Pattern of Presentation of Iatrogenic Biliary Injury Following Laparoscopic Cholecystectomy

Background: Cholecystectomy is one of the most now common abdominal surgeries performed every day. The incidence of bile duct injury (BDI) following open cholecystectomy is only 0.1% - 0.2%. After the introduction of laparoscopic cholecystectomy, the incidence has gone up to 0.4% - 0.7%. The present study is a prospective analysis of all patients with bile duct injury who were admitted to Dhaka Medical College Hospital during or at a variable period following cholecystectomy. Methods: To determine the pattern of presentation of iatrogenic biliary injury following cholecystectomy in the department of surgery of Dhaka Medical College Hospital, a total of 30 patients were purposively selected from May 2018 to November 2018. Patient particulars, records of physical and clinical evaluation, and operative details were collected by individual researchers. Data analysis was done by SPSS for windows version 21. Results: BDI was found very common among the age group 21 - 30 yrs (36%) and female dominant (60%). Majority of the patients presented with abdominal pain (96%), intra-abdominal collection (88%), biliary peritonitis (68%), cholangitis (60%), and obstructive jaundice (40%), and biliary fistula (40%). Laparoscopic cholecystectomy (84%) was the principal cause of biliary injury in our study. 48% of patients experienced clinical features within 7 days post-cholecystectomy. Per-operative diagnosis was done in only 12% of cases. 44% of patients in this study were recognized as Bismuth grade-3, followed by 36%, grade-2 patients. Management outcomes included wound infection (41.66%), minor bile leak (25%), peritonitis (8.33%), and renal impairment (8.33%). Conclusion: The effect of BDI is an extremely distressful clinical condition for the patients and their family members, hence proper care and management protocol should be followed.

An Assessment of Indoor Environmental Quality in School Buildings in the State of Kuwait

In this study, indoor quality and environmental comfort were investigated in secondaiy school buildings located in the State of Kuwait. Comfort variables such as temperature and relative humidity (thermal comfort), noise (acoustic comfort), illumination (visual comfort), as well as allocated classroom floor area per student (spatial comfort) were measured. Data was collected over a 7-month period on a spot basis during school hours in student-occupied classrooms at 46 selected schools. The measured data was then compared to international guidelines and standards related to indoor environment quality. The data for noise and allocated space were shown to be in the comfort ranges in all the schools. However, 11% of the schools are not adequately illuminated, 33% had temperatures not within the recommended limits, and 22% of the schools had humidity levels either higher or lower than the recommended levels. Also, 9% of the schools had low illummation readings. In addition, during the data monitoring, a survey was conducted by which the student occupants completed a questionnaire so that subjective and objective evaluations could be compared. The findings of the questionnaire displayed significant correlations between the measured data and some ailments and other corr^laints experienced by the students. Ultimately, the results found in this research will provide a baseHne for comparison with future indoor environment quality assessments in buildings. Furthermore, recommendations are suggested in order to improve the environmental quality problems encountered in some of the schools, which may be beneficial for policymakers, facilities managers, and design engineers.

Analytical case study of basal metabolic rate of mildly symptomatic COVID-19 patients with reference to mizaj

Air Pollution Risk Assessment Using GIS and Remotely Sensed Data in Kirkuk City,Iraq

According to World Health Organization(WHO)estimates and based on a world population review,Iraq ranks tenth among the most air-polluted countries in the *** this study,the authors tried to evaluate the outdoor air of Kirkuk City north of *** authors relied on two types of data:field measurements and remotely sensed *** air quality points were determined in the study region representing the monthly average measurements implemented for the one-year *** information systems(GIS)based geo-statistic and geo-processing techniques have been applied to collected *** distribution data related to Air Quality Index(AQI),and Particulate Matter(PM10 and PM2.5)were obtained by mapping collected *** sensed data of PM2.5 were analyzed and compared with the collected *** impacts were assessed per each air pollutant determined in the *** distribution maps revealed the hazardous air type in the study *** AQI ranged between 300 and 472μg/m^(3)referring to unhealthy,very unhealthy,and hazardous classes of ***,PM10 ranged between 300 and 570μg/m^(3)indicating the same class of air pollution from unhealthy to *** PM2.5 ranged between 40 and 60μg/m^(3)which represents unhealthy air for sensitive persons and unhealthy *** remotely sensed data revealed different air types for the study period ranging from 14.5 to 52.5μg/m^(3)represented in moderate and unhealthy air for sensitive *** correlations were obtained where the mean local R2(coefficient of determination)was obtained as *** assessed data were within high air pollution that requires immediate intervention for controlling causes and eliminating their effects.

Comparison of awareness of diabetes mellitus type Ⅱ with treatment’s outcome in term of direct cost in a hospital in Saudi Arabia

BACKGROUND Saudi Arabia is among the top 10 countries with the highest prevalence of *** of prevention and the indirect cost must be calculated to increase the awareness of society and to emphasize disease prevention and limit further *** To understand the importance of awareness and the impact on the expenditure of diabetes mellitus and treatments *** A prospective descriptive and comparative survey was carried out among patients with diabetes mellitus in Saudi *** One hundred and one participants were included in the study of which 40% were female and one third were above the age of *** mean of the first HbA1c reading was 6.95,and the median was *** mean of the second reading of HbA1c was 7.26,and the median was *** mean body mass index was 32.1,and the median was *** average yearly cost of the medication was 995.14 *** participants who think that a healthy low-sugar diet can affect blood sugar with those who do not,showed a statistically significant difference when cost was considered (P value = 0.03).Also,when comparing the group of participants who know when to take their oral hyperglycemic medicine and their yearly direct cost and those who do not know when to take it,by using independent sample T test,showed significant statistical difference (P value = 0.046).CONCLUSION It is essential for the governments to invest in ways to prevent and help in the early detection of such an expensive disease by performing national screening and education *** pharmaco-economic studies can be done to help the decision-maker in our hospitals think about strategies to help the patient to be physically fit by offering gymnasium or places to walk or contract.
