AIM：To assess the proportion of refractive errors in the Mexican population that visited primary care optometry clinics in fourteen states of Mexico. METHODS：Refractive data from 676 856 patients aged 6 to 90 y were collected from optometry clinics in fourteen states of Mexico between 2014 and 2015. The refractive errors were classified by the spherical equivalent（SE）, as follows：sphere＋？ cylinder. Myopia（SE〉-0.50 D）, hyperopia（SE〉＋0.50 D）, emmetropia（-0.50≤SE≤＋0.50）, and astigmatism alone（cylinder≥-0.25 D）. A negative cylinder was selected as a ***：The proportion（95% confidence interval） among all of the subjects was hyperopia 21.0%（20.9-21.0）, emmetropia 40.7%（40.5-40.8）, myopia 24.8%（24.7-24.9） and astigmatism alone 13.5%（13.4-13.5）. Myopia was the most common refractive error and frequency seemed to increase among the young population（10 to 29 years old）, however, hyperopia increased among the aging population（40 to 79 years old）, and astigmatism alone showed a decreasing trend with age（6 to 90y; from 19.7% to 10.8%）. There was a relationship between age and all refractive errors（approximately 60%, aged 50 and older）. The proportion of any clinically important refractive error was higher in males（61.2%） than in females（58.3%; P〈0.0001）. From fourteen states that collected information, the proportion of refractive error showed variability in different geographical areas of ***：Myopia is the most common refractive error in the population studied. This study provides the first data on refractive error in Mexico. Further programs and studies must be developed to address the refractive errors needs of the Mexican population.

Acid Dissociation Constants and Related Thermodynamic Functions of Protonated 2,2-Bis(Hydroxymethyl)-2,2’,2”- Nitrilotriethanol (BIS-TRIS) from (278.15 to 328.15) K

Thermodynamic dissociation constants pKa of 2,2-bis(hydroxymethyl)-2,2’,2”-nitrilotriethanol have been determined at 12 temperatures from (278.15 to 328.15) K including the body temperature 310.15 K by the electromotive-force measurements (emf) of hydrogen-silver chloride cells without liquid junction of the type: Pt(s), H2(g), 101.325 kPa|BIS-TRIS (m) + BIS-TRIS·HCl (m)| AgCl(s), Ag(s), where m denotes molality. The pKa values for the dissociation process of BIS-TRIS·H++ H2O = H3O+ + BIS-TRIS given as a function of T in Kelvin (K) by the equation pKa = 921.66 (K/T) + 14.0007-1.86197 ln(T/K). At 298.15 and 310.15 K, the values of pKa for BIS-TRIS were found to be 6.4828 ± 0.0005 and 6.2906 ± 0.0006 respectively. Thus buffer solutions composed of BIS-TRIS and its hydrochloride would be useful as secondary pH buffer standards and for control of acidity in the pH range 6 to 8. At 298.15 K the thermodynamic functions G°, H°, S° and Cp° for the dissociation process of BIS-TRIS·H+ are G°=37,005 J·mol-1, H° = 28,273 J·mol-1, S°= 29.3 J·K-1·mol-1 and Cp° = 36 J·K-1·mol-1. These results are compared with the dissociation of protonated bases structurally related to BIS-TRIS·H+.

Temperature Inversions,Meteorological Variables and Air Pollutants and Their Influence on Acute Respiratory Disease in the Guadalajara Metropolitan Zone,Jalisco,Mexico

The presence of temperature inversions (TI), concentration of air pollutants (AP) and meteorological variables (MV) affect the welfare of the population, creating public health problems (acute respiratory diseases ARDs, among others). The Guadalajara Metropolitan Zone (GMZ) experiences high levels of air pollution, which associated with the presence of temperature inversions and meteorological variations is conducive to the incidence of ARDs in children. The aim of this work is to evaluate the TI, MV, AP and their influence on the ARDs in children under five years in the GMZ from 2003 to 2007. In this period, the moderate and strong TI are the most frequent presenting from November to May. The AP shows a variable behavior during the year and between years, with the highest concentration of particles less than 10 microns (PM10), followed by ozone (O3), nitrogen dioxide (NO2), nitrogen oxides (NOX), carbon monoxide (CO) and sulfur dioxide (SO2), the most affected areas are the southeast of the GMZ. Annual arithmetic mean is 213,510 ± 41,209 ARDs consultations. The most important diseases are acute respiratory infections (98.0%), followed by pneumonia and bronchopneumonia (1.1%), asthma and status asthmaticus (0.5%) and streptococcal pharyngitis and tonsillitis (0.4%). Months with most inquiries were from October to March, mainly in the southeast, south and center of the city, coinciding with high levels of AP. Statistical analysis shows that the TI have significant correlation with ARDs in three years, temperature (Temp) in two, relative humidity (RH) in two, wind speed (WS) in three, wind direction (WD) in two, while that air pollutants NOX and NO2 showed significant correlation with ARDs throughout the period. CO and SO2 showed significance in two years, while the PM10 and O3 in one.

