AIM: To evaluate the accuracy of endoscopic ultrasound (EUS) in the staging of esophageal cancer. METHODS: Only EUS studies confirmed by surgery were selected. Articles were searched in Medline and Pubmed. Two reviewers independently searched and extracted data. Meta-analysis of the accuracy of EUS was analyzed by calculating pooled estimates of sensitivity, specificity, likelihood ratios, and diagnostic odds ratio. Pooling was conducted by both the Mantel-Haenszel method (fixed effects model) and DerSimonian laird method (random effects model). The heterogeneity of studies was tested using Cochran’s Q test based upon inverse variance weights. RESUlTS: Forty-nine studies (n = 2558) which met the inclusion criteria were included in this analysis. Pooled sensitivity and specificity of EUS to diagnose T1 was 81.6% (95% CI: 77.8-84.9) and 99.4% (95% CI: 99.0-99.7), respectively. To diagnose T4, EUS had a pooled sensitivity of 92.4% (95% CI: 89.2-95.0) and specificity of 97.4% (95% CI: 96.6-98.0). With Fine Needle Aspiration (FNA), sensitivity of EUS to diagnose N stage improved from 84.7% (95% CI: 82.9-86.4) to 96.7% (95% CI: 92.4-98.9). The P value for the χ2 test of heterogeneity for all pooled estimates was > 0.10. CONClUSION: EUS has excellent sensitivity and specificity in accurately diagnosing the TN stage of esophageal cancer. EUS performs better with advanced (T4) than early (T1) disease. FNA substantially improves the sensitivity and specificity of EUS in evaluating N stage disease. EUS should be strongly considered for staging esophageal cancer.

Articles were searched in Medline, Pubmed, Ovid journals, Cumulative index for nursing & allied health literature, International pharmaceutical abstracts, old Medline, Medline nonindexed citations, and Cochrane control trial registry. Two reviewers independently searched and extracted data. The differences were resolved by mutual agreement. 2 × 2 tables were constructed with the data extracted from each study. Meta-analysis for the accuracy of EUS was analyzed by calculating pooled estimates of sensitivity, specifi city, likelihood ratios, and diagnostic odds ratio. Pooling was conducted by both the Mantel-Haenszel method (fi xed effects model) and DerSimonian laird method (random effects model). The heterogeneity of studies was tested using Cochran's Q test based upon inverse variance weights. RESUlTS: Initial search identified 1620 reference articles and of these, 376 relevant articles were selected and reviewed. Twenty-two studies (n = 1896) which met the inclusion criteria were included in this analysis. Pooled sensitivity of T1 was 88.1% (95% CI: 84.5-91.1) and T2 was 82.3% (95% CI: 78.2-86.0). For T3, pooled sensitivity was 89.7% (95% CI: 87.1-92.0). T4 hada pooled sensitivity of 99.2% (95% CI: 97.1-99.9). For nodal staging, the pooled sensitivity for N1 was 58.2% (95% CI: 53.5-62.8) and N2 was 64.9% (95% CI: 60.8-68.8). Pooled sensitivity to diagnose distant metastasis was 73.2% (95% CI: 63.2-81.7). The P for chi-squared heterogeneity for all the pooled accuracy estimates was > 0.10. CONClUSION: EUS results are more accurate with advanced disease than early disease. AIM:To evaluate the accuracy of endoscopic ultrasound (EUS), EUS-fine needle aspiration (FNA) in evaluating mediastinal lymphadenopathy. METHODS:Only EUS and EUS-FNA studies confirmed by surgery or with appropriate follow-up were selected. Articles were searched in Medline, Pubmed, and Cochrane control trial registry. Only studies from which a 2 × 2 table could be constructed for true positive, false negative, false positive and true negative values were included. Two reviewers independently searched and extracted data. The differences were resolved by mutual agreement. Meta-analysis for the accuracy of EUS was analyzed by calculating pooled estimates of sensitivity, specificity, likelihood ratios, and diagnostic odds ratios. Pooling was conducted by both Mantel-Haenszel method (fixed effects model) and DerSimonian laird method (random effects model). The heterogeneity of studies was tested using Cochran’s Q test based upon inverse variance weights. RESUlTS:Data was extracted from 76 studies (n = 9310) which met the inclusion criteria. Of these, 44 studies used EUS alone and 32 studies used EUS-FNA. FNA improved the sensitivity of EUS from 84.7% (95% CI:82.9-86.4) to 88.0% (95% CI:85.8-90.0). With FNA, the specificity of EUS improved from 84.6% (95% CI:83.2-85.9) to 96.4% (95% CI:95.3-97.4). The P forchi-squared heterogeneity for all the pooled accuracy estimates was > 0.10. CONClUSION:EUS is highly sensitive and specific for the evaluation of mediastinal lymphadenopathy and FNA substantially improves this. EUS with FNA should be the diagnostic test of choice for evaluating mediastinal lymphadenopathy.

