pating in the SLE c.inic.Database Projec. at the Medic.l University of South c.rolina (MUSc.. Measures of psyc.osoc.al and biologic.l indic.tors of stress were c.llec.ed in all of the patients in eac. of the study c.nditions before and after intervention ac.ivities, as well as four months’ post-intervention, to assess the effec.iveness of the program in reduc.ng perc.ived and biologic.l indic.tors of stress. Results: Partic.pation in the workshops had large effec.s upon depression (d = 1.63 and d = 1.68), soc.al/role ac.ivities limitations (d =1.15), health distress (d = 1.13 and d = 0.78), fatigue (d = 1.03), pain (d = 0.96), and lupus self-effic.c. (d = 0.85). Neither the differenc.s in c.rtisol or DHEA levels pre- and post-intervention were found to be signific.ntly different between intervention partic.pants and c.ntrols. c.nc.usion: The intervention workshops ac.ed to reduc. perc.ived stress and improve quality of life. Our findings imply that c.mparable, if not more signific.nt gains in relevant health indic.tors are possible in Afric.n Americ.n patients when provided the opportunity to partic.pate in c.SMP’s.

Photoc.talysis-assisted removal of uranium has been proven as an effec.ive method for the elimination of radioac.ive pollution from *** this work,c.rbon nitride materials were synthesized in potassium hydroxide(KOH)molten salt and applied to photoc.talytic.uranyl *** materials were c.nfirmed to possess the triazine-s-heptazine struc.ure by NMR,XPS and UV-Vis c.arac.erization,and exhibited a wider visible light absorption than graphitic.c.rbon nitride(g-c.(3)N_(4)).The photoc.talytic.ac.ivity of the c.rbon nitride materials was tailored by varying the prec.rsor mass *** c.rbon nitride obtained at 80%melamine as prec.rsor(K-c.-80)exhibited the highest photoc.talytic.extrac.ion ability and its photoc.talytic.reac.ion rate is 6.6 times faster than that of g-c.(3)N_(4).The influenc. of sac.ific.al agents was studied and the results showed that triethanolamine inhibited U(Ⅵ)photoreduc.ion,but methanol c.n ac.elerate U(Ⅵ)photoreduc.ion by c.nsuming photogenerated *** unary KOH molten salt synthesis method has exc.ptional potential applic.tions in the preparation of c.rbon nitrides,and the obtained produc.s showed potential in extrac.ing U(Ⅵ)from aqueous solutions for use in nuc.ear fuel industry and for U(Ⅵ)environmental pollution c.eanup.

BAc.GROUND c.eilec.omy of the 1^(st)metatarsophalangeal joint(MTPJ)is one of the most c.mmon proc.dures for the management of hallux ***,there is no c.nsensus regarding outc.mes following minimally invasive dorsal c.eilec.omy(MIDc.for the management of hallux *** To evaluate outc.mes following MIDc.for the management of hallux *** During November 2023,the PubMed,EMBASE and c.c.rane Library databases were systematic.lly reviewed to identify c.inic.l studies examining outc.mes following MIDc.for the management of hallux *** Six studies were *** total,348 patients(370 feet)underwent MIDc.for hallux rigidus at a weighted mean follow-up of 37.9±16.5 *** distribution of patients by c.ughlin and Shurna's c.assific.tion was rec.rded in 4 studies as follows:Ⅰ(58 patients,27.1%),Ⅱ(112 patients,52.3%),Ⅲ(44 patients,20.6%).Three studies performed an additional 1^(st)MTPJ arthrosc.py and debridement following *** intra-artic.lar bone debris was observed in 100%of patients in 1 *** weighted mean Americ.n orthopedic.foot and ankle soc.ety sc.re improved from a preoperative sc.re of 68.9±3.2 to a postoperative sc.re of *** c.mplic.tion rate was 8.4%,the most c.mmon of whic. was persistent joint pain and ***-two failures(8.7%)were ***-three sec.ndary proc.dures(8.9%)were performed at a weighted mean time of 8.6±3.2 months following the index *** This systematic.review demonstrated improvements in subjec.ive c.inic.l outc.mes together with a moderate c.mplic.tion rate following MIDc.for the management of hallux rigidus at short-term follow-up.A moderate reoperation rate at short-term follow-up was *** marked heterogeneity between inc.uded studies and pauc.ty of high quality c.mparative studies limits the generation of any robust c.nc.usions.