To editor:Cesarean scar pregnancy(CSP)is a rare pathology,with an increasingly clear association with morbidly adherent placenta(MAP).1 Although the most recommended treatment is pregnancy termination by cesarean section and scar resection shortly after diagnosis.2 The final decision regarding management depends on the patient's choice,medical advice,the fertility desire,and the social,religious and emotional background that should also be taken into account.

Dichloro-Diphenyl-Trichloroethane (DDT) and Endosulfan in Sediments of Sabancuy Lagoon, Campeche, Mexico

DDT and Endosulfan are persistent organic compounds which have been prohibited and their use has been limited in agricultural activities due to their toxicity, persistence and bioaccumulation potential. The use of DDT in fumigation campaigns against endemic diseases transmitted by vectors of malaria and dengue was prohibited in Carmen Island, Campeche, Mexico since 1995. In the surroundings of Sabancuy Lagoon, many and diverse agricultural activities are carried out and it is a common practice to use organ-chlorine pesticides as endosulfan. In order to know the levels of these compounds in this area, a sediment sampling campaign was carried out in ten sampling points randomly located within the Sabancuy Lagoon, Campeche and considering one reference sampling point located 2 km away from Sabancuy Lagoon within the Mexican Gulf. Three metabolites were identified in sediments samples belonging to DDT’s family: (4,4’-DDD;4,4’-DDE;4,4’- DDT) and Endosulfan’s family (Endosulfan I, Endosulfan II y Endosulfan sulfate). Higher concentrations in DDT’s family were found for 4,4’-DDD, followed by 4,4’-DDE and 4,4’-DDT. Endosulfan’s family, showed the following order for concentrations: Endosulfan II > Endosulfan I > Endosulfan sulfate. Mean concentration for S-DDT and S-Endosulfan in Sabancuy Lagoon sediments was twice higher than those found in other studies carried out in Términos Lagoon, Campeche, Mexico in 2002.

Extrapancreatic malignancies and intraductal papillary mucinous neoplasms of the pancreas

Over the last two decades multiple studies have demonstrated an increased incidence of additional malignancies in patients with intraductal papillary mucinous neoplasms(IPMNs).Additional malignancies have been identified in 10%-52% of patients with *** majority of these additional cancers occur before or concurrent with the diagnosis of *** gastrointestinal tract is most commonly involved in secondary malignancies,with benign colon polyps and colon cancer commonly seen in western countries and gastric cancer commonly seen in Asian *** extrapancreatic malignancies associated with IPMNs include benign and malignant esophageal neoplasms,gastrointestinal stromal tumors,carcinoid tumors,hepatobiliary cancers,breast cancers,prostate cancers,and lung *** is no clear etiology for the development of secondary malignancies in patients with *** population-based studies have shown different results from single institution studies regarding the exact incidence of additional primary cancers in IPMN patients,both have reached the same conclusion:there is a higher incidence of extrapancreatic malignancies in patients with IPMNs than in the general *** f inding has signif icant clinical implications for both the initial evaluation and the subsequent long-term followup of patients with *** a patient has not had recent colonoscopy,this should be performed during the evaluation of a newly diagnosed *** endoscopy should be performed in patients from Asian countries or for those who present with symptoms suggestive of upper gastrointestinal *** screening studies(breast and prostate) should be carried out as currently recommended for patient's age both before and after the diagnosis of IPMN.

Prevention of Postoperative Nausea and Vomiting in Elective Hysterectomy: A Prospective, Randomized, Placebo Controlled Outcomes Trial of Aprepitant NK-1-Receptor Antagonist

Objectives: Current prophylactic interventions fail to completely eliminate postoperative nausea and vomiting (PONV) for a substantial number of patients. A new antiemetic (aprepitant) has been effective in preventing chemotherapy induced nausea and vomiting (CINV). We hypothesized that adding aprepitant to our current prophylactic regimen of dexamethasone and ondansetron would reduce the incidence of PONV in our elective hysterectomy population. Methods: 256 patients undergoing elective hysterectomy were enrolled in this prospective, randomized, double blinded, placebo controlled trial. Subjects received either oral aprepitant 40 mg or oral placebo 30 minutes prior to induction of standardized anesthesia. The primary outcome was vomiting within the first 24 hours after surgery. Postoperative nausea, vomiting, and use of rescue antiemetics were documented over a 24 h period. Additionally, adverse events, hospitalization days, and readmissions for PONV were compared. Results: There was a trend towards reduction of postoperative nausea and vomiting in the aprepitant group. Nausea and vomiting were noted for 24% and 17% of women in the aprepitant group versus 38% and 29% of women in the Placebo group, respectively. Supplemental antiemetic medication was used by 42% of women in the aprepitant group versus 60% of women in the Placebo group. No adverse events were substantially more common in the aprepitant group than the Placebo group. Conclusions: Preemptive use of aprepitant prior to elective hysterectomy may reduce the incidence of PONV and diminish the need for rescue antiemetics postoperatively. Further studies with larger power are needed to confirm the trends observed in this study.