Hyoscyamine, anisodamine and scopolamine are tropane alkaloids present in some Solanaceae species and used in modern medicine. l-Hyoscyamine is hydroxylated to 6β-hydroxyhyoscyamine (anisodamine) and then epoxidated to scopolamine by the dual action of hyoscyamine 6β-hydroxylase (H6H), a 2-oxoglutarate dependent dioxygenase. A natural mutation in the Gly-220 residue to Cys was previously shown to be associated with the loss of function of H6H in Mandragora officinarum, preventing the accumulation of anisodamine and scopolamine in these plants. We show here that a deliberate Gly220Cys mutation in the Datura innoxia DiH6H protein caused a loss of both its enzymatic abilities and rendered it unable to hydroxylate L-hyoscyamine into anisodamine and to epoxidate anisodamine into scopolamine. By using protein modeling based on an available crystal structure of H6H from Datura metel, we show how the Cys220 residue causes a steric interference in the active site cavity impairing the interaction of both substrates, hyoscyamine and anisodamine with the active site of the protein.le="font-family:Verdana;"> We also address the enantiomeric preference of DiH6H based on molecular modeling.

Camel contagious ecthyma （CCE） has been reported in Mongolia, Kenya, Somalia and Sudan. CCE is caused by a Parapoxvirus affecting young animals by producing lesions around the lips and nostrils. The generalized form of CCE is uncommon. The aim of this study was to submit the first report of contagious ecthyma in Iran and also to help clinician to diagnose this disease with heavy economic losses easier. In the paper, an outbreak of CCE in dromedary and bactrian camels in Qom province of Iran was described and clinical signs were observed in one camel herd in October 2009. Nodules and scabs from seven affected animals were collected for virus identification. Total extracted DNA was used for polymerase chain reaction （PCR） to amplify a fragment of Parapoxvirus B2l gene. Results showed that camel calves （n = 27） less than one year old and one male bactrian camel were affected （no adult female camels were found to be infected）. The prevalence of the disease in the herd, adult camels and camel calves was 30.33%, 1.5% and 100%, respectively. Affected animals showed the swelling of head with nodular lesion around the lips. It then developed to pustules and fissured crusts. Previous involvement with this disease, history of contact with sheep or goats, food resources and season all can have a role in epidemiology of the disease.

This paper explores the implications of applying automation, and a technological force in which computer systems can fulfill human tasks, in the asset management industry. The investigation explores a number of significant topics in which managers should begin contemplating, including workforce origination post automation and the primary skills necessary to facilitate augmentation, and how robot advisors could challenge an organisation’s value proposition. The investigation was centered on Jupiter Asset Management （jam） to support their preparations for automation, as well as to provide insight from the “grass roots”. Research centered on interviews with experienced individuals within automation and asset management. The first interviewee was Simon Crawford, a Fixed Income, Multi-Asset Performance, and Risk Manager. The second interviewee was Daniel Hulme, CEO of Satalia and Advisor to the UK Home Office. Of crucial importance to the success of this investigation’s data analysis before and after the commencement of interviews, was the use of an analytical pattern matching produce, which examined qualitative information. The findings identify that current entry level occupations with systematic and repetitive tasks in a fixed domain, will be automated. Placing a greater demand for analytical abilities in junior recruits as the cognitive understanding of what data represents is a weakness of Artificial Intelligence （AI） thus strengthening augmentation between employees and technology. Automated investment profilers known as robo advisors will challenge the value proposition of organisations, such as jam, which in time will need to be onboard with the technology to remain competitive within a growing millennial market. Vehicular Networks (VANET) are the largest real-life paradigm of ad hoc networks which aim to ensure road safety and enhance drivers’ comfort. In VANET, the vehicles communicate or collaborate with each other and with adjacent infrastructure by exchanging significant messages, such as road accident warnings, steep-curve ahead warnings or traffic jam warnings. However, this communication and other assets involved are subject to major threats and provide numerous opportunities for attackers to launch several attacks and compromise security and privacy of vehicular users. This paper reviews the cyber security in VANET and proposes an asset-based approach for VANET security. Firstly, it identifies relevant assets in VANET. Secondly, it provides a detailed taxonomy of vulnerabilities and threats on these assets, and, lastly, it classifies the possible attacks in VANET and critically evaluates them.
